Thanks to Drs Maggie Watson and Greg Lof for providing this evidence-based Q&A on the problems with using nonspeech oral motor exercises. Dr. Cate Crowley, Dr. Erika Levy and Teachers College Columbia University graduate students, and David Yanowitch created this video to share this information as widely as possible. Almost all of the text is from a document Maggie Watson and Greg Lof presented at the American Speech-Language and Hearing Association.
Please share widely. These approaches are simply not evidence-based and waste valuable time and money for goals related to speech!

I've taken my daughter to several SLPs since only a few weeks old due to feeding troubles with cleft lip and palate. She's 18 months old now. I've heard most if not all of these statements from our therapists. She's speech delayed, and they're still focused on feeding, ie chewing, drinking from a straw and cup. While she could benefit from eating a little better I suppose, my main concern is her lack of speech development. She was also diagnosed with a genetic condition that has speech problems. So, I'm a little frustrated at the moment because it seems like no one is helping her develop speech and language besides her family at home who are always talking, signing, playing with her.

Most important now is to stay connected to your cleft and craniofacial team where you will get recommendations from experts who know your child and know cleft speech and feeding and what your child needs. Of course, proper nutrition and feeding is critical. At 18 months old, her palate is likely repaired. Sometimes it is hard to find an SLP who has cleft speech expertise but there will be an cleft SLP on the team who can guide you. What your family is doing is right on for her language delays. She should also be able to receive services through your State's early intervention services. She would have to be evaluated to see if she qualifies. Here is a link for a free course on cleft speech and feeding that can provide you with some information. www.leadersproject.org/ceu-courses-2/english-cleft-palate-speech-therapy-evaluation-and-treatment-asha-0-5-ceu-self-study-course/

As an slp i believe and have very postive outcomes using oral motor in speech and swallowing therapy. There are many different oral motor exercises that I use to promote change. Oral motor will not improve language but can help with accuracy of productuction of vowel and consonant sounds. Some examples of programs that facilitate speech clarity is Beckman Oral Motor, MNRI Oral faciel program and more. When such exercises are done in conjunction with conventional articulation exercises, I've seen significant improvement in speech clarity.

Simply saying that these nonspeech oral motor exercises work in your clinical opinion is just that--an opinion that is contrary to the evidence. Too many SLPs/SLTs/fonos/logopedas use nonspeech oral motor exercises to improve speech and resulting in children continuing to speak often with greatly reduced intelligibility. Here are some scientific peer-reviewed articles showing that these nonspeech exercises do not work. Parra-López, P.; Olmos-Soria, M.; Valero-García, A.V. Nonverbal Oro-Motor Exercises: Do They Really Work for Phonoarticulatory Difficulties? Int. J. Environ. Res. Public Health 2022, 19, 5459. doi.org/10.3390/ijerph19095459 (open access). Ruscello, D. M., & Vallino, L. D. (2020). The use of nonspeech oral motor exercises in the treatment of children with cleft palate: A re-examination of available evidence. American Journal of Speech-Language Pathology, 29(4), 1811-1820. doi.org/10.1044/2020_ajslp-20-00087. Here is an open access article that is not peer reviewed but provides information for those who do not have access to copyrighted journals. What it says is consistent with the research as the author did a review of the current scientific research for the article. Angela (Dixon) Frazer, July 2022.
