Local anesthetic systemic toxicity (LAST) is a rare but potentially life-threatening condition that can occur when local anesthetics like lidocaine or bupivacaine are administered. LAST happens when the local anesthetic is absorbed into the bloodstream in large amounts, causing toxic reactions throughout the body. This can lead to symptoms ranging from mild, such as dizziness and confusion, to severe, including seizures, cardiac arrest, and death.
Plastic surgery and liposuction procedures often involve the use of local anesthetics to numb the targeted area and reduce pain during the surgery. While these anesthetics are generally safe when used in appropriate doses, they can still pose a risk of LAST. This risk is particularly high in liposuction procedures, where large amounts of local anesthetics are often used to numb large areas of the body.
One way to treat LAST is through a technique called lipid rescue. Lipid rescue involves the administration of a lipid emulsion, like Intralipid, to help bind the local anesthetic in the bloodstream and prevent it from causing further harm. Intralipid is a type of lipid emulsion that is commonly used for this purpose.
Intralipid works by acting as a lipid sink, which means that it absorbs the local anesthetic and keeps it away from the organs and tissues that could be damaged by it. When administered correctly, Intralipid can reverse the symptoms of LAST within minutes. This makes it a critical tool in the treatment of LAST and can be life-saving in situations where LAST is not recognized immediately.
The importance of having an experienced anesthesiologist present during cosmetic procedures like plastic surgery and liposuction cannot be overstated. An experienced anesthesiologist can monitor the patient's vital signs and adjust the dosage of local anesthesia as needed to prevent LAST from occurring. They can also recognize the symptoms of LAST and act quickly to administer Intralipid or other interventions.
In addition to the risk of LAST, plastic surgery and liposuction procedures also carry other potential side effects and risks. These can include bleeding, infection, nerve damage, scarring, and even death. It's important for patients to be aware of these risks and to have realistic expectations about the results of the procedure.
Patients should also be informed about the potential side effects of the medications used during the procedure, such as propofol, which is commonly used for sedation during plastic surgery. Propofol can cause a range of side effects, including respiratory depression, hypotension, and even cardiac arrest in rare cases. That’s so scary and sad! I would never do surgery on myself, I have had 14 surgeries because of my cerebral palsy, I really enjoy donating to the anesthesiologist Foundation, since it was hard for me to go to college because of my cerebral palsy , I really wanted to be a anesthesiologist! I think it takes special people and skill to be a anesthesiologist! And you’re one of them! I’m one of them too! You rock man

I've done my own minor dental surgeries. I've known people who've stitched their own wounds. Relatively safe and necessary, but anything else seems like a bad idea.😬

I knew a doctor that passed about 8 years ago, doing her own after hours liposuction by herself! It was a horrible event that traumatized her family and the responding officers.

I'd love to watch a video on PONV. Some people go through postoperative period smoothly, others vomit like crazy. When my son was to undergo general anaesthesia before cardiac surgery I asked anaesthetist to maybe add an antiemetic to make the first 24 hrs post-op easier for him. Doctor smiled and said "I'll make the right cocktail for him, no worries". My son was the only child who didn't suffer from PONV afterwards. All the other children on the ward had really hard time. What can an anaesthetist do to help in this matter? My mother had cardiac surgery 3 weeks ago and she did the same. She asked her anaesthetist for antiemetic so when she woke up she didn't feel any discomfort as far as nausea is concerned. Why isn't it a routine procedure for all patients? I'm writing from Poland so I'm not sure how things work in other countries, though.

I didn't know you could overdose on lidocaine. Makes me wonder if that ever happens at the dentist. I had to get a tooth pulled and they had to give me like 4 shots because I still had feeling after the first 2. I started feeling shaky and lightheaded although he said that was from the epinephrine it was mixed with. They didn't give me anything else besides those numbing shots and I didn't have an IV for him to give me that white stuff if something went wrong. I don't know if dentists even stock that stuff. And I still felt pain while he was pulling it.

Lidocaine can cause the blood to be unable to carry oxygen. I learned that after I had put some in capsules to swallow. Bad idea. A good dentist knows how to hit the right spot with the needle, but finding one that can seems rare. I've only ever found one and wow! Never felt any pain at all, not even afterwards. I almost feel asleep in the chair from the pain relief, and then heard his voice," O.K., you're good to go". What? Done? Never felt a thing.

In the past 32 years, I already have 16 surgery and I give Thank you to Jesus and all the Dr taking good care of me. I don't have any health issues I just had my left knee replacement and I'm doing good no issue either Thank you Jesus and thank you to all the teams of Dr takings care of me. My understanding is Liposuction only targets subcutaneous fat. It doesn't get rid of the visceral fat.

I had a bronchoscopy a few months ago. I don't know what they gave me but I was sick for 4 days after. Part of it was fentanyl . Headache , fatigued , just feeling ill. They said you normally don't feel well for 24hrs after. This was bad. Is that normal.

I nearly died from this at the dentist. Every pregnancy leeches calcium from the mother's bones for the baby. Years of being on Depo had weakened mine to the point I had undiagnosed Osteopena by the time I had my 3rd child. I always have completely natural childbirth, but they still insist on an I.V. anyway. After I had her, the nurse, for some reason, decided I needed my I.V. flushed. When she pushed the saline through my entire body got so cold my teeth began to chatter resulting in every tooth breaking. I was chattering so hard that pieces of my teeth were dropping out of my mouth & nobody could understand I was asking for a rolled washcloth to protect my teeth. When I went to the dentist, he said there was nothing left to save & I needed all my teeth pulled at age 35. I was devastated. We scheduled my appointment & he planned on pulling them all right there in the office even though I raised concerns because I do not numb to locals. Never have. I explained that even for all 3 breast biopsies & chip implant surgeries I've had I felt the ENTIRE procedure & they wouldn't believe me either. That resulted in what I now know is medical trauma. This dentist promised if I felt anything he would hit it again. I told him I wasn't numb when he started & he gave me another injection & waited 5 minutes. I still felt it. He started to work anyway & had to stop about every so often & try to numb me again. When he got to my eye teeth there was NO WAY. I had had more than enough by then. He had to try something else. So he tried a different nerve block but I still felt it. 10 injections total & he'd pushed me to the point of Novacain Toxicity.

I went into a simple hernia surgery and I got screwed. After surgery when I woke up I had pain on my right arm where they put the IV. I had ringing in the ears. Three days later I had my pinky going up and down without I trying to do it. I had some weird hand pain on both sides. Two weeks later started to have a major headache, nausea, muscle paint, joint pain, dizziness. Than 4 months later started to get walking issues, uncoordinated hand movement's and throat swallowing issues. I seen alot of specialist and nobody knows what happened during the surgery. The Doctor and the anesthesiologist do not want to take my calls. Just be careful with surgeries. I had another surgery a year later which I had the best Doctor and anesthesiologist. The anesthesiologist was the awesome. He was there asking me alot of questions and was there for me. Best anesthesiologist. He works in Sinai Cedar. Best surgery. The other one I will not mention where I had it done but please research the surgery and ask for a decent anesthesiologist. I am not able to work or perform regular household chores.

@@caroldurand6803 I honestly had a freak accident at the end of May and hit a deer on the highway, went to the hospital and they found pulmonary embolism! That deer saved my life. I’m on blood thinners now until November so the surgery had to be postponed until December. I’m still planning on the Lopo-fat transfer. I just haven’t had the surgery yet! God saved my life with a deer!

I had emergency appendicitis surgery years ago. The doctor put in discharge papers for me to stop taking xanax. I took it because I was caring for my paralyzed husband that had a stroke. My question: why would the doctor put in his report I stop xanax? Something happened during surgery possibly? I did stop taking xanax btw.

A lesbian friend of mine proposed marriage to her male surgeon! She was already married to a woman. It was so funny. But also I think that there is almost a child-like vulnerability when we entrust our lives to an anesthesiologist, surgeon and team, and it comes out when we are waking up from the anesthesia. The flirting comes from a feeling of strong attachment to the person who made sure that we came out of surgery alive!!

Liposuction??? Vain and/or unrealistic body shape expectations. Body dysmorphia. Im sincerely 😢 sad this happened. Unfortunately, I can't help find the situation funny 😂. Im thinking of the court scenes in Legally Blonde. The fitness guru's secret that became her alibi. People lay off the ridiculous cosmetic surgeries you probably don't need! If you're still fixated to have a cosmetic surgery, take it seriously please.

Not sure where you got your liposuction under general anesthesia (GA) information from. Plastic surgeons do prefer GA for liposuction compared to dermatologists who prefer to perform liposuction under tumescent anesthesia. There was a big debate about relative safety of GA vs tumescent anesthesia in Florida. Here is the paper for. the 4 year report... Coldiron B, Fisher AH, Adelman E, et al: Adverse event reporting: lessons learned from 4 years of Florida office data. Dermatol Surg 2005;31:1079-92. doi 10.1111/j.1524-4725.2005.31901 The finding was that plastic surgeons performing liposuction under GA had more deaths than dermatologists. FWIW, all the hundreds of liposuction cases I was involved with for cosmetic surgeons were performed with propofol sedation followed by 50 mg IV ketamine then tumescent anesthesia injection between 1992-1998. Then, brain monitoring of propofol from 1998--2018. GA was never needed & bupivacaine was never used. In fact, one San Diego dermatologist did succeed in killing a patient when she substituted bupivacaine for lidocaine in the tumescent solution. .
