The psoas does not cause an anterior pelvic tilt! It is a victim, not a culprit.
This means that any work done in attempting to address the anterior tilt of the pelvis is bound to fall short unless you address your ability to breathe with your left diaphragm.
The influence of the larger right diaphragm is what causes an anterior pelvic tilt. This video explains why.

I’ve never heard of this anywhere, after years of trying to sort my anterior pelvic tilt + scoliosis + hip tilt (to the right), i’ve always known my breathing to be inhibited but i never took proper measures to improve my breathing. My physio was not helping and i self-diagnosed as having a leg length imbalance because it seemed like my posture corrected itself with a heel lift, but i’m still in anterior etc IM GOING TO TRY THIS

This makes a lot of sense and I think my imbalances stem from my unconscious tendency to suck in my stomach which I've likely been doing since childhood. There is the factor that you bring up of one diaphragm being bigger than the other, and on top of this, perhaps I was pulling in harder on one side than the other, exacerbating the imbalance-not sure. I am making a conscious effort now to breath through the diaphragm when I walk now in order to fight this unconscious tendency.

Thanks Neal- This is the video that made the penny drop for me, I was getting a bit lost in all the other great videos youve posted but this brought me back to the primary focus on breathing I need to start with.

I watched this video a few months ago and didn't pay much attention because I have always felt that my right psoas is so much more active and tight then my left. I just watched it again and it makes so much sense that the diaphragm is the main driver for pelvic rotation and tilt. You chose not to go over the right psoas positioning in detail in this video but it would be awesome if you did!

Absolutely right...broke my left leg in a bike accident and for awhile i figured the atp was because of a slight length or tension difference. But soon realized my breathing definitely had a part to play but could never put my finger on it....You are one smart dude

We talked in the comments on your last video about keeping the ribs internally rotated on the left hand side on the exhalation and expanding the ribs on the right on inhalation. The thing that confuses me, is that you mentioned breathing better into the left diaphragm , which makes sense. But doesn't that mean we would be better concentrating on feeling the breath in the left diaphragm as opposed to in the ribs on the right? I am not sure if that's a stupid question or if it makes sense what I am saying.

It's a good question. It's a matter of what you can actually sense. I'm not sure people will "feel" their left diaphragm working. However they can "feel" their right ribcage "opening" with inhalation. If the right ribcage opens, by default it means the left diaphragm is positioned properly to pump. Similarly, if we "feel" our posterior ribs expand, or move back, with inhalation, then we know the left diaphragm is positioned properly to pump. Basically, if you can "feel" expansion of your right ribcage and left posterior ribcage with inhalation, your diaphragm can pump because your pelvis and ribcage are positioned properly to allow the pumping to occur. Is this where the concept of biogenetics comes in? Elliot Hulse used to lean backwards over a stool with a pad on it, stretching his anterior chain, and breathed deeply into his lower abdomen. He swore by it. I wonder if it’s connected

I suffer from hard chronic back pain for almost 8 years trying to strengthen my legs with sitting squat/splits and strech every day almost and nothing helps... meanwhile all this years I felt like something in my belly and breathing is "stuck" ! could this be the concept of what you ralking about?? could my chronic pain which therapist said strech and train can be connected to me breathing wrong my whole life???

I kid you not I have apt and this explains my situation to a T. My left hamstring cramps often and my adductor is always in pain. It extends to my left glute. Nice analysis 👍🏽

I'm having right pelvic tilt, and it's jammed up, how do I find someone who can actually fix this? It's causing severe foot pain in my heel. I've gotten it to loosen up some, but so far 3 chiropractors totally missed this and I need it fixed. I can feel the heel pain when I breathe sometimes so that makes sense. I have a physically demanding job and I'm sick of the pain!

Since the psoas connects to the posterior portion of the diaphragm, I can see how it may bias that portion to be more active, tight, descended than the anterior portion. Does that make sense?

As a yoga instructor, these PRI techniques provide a major missing link on how to get the diaphragm moving properly and addressing some very common tight and weak spots I see with myself and my students. I have a question: is it common for the imbalances you describe to be reinforced or at least continued during sleep? I often find myself waking up with very tense hip flexors and thoracic spine as was wondering if this is related to left AIC right BC pattern?

Many people breathe through their mouth, or grind/clench their teeth. Both of these promote overactive neck musculature, which will certainly cause tension further down the line. So sleeping isn't the issue, it's usually the non-conscious behaviors that occur during sleep that are the issue.

Hi Neal, thank you so much for all your videos. I've been seeing a PRI expert in my home state of California and I present symptoms of the standard Left AIC diagnostic pattern for the most part, but my primary complaints were actually right hip flexor pain and right knee pain which cause me some confusion when watching this video. Do you have any reasons you could see the right psoas and knee acting up so much more than the left?

Yes. Right psoas is often overused on the right side as a stabilizing muscle. Since you are always on the right side, it's put in a position that it's overworking to stabilize the whole right hip/femur complex. It could also get overworked attempting to externally rotate the right femur (but never accomplishing the task. A common compensation for the typically internally rotated right femur is a tibia that externally rotates. So the right knee can be experiencing torque due to the upper and lower leg orienting in opposite directions.

It’s not just NYers, don’t worry! I think Neal’s referring to how daily stress (city-living or otherwise) can push our bodies from a unilateral APT into a bilateral APT. I live in the French countryside but have a stressful job, and I developed bilateral APT (I hadn’t even been aware that beforehand I was unilateral ATP). I started with Neal’s example beginner exercises on his website - but first of all had to get out of the right-sided APT before working on the left side. PRI calls this having a bilateral PEC pattern - he has links on how to do this. Once a person gets out of PEC pattern you can then focus on getting out of the left AIC pattern (left APT) and getting your pelvis neutral. It’s a good starting place. If you can get help from a PRI certified specialist then go for it. I hazard to guess that currently more than 99% of non-PRI physiotherapists will have no idea about this. Good luck.

@@mogbob7336 Thanks for sharing your knowledge! This makes a lot of sense. I suspect I'm bilateral PEC as you said but I'm wondering if I might actually be patho PEC. I am going to do some home tests i guess to attempt to find out before committing to working with a PRI certified specialist since I'm in between jobs at the moment. I am hyper mobile, joint wise as I have ADHD and this is common with Neurodivergent folks, so I wonder if that could affect my results, as I've been told before by physical therapists they're surprised how much ROM and mobility I have for being so out of shape and obese!

Dear Neal ! My daghter have mild idiopathic "S" type scoliosis (pelvis rotation too). What do you think is the reason of scoliosis ? Is it idiophatic ? What kind of ecercise do you suggest ? (I think it isn't side-to-side issue, it is rotational issue and the culprit is PSOAS. If we strenghten or strech PSOAS it rotate the pelvis)

The right psoas can be tight. That's normal. It's probably being used to help stabilize your right hip. It can only relax once you teach your brain to use the left side of your body correctly.

Aaron Shafer it’s an underlying pattern of right sided dominance (due to organ placement, bigger right diaphragm, neurological function, dominant right peripheral vision etc..) that makes us underuse the left side of the body/diaphragm

I have question about something else. How does our pattern affect the pelvic floor and obturator internus from one side to the other. Would the left side of pelvic floor be more contracted as well as left obturator internus? Cause thus is what I have. Thanks

The left side of the pelvis floor would be in a descended position in a left AIC pattern. Probably tight. Not sure about the obturator. Can't recall which side is lengthened and which side is shortened. I can look it up.

@@JazzyScat From my manual: left obturator is short, straight, and weak due to anteriorly rotated pelvis. on the right: long, angled, and weak due to posterior rotation of right pelvis. Generally obturator issues are taken care of in PRI programs through repositioning of the pelvis and then adductor exercises (on the left) and glute exercises (on the right) to put them back at their proper length

Neal Hallinan . I did a lot exercises to fix that with o result. My right side is so tight, left is weak who rotate my femur interior caused by tight hip flexor. It is crazy.

Not really. The psoas is being put into an impossible situation and acting out as a result. It's not the culprit. It's being put into a position in which it is being told to work. So it's trying to work. It will do so until the pelvis can attain a new resting state of neutrality.

So essentially, one can completely correct their anterior pelvic tilt simply by establishing a more conscience left-sided diaphragmatic breathing pattern? How long would it take after initially implementing this change to start to see some dramatic and lasting results?

Anterior pelvic tilt doesn't exist in a vacuum. It's part of an underlying pattern of "extension" that can be caused by many different factors. So I can't really give you an answer. I recommend watching as many videos as you can to see how an anterior pelvic tilt is just a small part of a total body issue.

No, their testing comes up normally. In fact, lefties can often be more difficult to treat. Probably due to the fact that their brain isn't "normal" in the sense that 90% (roughly) of the world is right handed. PRI at its core is neurological. Muscle movement is just how our neurologic patterns express themselves in the physical realm.

@@NealHallinan hey Neal I'm left handed and my osteopath says my right pubic bone is anterior tilted. I've been working on the left and found relief but it's short lived even with orthotics. I get pain throughout the right and this includes PEC minor, lats, rib, psoas, TFL and groin. Any tips would be greatly appreciated. Thank you.

Kevin Lopes APT is just a physical manifestation of an underlying pattern. You also see compensatory deviations is the torso, arms, and legs.
