To anyone seeing this in the future; the plug-in is very useful and simple to use, but it is limited to 50 questions per month. If you want more you have to pay. But all-in-all a useful tool and a good video thank you mister.

Form builder is not entirely free. After the first 50 import, let's say 50 questions, you have to give up the free basic quota, and pay for further use.
