The fatal overdose epidemic in the US, which began in the 90s with increased overdoses on prescription opioids, finally looked like it was starting to take a turn in 2018. But then the covid pandemic hit, and amid increased isolation during lockdowns, fatal drug overdoses in the US skyrocketed, crossing 100,000 fatalities in a single year for the first time in 2021.
The main drivers of fatal overdoses over the last ten years are synthetic opioids, like fentanyl. Fentanyl is a highly potent narcotic that often gets mixed into the American drug supply to make drugs - ranging from heroin, to cocaine, to adderall - stronger and more addictive. The result is that huge numbers of people unknowingly consume a lethal dose of fentanyl laced into other drugs.
America’s “War on Drugs” and punitive approach to curbing drug use hasn’t solved this. So now American communities are turning to methods that emphasize “harm reduction” - creating environments for people with substance use disorder to find support, rather than punishment.
A big part of that is the distribution of the drug naloxone, also known as Narcan. Naloxone is an opioid-antagonist, and can temporarily reverse an opioid overdose as it’s happening. It does this by blocking opioid receptors in the brain.
Naloxone has been credited with saving tens of thousands of lives so far according to the CDC, and is a tool for fighting the ongoing overdose epidemic that some medical experts are now urging all Americans to have on hand.

I personally knew several people that OD'd on narcotics. Everyone of them got addicted by LEGAL painkillers given by a doctor, and everyone of them tried to find help once addicted. The same doctors that got them hooked were not interested in fixing the problems they caused. Harm reduction can be a worthwhile endeavor, but fixing the root cause of the epidemic will save far more lives.

There is a government initiative here in Australia where they did a trial of making it free, saw its benefits and now it’s free at participating pharmacies. I’m sorry it’s not free everywhere. It really should be.

It’s clear a lot of the people in the comments are thinking about this issue for the very first time, and that’s okay. But no one, including this video, is proposing Narcan (or harm reduction) as the *only* solution. This is a legit crisis and it’s going to take multiple different strategies to fix. This is one of them. It doesn’t mean the situation is okay, just that we have to recognize what it is.

When there's a crisis like that one, it may probably take more than just Narcan, it requires that people must acknowledge that fentnayl's really hurting people more than being isolated... But it may still work...

I see a lot of people in the comments basically complaining that it’s not enough but like…it’s something. Getting CPR certified won’t fix all the problems contributing to heart disease, and hopefully you’ll never have to use CPR, but it helps. Narcan is similar.

the problem is more so that Narcan as being seen as a solution in the first place, it isn't, it's a response. Chemotheraphy is a treatment for cancer, it does nothing to reduce rates, sunblock, better diets and etc are solutions. In this case there is just a hyperfocus on responding the issue without a clear strategy for treating the root.

@danycashking Science-Based treatment is slowly replacing abstinence and faith based programing here in Canada. We need both, even if traditional rehab programs have low rates of success. Like with some other health problems, there's nothing that works 100%, so we need options like groups, in patient and out patient programs, harm reduction, replacement therapies, and so on. If we want to address the root cause, then we'd need to educate people more on what causes dependency in a person outside of taking the substance. Like genetics, trauma, or/and mental health issues.

Pretty interesting that Narcan is available over the counter in the US. In Germany it’s only available via a doctors prescription, can only be prescribed to addicts and has to be picked up in a pharmacy. This makes it way less prevalent and impossible for the general public to carry it, but on the other hand opioid overdoses are magnitudes less common here.

In canada it's handed out by pharmacists with opiods. I was given one when I was prescribed morphine for a kidney stone. Downside is it needs to be stored a fairly stable temperature so where I live I cannot keep it in the car... where it is most useful for me to keep.

Tips: - Always call 911 over a suspected OD - Carry more than 2 doses. Some people need 3+ Naloxone nasal spray doses in order to come back - Recommended to take a first aid course - The person recovering may be agitated once they come back from their OD. That's normal. Don't take it personally

my university provides Narcan free for students if you request it. I keep it on me on spring break (I live in FL) or at parties even though I don’t use drugs. I couldn’t handle losing someone I love if I know I could have prevented it. However, university staff has always treated me poorly when I requested it and I can see the pharmacy students whispering behind the counter. I imagine that discourages many from making use of these resources.

I notice in the comments that people are a bit uncomfortable with regular people having Naloxone on them. Thinking that governments and professionals should be tackling this. Remember that your community is only as strong as the individuals who comprise it, and who are bold enough to keep it healthy. In our lifetime, we will need to protect eachother from violence, disease, and environmental catastrophies. Though governments and professionals have an important role to play, in the here and now we need eachother to survive and thrive. We cannot substitute ourselves.

I think what people are trying to say is rather than give people Naloxone, why doesn't the government solve the root problem. Most addictions start from prescription medication prescribed by doctors. If the US government heavily regulates doctors and pharmaceutical companies, the issue would be resolved in no time. In kenya we're I'm from we really have overdoses to opioids coz the are really prescribed. If you stop the issue from where it begins, then it will never happen

I literally saw someone's life get saved this week with Narcan after an overdose on the street in SF. I instinctually went to help to see if they needed any naloxone and my friend who was with me was confused because they had never heard of it. I grew up in Canada where it was legalized in 2016 and I can personally say that these PSAs save lives

The War on Drugs made things worse, but this is not a uniquely American problem anymore. Just ask the EU, Poland, and a lot of central america countries going through the same thing. These drugs are unlike anything that has come before them.

This is incorrect. Purdue makes an IV version of a different molecule called Nalmefene - it is not Naloxone (which is the generic of Narcan) and it is not a nasal spray. And OPVEE is a nasal spray form of Nalmefene, but it is produced/sold by Indivior. Narcan is sold by Emergent Biosolutions.

its deeply disturbing that a video like this is made, where the basic premise is that people can be dying on the streets from drug overdose anytime, anywhere. I do get that adressing the opioid epidemic is not within the scope of this video, since it is purely about harm reduction but the implicit acceptance of the situation is just dystopian.

As a current EMT who has used naloxone dozens of times on people, I think it's important to keep in mind that althought it may reverse an active overdose the effect is only temporary and if the person has, well, taken a lot of drugs, they may go right back into an overdosed state so if you ever use narcan on somone PLEASE still either take them to a hopital or call 911

I'm not a user either, noticed the grocery store was giving kits way, Grabbed one for my backpack. even though it was free (as per signage) , the grocery store wanted me to pay $80 for the kit.

Having Narcan available in the streets for emergencies are a great idea, but in the same time, asking citizens and businesses to have Narcan on them is very sad. The local, state, and federal government should be the ones preventing as many people as possible from getting drugs in the first place, and helping those that have a substance abuse problem such as rehabilitation programs. Citizens shouldn't have the burden place on them to help out their communities from a huge and complicated drug problem. Carrying Narcan around is a bandaid solution.

would the citizens like the burden of not hitting pedestrians crossing the street with their suvs that make it so they cant see people on the very wide streets which encourage higher vehicle speed with no effective speed enforcement because those cameras "need to pay for themselves"? i bet they wouldnt like that either. you can blame the system all you want and/or you can choose to be very small part of the very likely multifactorial solution?

I wish we had Narcan nasal spray here in Canada. The widely available version of naloxone here is injectable. Let me tell you, when you have someone on the ground in front of you who is not breathing and turning blue while you're trying to draw up naloxone with a needle and syringe while your adrenaline is pumping and your hands are shaking is BRUTALLY HARD.

Na, they should know and be encouraging people to carry it. I'm in Canada, since Fentanyl we have the Good Samaritan Drug Overdose Act, which prohibits police from charging people for drug possession. They had to do it because people were just leaving other people to overdose. Plus no one is using nalaxone as a drug, as it puts someone into withdrawal. Hopefully the USA has or makes a similar law. Otherwise overdose death rates aren't going to change much.

I don't think that would pass probable cause. If they searched you after you explained it's an EDC item, similar to a gun is an EDC for them, you could have grounds for a 4th amendment lawsuit.

That's a good question and I'd like to know as well. I do know you can't hurt someone by giving them narcan and it not being the right treatment. It doesn't have adverse effects if you're wrong which is pretty cool.

They list symptoms in the video, and also the conditions around them might give you a clue. And as the commenter above said, there's no adverse effects for someone who doesn't have opioids in their system.

You don't have to be sure. You can't hurt them if they're not. Just make sure to call 911 as well, in case it's not an OD (and they probably need medical attention as well if it is)

Sad how many people push the blame for the lack of support towards this crisis onto others instead of trying to help be a part of the solution. It takes a community as a whole to tackle an issue this big. We are all a part of the community. Its time to vote, volunteer, and provide other forms of assistance.

As a non-USA citizen, this is shocking and speaks clearly of the authorities' incompetency on addressing the drug issue in this country. Yet, they blame it on countries which have a history on drug manufacturing but do not exhibit death rates as exorbitant as those.

A lot of the comments on this video are pretty hardline against second chances. It feels super desperate, some folks have fully given up on their community. The strange thing is how it's always phrased as an us vs them thing, like "people who do drugs" are in a separate category from regular people. I guess the video is assuming a basic understanding of how the opioid producers pushed stronger versions of their drugs on doctors and pharmacists, and also assumes the viewer understands that many normal people begin opiate use after an injury. Lots of commenters taking advantage of these details not being mentioned in the video to post comments trying to push away the scary reality. Not their problem, it's happening to those other people who use drugs.

@@Kiwibirdman1701 as someone who used to be in that category: we should have helped them before they gave up on themselves. For example, by investing in the mental health of children and teens, before they feel the need to turn to drugs. Or making sure that people who take opioids after an injury can't get addicted to them by close monitoring and not just continuing the prescription like it's nothing.

the problem with releasing this now is that naloxone isn't enough. it used to be when it was just opiates but now it's tranq dope and benzo dope. and naloxone only reverses the effects of opiates, not other drugs that decrease yr respiratory system.

I agree. But helping someone survive an overdose means increasing their chances at reaching recovery later in life, which would have been 0% if they died right then and there. If only the system would change and make recovery more easily accessible. But Narcan: wish I could get it in my country. Are you going to carry any yourself?

This is amazing idea and 4mg should be more then sufficient for such potent synthetic opioid. As EMT worker I usually see results with 2-3mg.

I think it can be a good idea for public institutions like schools and malls to stock this. However, I feel uncomfortable with the suggestion that everyone should be carrying this. Its unreasonable to place the burden of responsibility on unrelated people. While it’s something that can be encouraged, we should draw the line at expecting random strangers to carry live saving tools everyday. We don’t expect everyone to carry around epipens or know CPR. If the solution is to expect everyone to carry around narcan, that’s going a bit too far

we encourage people to learn CPR and get certified though. i don't do drugs, don't run in circles that do, but i also am in a downtown area a lot and work with the homeless population in my area several times a week. if i have the chance to give people another chance, why wouldn't i? same with CPR. you never know if someone is abusing, or if someone is going to have a heart attack. never hurts to be prepared for the literal worst of situations.

Anyone who gets Narcan please make sure you know when your Narcan expires. I believe it's 2 years from the manufacturer date. Check the date on the box and replace before the expiration date! Be safe everyone!

Narcan can still be effective beyond its expiration date. You should replace it as soon as possible, but never hesitate to use it even if it's well beyond its expiration date. Some studies have shown it can still be effective after 30 years!

I used to work in an ER (we have universal health care). When patients with opioid overdose come in, some doctors prefer close monitoring and supportive treatments, some doctors prefer naxolone. Both methods are safe for the patients when used correctly in a hospital. But naloxone is not very safe for the staff as patients are very upset about being waken up.

​@@cherie..cherry it's painful (all painkillers removed from their system instantly) and sends them into withdrawal symptoms of they are an addict. Most prefer not the nasal spray so you can use a little naloxone at a time instead of slamming it.

So you would carry Narcan on you at all times to help the random drug user, but no one would ever think to carry an epi-pen on them at all times for the random allergic reaction in dairy, nuts and so on, mainly in children. Just pointing that out, is all.

It just causes it to degrade slightly faster, but studies have shown that the extra degradation is so minor that it's likely to still be effective even past its expiration date - so don't hesitate to use out of date Narcan if there's no alternative available. Also, just remember if you do have to use it, since it immediately gets rid of the "high" the person overdosing might actually be angry at you before they realise you saved their life.
