The study using bodybuilders should set off alarms. Drug(s) use? Their weight? Etc.

I’m a clinical exercise physiologist and we don’t do any exercise with clients with the stage 2 levels of HTN prior to the session. If they don’t have any other significant risk factors for an aneurysm then given these levels of BP during exercise not causing a stroke, the sub maximal training might be safe for them. On another note, do you think those extreme acute BPs are causing any small tears in arterial walls? The arterial system is a hydraulic system so the pressure should be equal throughout the whole system, meaning the small coronary arteries have the same pressure spike as the main arteries.

Good questions. I don't think micro trauma is an issue in an otherwise healthy vessel or else we would be seeing more exercise related events but if a vessel was already diseased then I'd be more concerned about that.

My blood pressure and pulse get excessively high. I get a migraine that's throbbing and extremely dizzy and lack of balance if I don't have way more sodium than recommended. I've always been a person who tends to get dehydrated easily. My current diet model likely exacerbates that condition. I consume between 6 and 8 g of salt daily between my food and my water. I use what they call light salt which is a potassium chloride I believe and pink Himalayan salt and powdered magnesium in my water and I salt my food enough to taste the salt. I'm 51 years old 177 lb and 6 ft, and my my blood pressure ranges anywhere between 118/78 and 92/60 My resting poles, watching TV or just sitting down, reading or whatever between 60 and 68. My pulse while I'm sleeping ranges between 56 and 43. But if I don't add salt and other electrolytes to my water, I get extremely dehydrated and my pulse raises and my blood pressure raises a lot. I've had blood pressure. 160/99 in the ER when I was suffering with a throbbing migraine. Terrible dizziness and loss of balance. After a few IV bags and 30 or 40 minutes I felt normal again and my blood pressure dropped 40 points. I'm a very low carb diet for the last year and a half

mines always a bit low, today it was 115/52. I take 5mg ciallis daily as well as flomx/solafencin for bph

Is telsmisartan considered the safest arb long term with the least amount of side effects compared to losartan, valsartan, etc.? If u have high blood pressure, should u see a nephrologist or cardiologist? An ER dr mentioned nephrologist.

It's equally safe but has the longest half life and some additional properties that make it popular.

would you recommend athletes that train hard regularly to take vasodilators and if you do which VD`s would you recommend, the safest ones e.t.c for long term use with little to no side effects

Is there any preemptive Imaging diagnostic to detect intracranial aneurysms in healthy middle agers?

Routine screening isn't recommended for most people but we do CT angiograms and/or MRAs to pick them up in the ER.

Just curious based on what you shared about salt, what is your position on the observed dose response effect of dietary salt with elevation in blood pressure from the INTERSALT study of 1988 and some of the large more recent analyses also showing this relationship. More specifically, the 2022 analysis by Filippini et al reference below. Thanks in advance! Filippini, T., Malavolti, M., Whelton, P.K. et al. The short answer is...good study. Just confirms what we know in general but doesn't refute other studies showing variations in BP to a given salt load based on age and various genetic factors. There's people that say sodium intake has nothing to do with BP and the data doesnt support that. Its an important variable for most people.

How do you balance sodium for performance against the BP rise? I.e. if you are lifting and doing cardio you want electrolytes (salt) but then your BP rises some, in my case.

You don't need to supplement with sodium above your normal dietary intake for performance except under extreme circumstances.

Im recoveryn from sinus thromboises and killin myself in the gym 6 days a week. Should i be worried about these BP spikes? Im also on 2 BP medicines, and when I was at the hospital I had under 120/80 the last month (I had 180/100 when I was checked in). Have had high BP several years before I started the medication.

Started taking clonidine for ADHD mood support. Apparently slightly lowers blood pressure. Have not verified. Anyone familiar with its BP effects?

Clonidine has been around forever. It's a 4th line drug and causes rebound high blood pressure if you miss a dose so it's not very popular. It gets way more use these days for ADHD.

I have seen guys in the gym from time to time that have been pushing it on the weights and they blow a vessel in the muscle they are working on. They often have massive bruising and weakness for a few weeks to months. I suspect that is the same thing you are talking about.
