Colombia is no.2 in the world behind Thailand for sex tourism. It is foolish for locals and politicians to deny it or act against it when it brings big money into the country.
The problem goes both ways, the sex tourists and the Colombian women. I get propositioned for sex almost daily. They assume if you're in Medellin as a tourists you're willing to pay for it. If you don’t know any locals that can connect you with non sex workers, your options are very limited.

I miss Colombia. Spent 3 months there but the true beauty is exploring the country as I did on motorcycle. Hella dangerous but in the end it was amazing experience. Sad to see so much prostitution. It was crazy i’ll never forget walking in centro medellin during afternoon and if felt like there was unease and dark feeling in the air.. hard to explain. Like everyone watching you.
Would I never ever ever hear anyone ever talk about is what kind of Visa allows you to stay more than 3 to 6 months there? I know you can buy an apartment or real estate but who has an extra $150,000 to throw at that?

It is challenging to look for decent women in Medellin. Many of the locals will view you as a s*x tourist or s*xpat, and most decent women want to uphold their reputation and not be seen as a prepago, so they usually won't give you the time of day if you cold approach them. One of the ways I met normal people, including women, was going to Meetup events. If you're interested in doing language exchanges or hanging out with locals who work in your industry, that's a way to meet new people.
You mentioned the usual suspect cities in the US, and sir what is the common in those cities? A UN study stated that you take the aforementioned cities that raises the US into the top ten safest country!
