49:22 I like the presentation of the female speaker much better. This is should be the standard across the nation. Going to a medical provider is nerve wracking enough. If a patient is put on the defensive over their sexuality, that’s just going to amplify any anxiety they already had.

LGBT gets all lumped together out in the wild, but within the medical context being transgender or intersex are medical concerns. Someone’s sexual orientation is not. A straight man’s body parts and a gay man’s both work the same exact way. Same thing for any other cisgender person irrespective of who they’re attracted to. If a CB patient has an STI, the infection doesn’t care what the person’s orientation is. So neither should the provider. But transgender or intersex people have medical issues specific to that set of circumstances.

Of course, I support inclusivity, but me, personally if I had to walk on eggshells every day to do my job, I would quit that profession. I mean, you could still be a doctor, but just not dealing with any patients face to face.

39:12 I disagree with the ideas of trying to get the adolescent alone just on the spot like that. This idea should be tabled to parents separately. The doctor should not try to undermine the relationship between the parent and their children. Present the idea to the parents, and let _them_ be the ones to introduce it to their kids. That’s their job _and_ their _right._ Don’t try to wedge yourself in between them. That’s unethical and _not_ your job.

51:00 re: “*Brendon,* 24, has anal receptive sex.” I heard there’s no test for HPV in males. But maybe that only applies to heterosexual males? I guess, like a cervix, the doctor can collect cells from the rectum? 🤔

31:05 Only UNVACCINATED patients who have UNPROTECTED anal receptive sex are at risk for HPV infection at that site. And even if they suffered an infection, it doesn’t necessarily mean it will manifest into anal cancer.

To any practitioners who might check in on the comment section, some of us really don’t want to share ANY of this information whatsoever. Just tell us what we need to know, and WE’LL process it. We’re adults, and we don’t need sensitive details stored in a electronic medical record (EMR). It’s actually degrading , dehumanizing and humiliating for medical practitioners to try to pry such information from us. Those are OUR experiences, and we’re not sharing them with YOU! For example, why don’t YOU share all YOUR nasty behaviors FIRST, and then we’ll be on an equal footing. See? You wouldn’t like it if WE knew all the sordid stories about YOU. You would suddenly feel quite vulnerable. No. No. No. Some of us aren’t telling you ANYTHING until we know something about YOU. I had one male doctor for about several years. And would do just that, just let know something personal about himself. That was my little nugget of gold to carry in my pocket. And that allowed me to be open with him. I’m definitely not telling any doctors anything they don’t need to know. Some things we would rather forget. And if you ask about it and we tell you, then it’s not private anymore. Some people like a lot of external validation. Kids and young adults who haven’t achieved psychological autonomy, and maybe some traumatized patients. But don’t assume every patient will eventually open up.

Trans Health Fail… I called U of L Physicians Family Medicine to schedule a new patient appointment and they found my old information in their system. I gave her my name and current insurance info. When the appointment was booked I wanted to confirm with her that all traces of my old name were removed. She said she added my name as an aka and she wasn’t allowed to change the old name. I told her Megan was in fact my legal name and she’s not going to be able to bill insurance with the old name on there.. She said “oh, well the appointment is booked; I can’t do anything about it now, You’ll have to just tell them that when you come in for your first appointment.” #transhealthfail

Finasteride should never be used in MtF HRT. If testosterone is brought down to female levels, DHT levels will be insignificant and therefor Finasteride will not provide any real benefit to the pt. Instead, this drug will actually harm the patient. As a 5aR inhibitor, Finasteride also drops allopregnanolone levels as 5aR is the catalyst that converts progesterone to allo. This is what typically causes the depressive symptoms.

I've checked in with this speaker who also appreciates your wealth of knowledge. He is open to talking more with you about this and international guidelines that currently still recommend the estrogen plus an anti-androgen.
