Spinal stenosis really is a confusing topic, but it doesn’t have to be. Essentially it is when the nerves in the tunnel of the spine are getting squeezed!
-
There are many different reasons this could happen. The most common is due to arthritis that adds extra hard TISSUE in this area and squeezes the nerves. The other most common is a disc herniation that pushes out and squeezes the nerves. Just because you have a diagnosis of spinal stenosis does not at all mean it needs surgery, but it might.

Spinal stenosis can cause weakness, pain and paralysis- if it is severe. In this video, I will discuss the definition, basic anatomy, as well as the image studies were used to see it, as well as some of the newer Technologies that Spine Surgeons are using tonight only fix this condition, but to treat it forever at that location of the spine!

I fell in 2015 and had cages and screws to fuse l four and five. In six months the bad was as bad as before. Fast forward to 2023. My mri showed severe stenosis, a torn disc, two ruptured discs, facet disease, and a couple of other things. I don’t want another fusion. I am hopeful with this video that maybe in a few years I can get the latest and best surgery. I am a sixty seven year old female. I have a lot of pain, but I walk, push mow, rode a riding lawn mower, garden , clean house etc. I do take a mild pain pill four times a day. I hope with your help I can get help in the future. When it is cold, I am miserable. Thank you sooo much for this.

This was so interesting. I had surgery for spinal stenosis about 15 years ago when the surgeon inserted an IDD (intraspinal distraction device) between L3 and L4 vertebrae, which was very successful. However my symptoms of numbness/pain have now returned and are impacting on my mobility. I live in the UK and the surgeon I saw never mentioned any of these devices. Neither did I have a physical examination which surprised me, and I was interested to hear you thought that was important. I think this is due, in part, to funding of the NHS so maybe I will have to look into consulting someone privately. Your video has shown me how far techniques have advanced over the last ten years and I have a clearer idea of what is available. Thank you for posting this.

Government services do not use the latest technology. It cost too much and they don't care. When it comes to excellent health care, you have to pay for the best. I can understand your feelings and pain and wish you the best. Keep your weight in check, too.

Mine was cervical 3 level using cadaver bone graft wedges secured by a plate and screws...my neck was falling forward Best surgery ever Oh and the cost was approx 35k just for the surgery

You're not kidding. My mom has had a sudden severe walking problem after a fall on a padded carpet. And just because after a spinal tap and a theoretical temporary slight walking improvement, they want to opt for a shunt to drain fluid. But if it's spinal stenosis, then there probably won't be improvement. We should know for absolute certainty exactly where the problem exists that is causing the gait walking problem. Either the CSF (fluid) is slowed down by a pinch, OR a vertebrae is pinching the spinal cord. Now I have to go to the hospital to talk this through otherwise we're just theorizing with no guarantee of even a slight improvement. Normally, if it was csf slowed down by a pinch, then the shunt would be correct.

I have Lower Lumbar Spinal Stenosis and have tried to avoid surgery because I was warned that it is not always successful. I have had cortisone type injections but they are no longer effective. Your video gives me hope that surgery using modern technological advances might be an option for me. I wish you were my orthopaedic surgeon. Thank you very much for giving me hope.

I am a 71 year old engineer with spinal stenosis and I deal with it everyday. At least I'm retired but I do miss my active days. I used to get up before work in my forties and run four miles before work. Watching the surgical youtube videos freaks me out. And I know better than to think narcotics are the answer. So you are not alone out there! 🤔

This is a excellent overview of new technologies in fusion surgeries. I had a microdisectomy about a year and half ago but have continued issues from foraminal stenosis. After one more ESI I may have to decide on an offered TLIF surgery. This video gives me some more questions to ponder.

I'm 65 years young, and have 3 to 5 Herniated Disks in my back, Spine is curved, Spinal Stenosis, and other issues with my back. Doctors at Pain Clinic said get cortisone shots in my back. Would give me some relief. Explained that I've tried and it works for about 4 weeks. Also, the 4 th cortisone shot. Doctor hit a nerve ? It was such extreme pain. All I could do was pray and cry. I was told pain pills will not work, and getting anxiety just thinking of this shot again. Last option is back surgery. This issues started in 2007 and gotten severe enough. Thank you for your interesting video. I was just diagnosed with lower back stenosis and orthopedic doctor suggested having surgery to put pins in my back. My concern is that he told me he would go through my belly button. I told him I needed to do more research. I do suffer extremely painful lower back, buttocks side of my hip, down my left leg and foot now. The pain is more extreme at night and have a difficult time sleeping. Thank you very much.

Thank you for all your information I have arthritis in the cervical and lumbar facets and several years ago had multiple ablations for lumbar facet pain. I have lower back and SI pain all the time-I just had X-ray of painful left hip and was told I have hip displasia and mild arthritis. Dr. Said the lumbar spine showed severe arthritis with severe narrowing of the discs. I will be doing PT again for the hip pain -but what can be done for the back pain? Thank you

I thank you for your explanation. None of my Dr. Ever explained like the way you explained. I had spinal stenosis surgery in 2019. I was able to walk before the surgery. But after the surgery, I wasn't able to walk until today. Pain and burning sensation are getting worse than before. Now the want me to have neck spinal stenosis surgery. But for me, since I have been suffering for 4 years now, I decided to say no. This is enough. No need for surgery again.

Thank you for this! I have stenosis in my neck & lower back. Also my tailbone has issues. 13 bones/joints in total. I’m jumping through the insurance hoops atm. Which is terrible. No pT is going to help. No shot in the spine is going to fix it. But the insurance companies want to waste money, my time, and give me more pain 🙄 I hope my surgeon is as good as you seem. ✌️

I have Spondylosis, of course arthritis and disc compression. You said that these are disqualifying for disc replacement. But if you have the capability of smoothing the bone around the facet joints via the oscillating tool, why would the artificial disc not be able to be used? You could clean up the arthritis and re-center the spine. Would that stablize the spine and qualify for the disc replacement? I understand that the vertebrae is slipping, but would the artificial disc correct that also? Also, the surgeon i have consulted didn't mention lateral or front entry for a "larger" correction cage. Would this maybe a preference or maybe i don't need a larger correction since he is recommending fusion for my issues? Thanks

Thanks for all of the info I was diagnose Diffuse Disc bulge in L3 - L4 Grade 3 disc dessiccation in L4-L5 Grade 2 disc dessiccation in L5-S1 Then what they did is Epidural Steroid Injection As of now no changes happen. still I feel the weakness and numbness of my lowerback going to the feet

I just had back surgery but want to ask if once the Thecal Sac is damaged, can surgery repair the damage so that I can get control of my bladder again? I am told it takes a long time for nerves to heal. It has been 3months.

Wow, I learned a lot. I still believe I would still have had to have rods and screws put in, back in 2014, but I found some of the newer things very interesting. I do have a question after someone has the rods and screws done, how often should it be checked by the surgeon? I had myeomalasia in my thoracic spine. Also have had 5 fx since, all thoracic. Thanks for the new info., and how you explained it!

About 15 years ago I was diagnosed with prostate cancer. After asking questions, I opted for what was new back then: robotic laproscopic surgery. It was my introduction to the Intuitive's DaVinci Surgical System. At that time, it was a fairly new way to do surgeries, but these days it is quite common. I tell this only because the urologist who did the surgery was a very young guy. As I talked to him, he told me that most of the surgeons at that time who were doing robotic surgeries were younger guys. Why? Because 1) the younger guys grew up playing video games and were used to using computerized devices, and 2) the older guys were set in their ways and reluctant to put in the time to learn how to use the more modern machinerys. As I listen to Dr. Donnally, i hear him talking about these newer "tools" available to aid in spinal surgeries. Sure, these tools are super cool and all, but will the older surgeons be inclined to reject using them because they don't want to spend the time learning how to skillfully use them?

I have been told my herniated disc has begun calcification and is hardening while also compressing on my nerve roots in my lumbar spine. Does the ultrasonic bone shaving device work to eliminate this hardened disc material or does it only work on bone?

I so appreciate this video! Thank you so much Dr, Donnally lll! My back doctor wants to fuse my 5 discs in my lumbar spine. I have mild stenosis in t12 to L1, severe stenosis in L1 to L2, moderate stenosis in L2 to L5. Degenerative Facet Disease in T12 thru L5 . Grade 1 anterolisthesis of L1 to L4 with small circumferential disc bulge and unroofing of the disc. Moderate disc bulge in L1 to L2. Mild disc bulge in L2 thru L5. Are there any new procedures that could be done instead of fusing all 5?

Awesome presentation, I am only 29 years old and have to meet with a local surgeon next week and i am hoping these newer technologies will be offered as i have L3-4: There is disc desiccation with mild height loss. There is a broad right central protrusion. There is narrowing of the subarticular zones on the right more than left and moderate central canal stenosis. No significant neural foraminal stenosis. L4-5: There is disc desiccation with mild height loss. There is a large, broad right central disc protrusion. There is severe spinal canal stenosis with crowding of cauda equina and effacement of the subarachnoid space. The thecal sac measures 3 to 4 mm in diameter. No significant neural foraminal stenosis.

Do you take Medicare and AARP gap policy F Thanks for explanation of new technologies and how to do telemedicine with you . I have had Injections in my lumbar and cervical spine and Occipital Nerve injection with success .I do p t couple of times a year for flare ups. I have pain management Dr that keeps me active- gardening care of my home walking.He prescribes Narco -one week worth 4 tablets a day -I only use1/2 a tablet usually less then once a day. So my RX last a very long time. Retired RN-Orthopedic Being an active person at 77 is possible if I have my team of Dr Pain maintenance PT Keep moving Positive Vibes Posture is IMPORTANT Thank You

Hello,Dr Donnally fantastic video! Very nice animations and explanations. Dr have you heard about a situation called “back mice?” Like a hernia/lipoma that sits right above the SI joint that is a lump probably the size of a half dollar maybe that can be palpated and it moves under the skin. These press on nerves and it’s so painful. I’m hoping to have an orthopedic surgeon look at it and remove it. Any experience with Back Mice?

@@TakeTheRide that’s because lots of them have never heard of it. And god forbid they read and research some a bit after med school for the love of god. It’s basically a episacral fat herniation, subfascial herniated lipoma. Usually form over the iliac crest both sides. My left lipoma has pretty much paralyzed me in bed. The most debilitating pain I’ve ever had. I found a Dr. Plastic surgeon that’s familiar. I’m having it excised in a week. I’ve been begging to be seen earlier the pain is off the charts and pain meds don’t even touch it really. He’s out until next week.

I have multi level pathology from cervical through lumbar. Describing all of it would be far too time consuming. Had MRI series completely through cervical, thoracic, and lumbar. I'm covered by a Medicare Advantage plan in NJ. What would be your fee to review and consult after receiving copies of entire workup? I've been offered laminectomies at multiple levels. I won't do it. Wife's a retired Neurologist...seen too many post laminectomy/failed back syndrome cases. I'm in Prolia treatment for osteoporosis, so it will be at least a couple of years before corrective surgery is a practical option. What's your mailing address?

Dr. Donnally, how do you feel about the risks of Xlif or lateral surgery? I have spinal stenosis with spondy at L 4 5 and it was recommended to me by UPenn doctor to consider this. I read about many neurological risks that might happen, so I'm scared. Please advise. My other option is a regular fusion the traditional way.

Thoughts on the new barricade? Downsides? I have a herniated disc at l5 s1 and l4 l5 the latter is the severe one. Its been 1.5 years of pain in my right side back and butt a little but no leg pain. Only mild to mod DD. Likely a large annulus tear. Im 30, and I want to get back to weekend warrior baseball. Havent found a local surgeon who has used it so they say the dont like it.. Also under what conditions is the prema spine tops a good solution for people?

Oh also, my surgeon suggested my bulge is likely calcified so 80 percent success instead of 90 for me if i do microdissectomy. Looks semi dark to me on mri, do discs still reabsorb and or dehydrate and shrink after 1.5 years if i wait? Or is it usually going to spotaniously resolve withing that 6 to 8 week time frame or else it probably wont? Mri done 3 months after pain presented, repeated 1 month ago the bulge at l4 l5 got worse. Radiology report is labled sever stenosis clear nearve root contact but yet pain is not in my leg yet, just back and butt and feels weird when i step off a high step sometimes. So hard to navigate what to do and i just want to find a good surgeon that knows about the new stuff. Had a few consults in FL none of the orthopedic surgeons ever heard of partial tear smith and nephew collegen implant for supra spinatus repair. Raleigh area UNC department i had to ask for it but they heard of it and it saved me many screws and my shoulder is great now. Research saved me there. Partial tear dissapeared and i can throw again. Trying similar with back but im exausted at this point.

I had surgery for spinal stenosis 12 years ago. I had 2 tintanium rods inserted from L3 to S1. Now I have tingling and numness in my legs and feet. I have recently had a MRI and the results show I have 3 bulging discs at L1,2, & 3; compression fracture at T12 and spinal stenosis at S1. I have been referred to a neurosurgeon in Toronto, Ontario. I had a very difficult time 12 years ago. I do not want to have invasive surgery again. Is there a neurosurgeon in Toronto that does the type of surgery you have talked about ?

In June of 2018 i awoke at around 3 am, I told my wife that I had the worst pain in my right hip and running down my leg that I had ever experienced in my life. I got up and couldn’t get any relief. I went to urgent care as soon as they opened. I was diagnosed with spinal stenosis. Long story made short they orthopedic doctors wanted me to go to a different city telling me that I needed as many as four discs fused in my lumbar. Thankfully I found a neurosurgeon here in my city that got rid of my stenosis without doing all those fusions, I’m not sure how he did this but I’m thinking it might have been the hydroponic scalpel. My pain is completely gone and this was done in March of 2019. I’m so glad I found this Dr. My life would have been so altered if I would have had four discs died in my lower back.

Hi everyone , really would appreciate some insighful comments on this please .....I've had problems with my back since i was 17 years old I'm now 55 , i was always sporty but my diagnosis of spondylosis flew over my head 2 years ago ! I didnt ask questions that i should've, i was told that the stenosis is cervical and lumbar , over the last year my health has changed and i admit i starting thinking more about what if ??? I am wondering after having this for however many years undiagnosed because i just got on with work and life in general and accepted that exercise and pain meds went hand in hand , if i decided to have surgery and when the surgeon opens me up ....what kind of mess is it going to be ? I mean over the years i remember at least 8 times i couldn't walk or sleep in my bed even toilet properly for 2 or 3 months , i hope whatever mess my spine is in that it can be sorted with the least amount of fuss , i know there are some guys n ladies out there less fortunate and you fight the pain barrier every day , i am not 100% on going for surgery and argue with myself.

@@DonnallySpineConsult Mr Donnally , could I say that I am not looking for advice as I'm sure you will agree that this should come from my own medical professionals, merely your opinion on stenosis both cervical & lumbar , is prevention the best cure here ? What I mean is I've been having problems with passing urine were it can take me 3 to 4 minutes to urinate , the flow is poor although I do have BPH but not sure if it is to do with that or stenosis and also I have bad headaches 2 or 3 most weeks and wear a prism patch on my glasses because of double vision -diplopia and various other symptoms ......is spinal surgery the answer here ? in your opinion.

I was intubated and coma for 21 days learned to walk again and re gaining very slowly control of my hands. Neurosurgeon said infection got in my spine and recommending urgent spinal cord decompression with fusion surgery. What are your thoughts on infections? I’m going to get other opinions of course.

I had an L4/5 disk replacement done at Walter Reed in 2007. It never did give me the pain relief and mobility I had hoped. At the time, as I understand, these were still under research because I had to sign a lot of paperwork acknowledging this ( even though I was still on active duty in the Army). Is there anything I can have done to help with this? I have a spinal stimulator with the new burst technology and I’ve had every injection and nerve block known to man I think. I still take narcotic pain killers and the injection that seems to help the most is called a Codel and I’m sure that’s spelled incorrectly. Thank you for the video. Very informative.

can the MRI be without contrast? EMG shows L5-S1 damage but the MRI had in past show nothing with nerves. Gonna repeat MRI.

I have spinal stenosis and had surgery on a herniated disc in lower back, 5 days after surgery I sneezed and blew it out. They went back in and after that surgery , came out with 2 pinched nerves in lower back. Instead of trying to fix that they closed me up. Now I suffer from pain all the time, cant do anything now. They want to do an fushion. I dnt want any parts put in me. Is there some way of repairing it without the pins and rods?I also have compressed disc in neck. Which started becoming numb. Where are you out of ? And do you accept medicaid or medicare?

My Father went for a lumbar spine micro-desectomy surgery at the time of suture removal we observe infection at the surgical site infection and not cured by antibiotics hence re-open surgery once again is done but after 1month pain comes and get again spinal infection comes why and what is the reason of repeated infections
