In this video I review the ways the spine SURGEON helps guide the patient away from surgery until it is absolutely indicated!

I do about 15 new patient telemedicines every week. Here is the best path forward:
1- email your insurance card to Spine@TSCspine.com
2- if we take that insurance THEN upload your images (get the CD from image center) to my portal for our 1st visit
3- I must have X-rays and MRI, X rays are critical. Just MRI isn’t enough. Need to be from last 12 months. Nothing older is needed, or wanted.
4- You need to get your CD 💿 of your X ray and MRI (imaging centers will never send them to us)

regarding your editing. I was impressed by how quickly you covered all the material. it was 1:50 at the beginning of the video - you were able to squeeze it all in by 1:50. Either very very very fast or the filming took 12 hours i.e. 1:50pm to 1:50am. ya I know, green screen

Surgeries were on L5 and S1. I've had steroid shots and epidural, which help with pain. I have weakness, I stumble and sometimes fall.

L5 S1 radiculopathy here. I'm on week , 15 of PT. Pain in left foot only getting worse. Begging for shots or oral steroids. No NSAIDs allowed. Back issues for 50+ years. Best presentation I have ever seen Thank you,!

71 year old athlete in Tulsa. Sprinted in yard and tripped Jan 6. Many visits to 3 orthopedic surgeons and 2 MRI revealed torn hamstring and an extruded herniated disc in the L-5, S-1. Two months of excruciating sleep-depriving constant pain. My Primary Care DO referred me to the Pain Management Center. Yesterday morning while prepping for Epidural, the techs mentioned that "sometimes" if no relief people will schedule another treatment the next week. During procedure, the doc and her female techs maintained what seemed a super casual conversation about plans for St Paddy's day while I was excruciating for the few minutes of treatment. You mentioned at 10:24 "unethica pain management. When a tech plants the notion of another treatment the next week my red flag goes up. Your presentation is the best!!

i live near tulsa to and i can say all the doctors i have been to there are not my cup of tea i have seen dr harwell at spine and brain institute. not any doctors in okla for me that knows much.

I have spinal stenosis in L4and L5,diagnosed in 2018. Was consulted to a neurosurgeon by my neurologist but unfortunately up to now haven’t heard from him. It’s getting worse in fact I am using a walking cane as sometimes sudden buckling of my leg. I was finally seen by a pain specialist. With the treatment with medications it’s helping and am using a lumbar belt. The fortunate part of it is my right leg is good with no pain or any other symptoms and I can drive a car. By the way I also have seen a physiotherapist and am doing the exercises that was told to do it bid. Your video is quite excellent and I can fully understand my situation . Thank you.

Great information Doc. I'm a retired nurse. I went to pain management doc here in Tucson. The injections have greatly decreased my pain I have had two spinal surgeries. I have a fusion of S 3,4, and S1. Currently, I'm having aggressive Pt. I'm walking and have a deep appreciation of you caring docs.

Thank you for the clear and definitive explanation. EVERYTHING you discuss in this video had happened to me. It started with scoliosis, then enherniated discs, now severe, high-grade spinal stenosis. I have 3 bulging discs, spinal stenosis, and now the surgeon that I consulted in Georgetown says that without the cartilage, I have an upper disc that has permanently slipped below the lower one. He showed me the MRI and explained that at the lower discs, the nerves are not visible at all because of the severe compression. Not to mention degenerative disc disease. My conditions are also complicated by tearing both of my hamstrings and they didn't heal properly. My primary care doctor reviewed the MRI and said "WOW, that's an awful lot going on with your back!" No kidding. The surgeon said that he couldn't believe that I could still walk because I had the worst case he's ever seen. He wanted to do surgery, but a 3-month recovery with NO bending, lifting or twisting. Since I live alone, that's not happening. The constant excruciating pain has become so unbearable that I can't even sleep in bed anymore, but I have to sit up on my couch. On some days, it's tolerable. It's also exacerbated my chronic sleep disorders. I'm just a mess in general. I'm in Killeen.

I understand. The doctor that looked at my mri said he had not see one person that has so much bad going on in their back. I had one day this past week where I had hours without pain. It was wonderful. I try not to get depressed. Cold weather really bothers me. If you are a praying person we could pray for each other. ❤

Wow! That was an excellent presentation. I was a career Army officer and we always tried to comminicate very complex things 'at the 8th grade level' so everyone can understand the task. In order to do this you have to really know your topic. I think an 8th grader could understand your presentation. I'm going through the lumbar/sciatica process now. My chiropractor recommended I persue the pain injections. He mentioned the diagnostic benefit as well. I have found the injections very helpful for my neck. At the VA where I get treated, for a chronic problem you have to be referred to the spine surgeons by pain management or physical medicine, so they don't QB the injections.

This is the third video from other professionals I've listened to about lumbar spinal stenosis and I have to say Dr. Donnally convinced me to get the injection of steroids that my pain management doctor wants to give me. My back pain is between L1-L5 and has affected my quality of life so much that I cannot stand more than 4 minutes or walk more than 8 minutes without the pain nearly causing me to crumple. Dr. Donnally has a very good bedside manner with a clear voice, specific pronunciation and my guess would be very kind to his patient. Even though I have so much pain in my back you would think a needle wouldn't scare me but it does because I am deathly afraid of the pain the needle will cause. I have bookmarked this video so I can watch it again. Thank you Dr. Donnally.

My pain management doctor offered to prescribe two Xanex to take before the appointment. Hopefully I'll work up the nerve to get the injection. Thank you for responding.

All the doctors I spoke to and information I have read say that the patient needs to be awake to be able to answer questions the doctor will ask such as do they feel pain in their legs or feet. If they do that is bad and the doctor will either stop or put the needle in a different spot.

I have recently had x-rays and an MRI, both indicating lumbar spinal stenosis as well as disc degeneration. I have been trying to translate all the medical terminology used in the MRI report so I can understand the true situation. This video is excellent in explaining the various elements involved, the possible conditions and how they can affect each other, and various procedures - primarily non-surgical - to help relieve pain. Thank you, Dr. Donnally, for your clear, concise talk in lay terms so we non-medical folks can grasp and understand the body structure and elements involved, what can go wrong, and how different procedures work to counteract the problems. I especially appreciate your dedication to using non-surgical treatment whenever possible, leading to quicker relief and shorter recovery times as well as minimizing risks. I rate this video as 10 out of 10, and recommend it to others who need this information.

Had a MRI years ago, was sent to a doctor who does spinal blocks, he would not do one on me, said he didn't want to paralyze me. I am doing therapy now helps me straighten up, relieves some pain. Had back problems since I was 36, am 76 now. I always have pain, have learned to live with it. I like standing up straight, I don't feel like everyone stares at me.

I've watched a lot of vids on this topic and this cleared up a lot of the fog for me. A great explanation of what stenosis is and the treatment options. My local doc didn't do 1/10th of the education that I received watching this video. Thank you.
