IMPORTANT 2023 UPDATE re: IUD DURATION OF USE- US FDA now approves 52mg levonorgestrel IUD (Mirena, Liletta) for 8 YEARS use for contraception. When treatment of heavy menstrual bleeding is the primary indication, best results are usually seen in the first 5 years. Kyleena (19.5mg) is still 5 years. I am 49 years old and onto my 3rd Mirena. It was the best contraception choice for me and I haven't looked back. I suspect I'm perimenopausal due to the usual signs but I haven't had a period for over a year. Thanks Dr for showing me the way forward for the next few years for me and that I can get 7 years from my current Mirena. great news!

Dr. D, I want to thank you. I’m 46 and got my Mirena inserted yesterday for treatment of heavy periods that lead to iron deficiency anemia. I was nervous about the insertion but it was no big deal. It just felt like one deep cramp and then it was over. Literally five seconds or less! Your videos helped me know what to expect and to not be so nervous. I’m hoping to be one of those women who doesn’t bleed at all with the Mirena. Thank you so much!

Thank you so very much for providing such thorough information. I am 52 years old and my doctor has suggested this and unfortunately did not answer the many questions that I have. I know in medicine today providers are very busy and cannot take the time that they used to but not having those questions answered caused me to not be able to make a decision. So this video provided me with everything that I needed and I am now watching multiple videos on your RU-vid. I’m so thankful that I found you!!!

Am 46 in peri menopause phase and have been advised for Mirena to treat recurring uterine polyps, which cause heavy bleeding. This video has really cleared my doubts. Hi love your channel, I was diagnosed with pcos in my early 20s and the mirena has been amazing in helping with irregular periods etc. I am now 56 and blood work shows I am postmenopausal. I am coming up to the 7 year mark with mirena and will have it removed. I am using vaginal estrogen which I love and whilst I know I do not need progesterone with this, I was wondering how will my body react without the mirena and should I have it replaced with a new one. Thank you for everything you do ❤

Sorry for the late response I am catching up on comments! Progesterone is not required if a patient is using only vaginal estrogen (the dose of estrogen is much lower than other HRT regimens and so endometrial protection is not necessary). -Dr. Dervaitis

Thank you so much for your wonderfully informative videos. I'm 55 1/2 and just had a Mirena IUD implanted a few days ago for uterine protection to be used in conjunction with estradiol gel or patches for hot flashes and for protective aspects. Last period was 4 months ago, but I have been having hot flashes off and on for 3 years. Tried bio identical hormones (progesterone capsules and estradiol face cream) but after 6 months, it made my IBS so much worse I had to stop using HRT for a year. Recently tried Duavive for 3 months but it made me very gassy and constipated along with right rib pain (gallbladder?) which was remedied by drinking apple cider vinegar and lemon juice in water to help make up for lack of bile production (I'm assuming?). As I have significant bone loss, I'm now considering using HRT indefinitely to 1) reduce bone loss, 2) reduce risk of Alzheimer's , 3) reduce heart attack risk and 4) alleviate severe joint pain as estrogen helps lubricate joints, so I've learned. I'm hoping the Mirena IUD option will spare me the unpleasant gastro side effects that progesterone has caused me in the past as it will no longer be circulating throughout my entire body. Best of luck to anyone trying to navigate this difficult stage of life and finding the right HRT solution while managing IBS (they don't play nicely together for some of us).

Thanks for your comment and thanks for watching! For any viewers out there who are reading this comment, I would make three general comments about HRT: 1)- there is currently no evidence regarding the use of “bio identical” hormones (and definitely no evidence to show any superiority over traditional HRT). I do not prescribe or recommend bio identical hormones to my patients. 2) The Society of Obstetricians and Gynecologists of Canada (SOGC) Menopause guidelines state that the predominant indication for systemic HRT is treatment of vasomotor symptoms (hot flushes), and not any other indication. It is recommended to be on as low a dose as possible, for as short a time frame as possible (as long as it is required to manage the vasomotor symptoms and not longer). 3) I agree that using the levonorgestrel IUD is an acceptable and convenient way to provide the necessary progesterone endometrial protection that is required during HRT with estrogen (in women who have not had a hysterectomy). -Dr. Dervaitis

I was having some of the issues you are having... My doctor had me switch my diet and lose weight. One drug can affect another and make things even worst and I do not believe in relying heavily on prescription meds personally. I am not going to say which type, of diet... but all of my symptoms began to dissolve slowly. Maybe talking to your primary doctor about your nutrition or perhaps see a nutritionist may help relieve some of your issues. I use to have very bad hot flashes and haven't seen one in 2 years. I'm only dealing with high blood pressure and which would get better if I moved more. Good look... God bless!!

I’m 41 years old. My mom at the age of 43 was over period. Almost two months ago I changed my method from pills to copper IUD. It so weird because as soon I stopped the pills and got the copper IUD I have been fellings some flashes that just come a few seconds no more than a minute and then they are over. Next week I m going to have two months from the paragad insertion while I was on my period and since that day, I haven’t seen my period again. Contrary what they used to say my period has stopped with a copper IUD. I have had some spotting yes but really light. At this point I’m thinking seriously I have started the menopause. I m just giving some more weeks of adjustment. Maybe 4 more and then I have to consult this. I would like a video about experience menopause with copper IUD. Thank you

The copper IUD does not influence the onset of menopause. If a patient with a copper IUD stops menstruating, it is due to natural menopause, or pregnancy, or other potential causes of secondary amenorrhea (the cessation of menses). Is it common for periods to start up again after no bleeding for 7 months after having the mirena coil fitted? I never checked the strings as I didn’t want to become obsessed with checking but when I have checked I cannot feel any. I was never asked back to make sure it was placed correctly I am using the coil along with oestrogel for my HRT luckily my husband has had a vasectomy, so I didn’t need it for birth control just migraines and heavy bleeding. If it has fallen out I don’t think I would have another fitted because it’s constantly a worry now and don’t think I could go through it again

I’m in perimenopause but also have no thyroid. I am on hypothyroid Rx for the rest of my life. I’ve found that while on the pill for 6 months…my thyroid symptoms are becoming bad agin! Because the pill lessened the effects of my Rx. So, I HAVE to have the copper iud. I’m 47 soon. So, I feel I’m too you to not be intimate with my husband, but (in my personal opinion) I do not want to start over raising children. Please do a video where you talk about the copper iud for peri menopausal women and if those effects are any different. Ty

This video lifted my spirits. I am 49 i am anemic 10.5 iron and 5 saturation. My doctor recommend this iud for me to assist in reducing my mental cycle to eventually having it not come my cycle started to skip after my dad passed a year ago. Now it comes every 3 to 5 months. I could use a little extra progesterone. I have hot flashes, night sweats, heart palpitations. My cycle last 6 days with two heavy days. I am concerned of hair growth in areas i dont want and gaining weight this would be a nightmare. I am going to move forward with it. Thank you.

My mother had the most horrific menopause iv ever seen she was the demon from hell for 15 years I have a true and real fear of being like that my aunt on the other had didn’t even know that a had reached menopause but b had already passed it with zero issues she swears it because she had the merina coil in throughout so even though I have my tubes tied at 44 I went and got a coil only time will tell if I’m going to take after my mother or aunt as I’m already delicate mentally and wouldn’t survive what she went through

I am 64, very healthy and have been taking Premarin and Provera very happily for 10 years . I have not had any children. My doctor has advised a Mirena coil and estrogen gel to rub on my arms which I am happy to do. I have a retroverted uterus and wondered whether insertion will more difficult and painful. I have also been advised to insert Ovestrin ( Estrial) 1mg cream vaginally 2 weeks before insertion. I was wondering why I need to do that is it because I am older?

I’m not familiar with any protocols that involve vaginal estrogen prior to IUD insertion. Perhaps the idea was to try to thicken the vaginal mucosa (it’s thinner in post menopausal women) to make the speculum insertion less uncomfortable? (This however is not “routine” ). Whether a uterus is retroverted should not impact the insertion or the degree of discomfort (the inserting health care provider examines the patient prior to insertion to assess the position of the uterus and then conducts the insertion accordingly). -Dr. Dervaitis

Hello Dr D., I am 47 years old and my mensies have become irregular, after they came like clockwork for years. Sometimes I skip them, after which they are quite heavy and other times they come very lightly every week or so. They're not specifically painful, but uncomfortable, especially in the summer. So, I'm going to have the Mirena IUD placed for sure! My GP says that there can be additional estrogen administered as to completely get rid of any blood loss. Unfortunately, as I understand, I cannot have that, because I've had a heart attack, angioplasty and a double CABG earlier this year (May). In that respect, I do worry about the cortisol levels in the IUD as I need to reduce stress as much as possible. Is it a risk for me? Thank you!

The IUD does not release any cortisol, nor does it have any impact on cortisol levels. Most bleeding/menstrual issues that are treated with the levonorgestrel IUD do not require any additional estrogen treatment. -Dr. Dervaitis

Hello. I got my second Mirena in Nov 2018 and had irregular periods. I was 48 in 2018. Then from Jan 2020 to early April 2022 I had no period at all and now I am spotting. This is worrisome to me as I had some periods then nothing and now I have been spotting for 6 weeks now. I will be talking to my doctor ASAP, but sometimes she downplays things because I have the Mirena and that worries me more. Ottawa ON 🇨🇦

Sometimes bleeding can change, and someone may start to have bleeding or spotting after a time of not bleeding at all. But you are correct to see your doctor- with any significant change in bleeding it is a good idea to check the IUD and rule out other causes of new bleeding. -Dr. Dervaitis

Hello, you suggested I watch this video from a previous post I made and I did. I’m having a heavy period which makes me believe I’ve lost the hormones from my Mirena. I’m 51 years old and have had the Mirena in for 6 years and 3 months. When I get it removed, what would you suggest I use for birth control safe for my age as I sure don’t want to become pregnant at 51 (especially since there’s a chance due to me having heavy periods). Hi Kristin- Your doctor will walk you through which options are safe for you, based on your medical history. (Have you considered having a new Mirena inserted at the time your old one is removed? The pain during insertion is why I haven't had it removed yet, I'm afraid of another bad experience. I don't smoke, maybe it will be safe for me to be on a low dose BC pill. Hi im 48, havin symptoms of perimenapuse ect, blood showed id bit low estogen , my cycles are every 30 days depends ive pcos. Was told would not get hrt over my weight plus i still have periods. So my choise from dr was if i was to take estogen patch i have to put in merina coil is this reason like????

If giving estrogen supplementation, it is important to also provide progesterone to prevent overgrowth of the lining of the uterus (which could lead to pre-cancer or cancer of the uterus). This progesterone can be given in pill form, or via the progesterone IUD. -Dr. Dervaitis

I am 51 and my dr has thankfully given me hrt, at the mo i take a progesterone tab (beginning with a u, i cant remember its name) every night regardless, and 2 pumps of gel. My period is still regular. She wants me to have the coil. Will my periods stop if i do? I dont want them too, but the tablet is making me depressed, tearful etc. Thank you.

There is a lower chance of hormonal side effects with the hormonal IUD because most of the hormone is delivered to the uterine lining (and very little gets into the bloodstream). Approximately 30% of women stop menstruating with Mirena. -Dr. Dervaitis

Hi again Mil- I’m not sure if I fully understand this question, but both the copper IUD and the hormonal IUD are associated with a decreased risk of endometrial (uterine) cancer. The birth control pill also is associated with a lower endometrial cancer risk. -Dr. D

Dr. D. Unfortunately I'm watching this video after removing my mirena Coil yesterday. If only I had seen your video before I may have left it in to nearer the 7yr deadline for the ongoing progesterone benefit. I had mirena put in approx 5 1/2 years ago for heavy bleeding which I now know to have been a sign of me being peri menopausal which was later confirmed by FSH blood test. My GP recommended after Coil removed I can have FSH blood test followed by another one 6 weeks later to confirm if I have reached menopause. My questions are (1) can I rely on the blood tests alone to confirm I've reached menopause or do I also need the one year free cycle (2) you mentioned another blood test along with FSH - do I undergo both or is just one okay, and (3) what recommended hormones should I be taking going forward in my menopause journey? By the way I'm 55 and 56 next month.

Hello- The most reliable diagnosis of menopause is twelve consecutive months without menses. The FSH level (or FSH and LH levels) can be a surrogate marker for menopause (but are sometimes unreliable as these hormones can fluctuate). Based on your FSH results (on two occasions) and your age, you doctor must have been quite confident that you are menopausal. The topic of hormone replacement therapy in menopause is beyond the scope of this channel. Please talk to your doctor for more information. -Dr. Dervaitis

Are hormone IUDs safe for women over 40? There seems to be high risks with Birth control pills for this age group so how is this different for IUDs?

Yes, absolutely they are safe for women over 40. The difference is that they don’t contain estrogen (they are progesterone only, whereas the combined birth control pill contains both estrogen and progesterone). Although it may be preferable to avoid estrogen in women over 35, it may still be safe for these women to consider the pill if they have no other health issues or contraindications and are non smokers. -Dr. Dervaitis

I have read about a lot of people having heavy bleeding after removal. How common is this? Should I expect any bleeding if my IUD is removed once I am menopausal? Thank you

Hi Valerie- There can be some light bleeding after an IUD removal, but it’s usually not heavy, and doesn’t last very long (I would say a few days on average). If there is heavy bleeding beyond that, it is more likely to be related to the patient’s actual menstrual cycle rather than the removal process itself. If you are truly menopausal, there should not be prolonged bleeding. (I would also note that any new/future bleeding in a menopausal patient should be reported to the doctor and investigated). -Dr. Dervaitis

In Canada (where I practice) Mirena is approved for use up to 5 years (note: approved duration may vary from country to country, in the US, use is approved up to 6 years). Hi I am 48 years old, on the 28 day contraceptive pill, do not have heavy bleeding nor pain, I have not had children and am peri-menopausal. I do however bleed for about 2-3 weeks of every month, could having the Merina help with this maybe? Also, how does the level of progesterone in the Merina compare to that of the contraceptive pill? My wife is being using Mirena for about 15 years, she is 42 know. My cuestión is if she can still using Mirena or is it recomienden to stop and rest for a period of time? Is there any risk if for her if she decide re used the Mirena?

There is no “limit” to the amount of time a patient can safely use Mirena. There is no medical advantage or need to take a “rest”. Long term use of Mirena has been associated with reduced risk of uterine and ovarian cancer. Mirena can be safely used right up until menopause. -Dr. Dervaitis

Technically Mirena is approved for 7 years of use of it is inserted at age 44 or greater. However, recent pandemic guidelines have suggested that extended 7 year use can be considered if insert was at >age 25. I am 49 peri menopause my doctor prescribed me Mirena coil for heavy periods and part of progesterone as well as evorel patch! I had coil removed today and not sure hows its gonna be. My GP was not available to talk to.

@@dr.kristinadervaitis789 A question so sorry I in meltdown.... its been hell. Acne and hair loss bad sinec had Coil fitted and went on Evorel patches in Feb. They upped my patch to 100.... but think synthetic progesterone very bad for me...... going on more natural version by tablet. What are your views..... as I am feeling desperate and beginning to feel I had enough as feel so so ill.

There is currently no definitive/conclusive evidence to link the levonorgestrel IUD to breast cancer. There has been some observational data that has suggested this, but it has not been established in any large clinical trials. Keep in mind that there IS definitive evidence to prove a decreased risk of uterine cancer with use of the levonorgestrel IUD. -Dr. Dervaitis

Some women may experience irregular or increased menstrual bleeding in time leading up to menopause, and the levonorgestrel (progesterone) IUD reduces bleeding in most women. However, it does not change or impact the timing of menopause, nor would it assist with vasomotor symptoms (hot flashes) related to menopause (it’s estrogen that can improve those symptoms, not progesterone). -Dr. Dervaitis

The quoted failure rate is 2/1000 for all users of Mirena. There are not separate numbers published for each age group. -Dr. Dervaitis
