A Mooney M20K crashed in a wooded area about three minutes after departure from Rwy 13 at St. Augustine-Northeast Florida Regional Airport, FL (SGJ).
The aircraft was attempting an emergency return to the point of departure of SGJ due to an opened door.
The two occupants were fatally injured and, the aircraft was destroyed.

I hope that controller is recognized in his community for the outstanding work that he did. It’s very impressive to hear a professional manage us hugely stressful situation as clearly and calmly as this man did. Terrible tragedy, but the community did everything they did to find these poor folks. Unfortunately it was not going to help this time.

I also would like to commend the ATC, Police Helicopter pilot , medevac pilot and first responders for the quick and decisive actions and professionalism. This was my brothers plane and he was the pilot. He had just picked the plane up in North Carolina the day before, and he and his best friend were on their way down to homestead to visit with my family down here. We are dealing with the aftermath as is his children, his friends family and other family members. Life is precious and we don’t take it for granted.

As a retired controller, that tower operator did an outstanding job of maintaining control in a professional manner. As a retired corporate pilot & flight instructor, with about 1,000 hours of Mooney time, I can imagine the distraction created by that open baggage door flapping and banging. HOWEVER, it wouldn't adversely affect the ability to fly and land safely. Another commenter made mention of the laminar flow wing on the Mooney, and he's correct about low and slow and tight turns. That said, had the pilot flown a normal pattern at the correct airspeed, he would have landed safely.....

Yeah I was wondering about the effects of a blown open door, would that affect the flow over the horizontal stabilizer at all or is this just a case of distraction, low airspeed, base to final stall spin?

You are so right. All through flight training all of my instructors stressed when things go wrong, fly the airplane! A couple even popped the door open right after takeoff. It is loud. But the aircraft flies. Just shrug, and land.

The single most important thing stressed to me in all of my flying with instructors, starting with Privates thru ATP was.....FLY THE PLANE. So many accidents are caused by distractions instead of flying the plane. A door unlatched should have been nothing to worry about here. The pilot sounded like he understood this and wasn't worried. I can't fathom how this crash could have happened.

During my years of instructing, I often loaded the student, especially instrument students with distractions. The point of that... always fly the plane first. The baggage door flapping, a distraught passenger... who knows? The position of rest, as depicted above, shows the plane impacted right of the runway centerline after flying a left hand pattern. Further, it shows the plane crowding the airport on downwind. I'm seeing the pilot overshooting the turn to final and the age old... kick in a little rudder and skid it around, only to be at low speed in a steep, skidding turn, and a resultant stall spin. (Also keep in mind, depending on the conditions the pilot has created for himself, the 'stall-spin' could very well emulate a snap roll, rather than a mere stall with the outboard wing coming up and over). EVERY student pilot I trained, before I wrote the recommendation for the flight test, did spins. An hour of ground school to explain what a spin is (aggravated stall), explain what a person would sense, and the recovery. I'd use a Cessna 150 Aerobat and after the first demonstration, they were enthused, rather than scared. The coup-de-grass, I'd pick a long straight road and create the scenario... "We're low and slow, and over shooting the turn to final, so I'm skidding around the turn to final and adding back pressure to speed up the turn and............... over we go!" When the students responds with... "Well now I know why," you know you've possibly saved a life.

I'm a recently retired Airline Pilot with over 40 years of service & I'm an owner of Multiple G.A. Aircraft. In my experience, I believe that this Pilot simply became so distracted by The Opened Baggage Compartment Door that He lost his Situational Awareness & let His Aircraft become too slow & went into a Stall/Spin Situation. Unfortunately, being on Final he would have been too low to even attempt any Stall Recovery. My heart goes out to The Family & Friends of The Men that perished in this crash! I'm sincerely sorry for their loss! May G-d Comfort them in this difficult time!

Pilot didn’t say which door. Passenger door is not big deal, however Luggage Compartment door in Mooneys is an emergency. Mooneys don’t fly well with LC doors open, and also run the danger of the door separating and hitting the horizontal stabilizer. The LC door is locked with a latch and a key locks the latch in place for further redundancy. I owned a M20J, I added “LC door and latch locked” in my preflight chklst.

Open baggage door on an m20 has little to no effect on performance. Embarrassed to admit it, but been there done that. Distractions, if you let them, will kill you.

There's a video on YT of a M20 crashing in a field because of an open door.. I don't recall off the top of my head if it separated and damaged the h.stab or not.. but I know of a M20J that crashed because the bank angle was too steep on a base to final turn and the pilot couldn't roll out.. and I'm wondering if the pilot did that in his turn to final

I flew with my door unlatched once and it opened I looked to my left and said “OH SH*T” and my instructor said “do you feel like you’re about to fall out?” I said “no” and he said “then we’re alright”

The problem is wind resistance. One of my CFI was a 70 y/o navy pilot and he demonstrated to me how the door can act like rudder if you have them opened in flight. They probably didn't carry enough altitude to buffer for the extra resistance and potential deadly spin stall recovery since the door can act like a giant rudder.

St Augustine airport was not presenting a high density traffic situation. It was night 2's preference to remain on scene to direct foot traffic, and did so. The controller had made it clear to night 2 that they could just say the word and get immediate landing clearance to re-fuel. The police helicopter, night 2, coordinated safely, and effectively, with the controller who was then able to to handle inbound traffic as well. That professional sequence displayed an incredible synergy.

My CFI showed me what would happen if a door came open, which is pretty much nothing. Unless you push it out and you can move a 152 around and still fly.

For my first solo, my CFI inadvertently damaged his window latch and the window would flap in and out the entire time. My CFI apologized but made it very clear that it was not a safety issue. It’s so sad the occupants in this crash did not realize how trivial this situation was and that this outcome did not need to happen. RIP 😢

I had a Mooney 20C, This aircraft has a laminar flow wing.. you cannot pull tight turns at slow speed. An open door is NOT a safety issue!!

Agree - it is also aligned with the airflow so has no real impact on drag etc. It also didn't depart the aircraft as images of crashed aircraft shows baggage door open and attached. I actually think the pilot just became overwhelmed, let the airspeed decay on the turn and stalled.

Pilots initial voice is calm and seems to understand it is a non event. Must have totally lost his situational awareness and let it stall and spin low to the ground. You don't get to try again.

Yes and that's why it is absolutely ridiculous to fly a square pattern with two sharp 90° turns. It's much better to make a curving final approach from downwind as it reduces your bank angle and thus increasing the margin of safety with regards to load factor (bank angle). High performance seldom if ever fly a squared up pattern due to the risks as just noted.

Didnt know how to fly a close pattern and do a short approach too. Then stalled it when overshot final too. 3 student pilot errors. Plus leaving the door unsecured. 4 student pilot errors in a few minutes only. EFATO CFI for 3k hours..

I live here and saw this when it popped up in local news feed. No details until later. Sucks this happened to the 2 people from NC in their Mooney. St Augustine is lucky to have every possible resource available for emergencies: Sheriff helos, our beautiful Life Flight and crews, and an excellent ATC crew. But none of that helped these aviators. Rest in Peace. 😞😢

Bonanza guy here. I trust the Mooney folks who say that an open door is not a serious safety hazard. The couple of times when I have had unexpected events airborne, including a door opening on my V35B, I literally said out loud to myself; "THIS Is a screw-up opportunity!", and REALLY focused on flying the plane, airspeeds and GUMPS!

@@dohabanditGood Morning Mr. Bandit - I very much hope you are wrong. This plane never achieved very much airspeed, so coming off is not what I would expect. PRESUMABLY the cargo door hinges are designed to survive a door coming open at any IAS the plane is certificated to operate at. Hopefully this doesn't lead to a costly major AD for our Mooney friends.

I did my PPL at KSGJ in 2011. ATC was always very friendly and professional, as this controller here (I think I remember his voice from the numerous touch-and-gos I practiced there). It breaks my heart that two people died at "my" airport. R.I.P. from Frankfurt, Germany 🖤

I had a Mooney, M20F. On climb out the door popped open. Not the back luggage door but the access door. Plane flew normal and returned to land. Uneventful. This pilot seemed fine right up to the moment he wasn't. Probably distracted and stalled. Very unfortunate. Condolences.

He was approximately 600-700 m away from the centerline of the runway on downwind. Possibly too close. Then he made a tight turn (>30degrees bank perhaps) because he most likely overshot the turn. The result was low speed and too tight a turn. He could have widened up his downwind by flying up the river a bit and then maintaining the proper speed.

I had the window on my side of a 172 open right after takeoff with a loud woosh , that startled my instrument instructor but I remember not reacting to it... I guess cuz I'm I had it drilled into me that at critical times there is no way I going to needlessly be distracted by secondary issues.

I've not been able to fly since August 2006, when our plane was circling over Comair 5191 crash site at Lexington airport the morning of the accident. I didn't fly a whole lot before that, but watching emergency response vehicles and the smoldering wreckage changed my views of commercial flight. Got off the plane and extended my rental car to use for return trip home. RIP for the souls on board.

The turn from base to final is a common stall spin location because of excess rudder and low speed. This shows why flying your airplaine is top of your to do list. Obviously a sloppy preflight ended in two dead. Why people like this think they should be flying with passengers is anyones guess. As a CFIA&I it is something I have seen the results of poor airmanship!!😢

Just goes to show how tight the aviation community is. Probably none of them except maybe the two helo pilots and the Riddle pilots knew each other, but they all volunteered their services, skills, and aircraft to come to the aid of fellow aviators in distress. It's sad that such a non-event as an open door on a single-engine small private plane could result in loss of life. I was flying solo in a Cherokee once when the cockpit door popped open soon after takeoff. You can't reach the right door from the left pilot's seat, so I assessed the situation as a non-emergency and returned to land, following the published traffic pattern for the airport. I shut down, did a walk around for possible secondary damage, and after finding none, I reentered the cabin, thoroughly secured the door, fired it up and performed a second, uneventful take off. The flight back home was nice and easy, and I thanked myself that I didn't panic over a simple open door. I am NOT saying that this Mooney pilot did anything wrong, but any time there is non-standard action: Aviate, Navigate and then Communicate.

Very sad. I was in the back of a Mooney last year and our door popped open. My buddy flew a normal pattern and we landed real quick and secured the door. This humbles me a bit. Very sad for these 2 and their families. Prayers for them all.

Thank you for sharing this video. I just had a similar issue like this today when I took off from Charleston International Airport (KCHS) on an IFR flight plane to Allendale SC. The rear door popped open shortly after departure on the Diamond Da40 I was flying. Eventually the entire rear door flew off as I was making my right turn to return to the airport. @@emergencylowmaneuvering7350 I was also a CFII MEI in the 1990s, and my honest experience was that pilot skills at all rating levels were generally below what the FAA says it's looking for. Basically analogous to how everyone test-preps for the written tests.. so that we never really know how people would test if they didn't essentially train for the test itself. As I spent time actually flying w students and other pilots, it was like any other profession. Some people, despite having a certificate, are really not good at flying, most people are ok or average, and there's always a few who really have a feel for it & deserve to be called pilots. The system is designed to allow a pretty large ballast of mediocrity and naturally some small percentage of frankly not very good pilots.

I’ve owned Mooney’s for 45 years. IF it was the main door and NOT the baggage door it is not an emergency. You don’t even have to land to close it. There is a procedure to close the door in flight in the Pilots Operating Manual.

I used to have the right door on an older Cessna 172 (5483T) routinely pop open in flight. Because of the airflow along the fuselage it would only open 6 - 8 inches while making quite a racket and drawing a slight vacuum on the cabin. No problem if at altitude, I simply slowed to the best glide speed, reached across the cockpit and slam the door. If near the ground (where it seemed to happen the most) I waited until I had sufficient altitude to execute the above.

So sad to see this crash.RIP. I had the passenger door pop open on a M20K on rotation - that messed up aerodynamics a little and it was hard to climb, but returned to the runway without problems. Pilot must have become distracted and stalled on base to final.The controller is amazing.

I've seen other Mooney aircraft where a departed baggage door struck the elevator. This pilot seemed pretty calm and relaxed, which makes me wonder if something else may have contributed... like the door departing the aircraft and striking the elevator.

Images of crashed aircraft showed baggage door open and still attached. 13:02. If the main door opened, then it's not going to 'open' - just be unlatched - but yes if the entire thing just fell off and hit the elevator then that might explain it.

@@bikeguy3034That baggage door would not be torn from the aircraft at speeds the Mooney flies. Even if it struck the elevator, aircraft control could be maintained, and apparently was, given the communications.

Can someone explain to me why they didn’t close the arrivals given they were trying to find a crashed aircraft that potentially needed urgent medical attention? Seems weird that they prioritised the jet arrivals and delayed the search and rescue operation.

This is not the first Mooney baggage door incident I have seen, some time back in the U.K. a baggage door came open and then departed the aircraft hitting and warping its self around the stabiliser and restricting elevator movement. The aircraft was in the cruise but above an airfield , the pilot immediately landed at the airfield. He recorded the incident with a GoPro and it can be found on RU-vid. In my opinion the door opened because the safety pin on the internal emergency release was positioned the wrong side of the release handle and did not secure the door.

WOW - Although a tragic end to what's generally an easily manageable/survivable inflight incident, the professional conduct of ALL involved makes me proud to be a member of the GA community. In addition to the Tower Controller's excellent "multi-tasking" abilities, his initial situational awareness is something training videos are made of. Additionally, the mixture of Police, Corporate, Private, and STUDENT Pilots is awesome to listen to/witness. For those not familiar with aviation callsigns, the multiple "Riddle____" are training aircraft from nearby Embry-Riddle Aeronautical University. Those flights could be solo students or training flights with instructors onboard. In either case, you couldn't tell from the radio traffic which it is. My sincere condolences to the family and friends of the pilot and passenger, and "BRAVO" to everyone else involved from the Tower, Law Enforcement Air Crew, and @EmbryRiddle organization. I was in a Mooney in 1989 when the door popped open and sucked the owner’s newspaper out the door. He said - I told them to fix that.

The crash didn't happen on the airport so there's no need to shut it down, just coordinate search and rescue if needed.

I have had a door open a couple times in a bonanza. The door, although a huge distraction, pales in comparison to keeping my passengers calm. One time I had to land with a friend of mine practically setting in my lap after his door opened. (It only cracks open) The pilot didn’t panic as a good pilot wouldn’t. I suspect he just got a little distracted.

Reading over comments, some people said that it could be a problem with a door popped open on a Mooney specifically, others said not, just fly the aircraft. Regardless, one thought on the matter is if they had a purse or luggage that was threatening to fall out, the pilot could have been concerned/distracted especially if a passenger was complaining about it, and of course flying the plane should have been his only concern and getting back on the ground.

The ATC showed remarkable composure throughout this incident. Made sure to keep traffic separated, properly communicated with everyone. Not sure what to make of the situation. I wonder if the Mooney's door blew off and damaged the tail when it did so, leading to sudden loss of control? I also wonder if the pilot misidentified what opened. I can't imagine the door popping wide open, the airstream would tend to keep it shut. (Think of what happens when you open your car door at 100 mph.). However, if the cargo hatch popped, that could be catastrophic.

Pictures of crashed aircraft shows baggage door open and attached. 13:02. As it is aligned with the airflow, I don't quite think it would be 'catastrophic'. We regularly opened our side door in P-3 Orion to throw out survival packs etc - no big deal having a door open. I actually think the pilot just became overwhelmed, let the airspeed decay on the turn and stalled.

Since it didn't detach, I agree with you that the fact that it was open was no big deal. So why would the pilot become overwhelmed? It was otherwise a routine return to base. I wonder whether an autopsy would reveal some clues.

This is very sad, my heart goes out to the families. but what always strikes me about tragedy in aviation is the professionalism in the face of tragedy. Everyone in the air acted calmly and professionally, dropped all other plans, to do their best to get help where it's needed. Likely a bunch of strangers, working like a well oiled machine. After a tragedy, there is mourning, of course, but it's never left at that, we learn from it. Sometimes that lesson is a reminder of basics, sometimes that lesson finds a fatal flaw in an aircraft that requires modification, but we strive to learn the lesson, whatever it may be. More areas of life need this.

Wow this controller is a true professional. I really hope he sees comments on videos about this. Really well done. And im a pilot, i hope if i ever have a problem i get a dude like this talking to me. The vast majority of air traffic controllers in this country are absolute polite professionals. But you definitely get some grumpy ones at times. Understandable it's very stressful, but in an emergency everybody needs to be like this guy.

The position of the aircraft at impact, slightly north and west of the active, suggests the pilot may have attempted a steeper turn than be intended turning base to final to compensate for his position. We all screw up. Some of us just pay a dreadful price.

Aviate, Navigate, communicate. Rule #1: Fly the aircraft! An opened door is not an emergency, don't turn it into one. Can't count the number of times i've had doors pop open in flight. My door, Passenger doors, etc. It's unnerving. But it's not an emergency.

Pilot seemed calm and under control. Could be that additional damage caused the crash or that he obviously overshot the runway and pulled too hard too slow to make the runway centerline.

My instructor opened the door when I was on base to land and I landed the plane. She said good job because I didn't get distracted. Not sure what. Happened with them... But we learn from all of these experiences. Always keep your airspeed

The baggage door opening on that Mooney is not on its own a threatening event....Sounds like he slowed too much ...hindsight always painful....fly the plane always....no matter what.
