I found this really helpful. So much so that I’m going to print out the slides from the presentation. I am on an oral chemotherapy drug that costs $22,500 PER MONTH which means I will be in and out of the coverage and into catastrophic coverage in January, 2024. Five percent might sound like not very much but since July, 2023 I have been paying the 5% every month. Needless to say: the IRA is a god-send. Thanks for breaking it all down.

Thank you for answering a question I've had for years... my tier 1 and 2 drugs did not generate a "deductible" situation just a co-pay and I never understood why. Good info in this video

I must point out you totally ignored the $545 deductible in your illustration of annual costs. That increases every year and what is the expectation regarding “substantial” increases by the insurance industry going forward to make up the difference they apparently will be losing by the $2000 cap on out of pocket expenses? This should not be ignored and you should include these costs in your illustration slide much like you did in your initial slide to show “true” projected annual costs. The rising deductible costs might very well eliminate any real savings in total “out of pocket” costs. It is hard to imagine the pharmaceutical industry just giving up revenue so easily.

Thank you for the feedback. The deductible is certainly explained in the slides. Also, it’s interesting to see the changes in premiums on the Part D plans for 2024- some carriers have increased premiums substantially, while others have dropped their premiums. One carrier has actually dropped to $0/month in certain states. Thanks for watching and the feedback.

I agree with @robward155, I think it's important to discuss Biden's Inflation Reduction Act and how the out of pocket cap going forward is obviously being thwarted by these greedy insurance companies. This is a loop hole that needs to be closed!!!

I am in the catastrophic stage by the second month. It will help seniors to apply for the extra help from SS if single income is $22.000 and $30.000 married.

Understanding the complete pictures of health care and coverage is a must, but for all seniors that don’t have any ways to understand is the saddest to have in their lives and they have much more complications between illnesses,diseases in addition to being lost, confused to this whole “Medicare” either we are stuck or reached dead ends it’s a double kill!

Most states have a senior advisory advocacy group that helps them with things like this. Although, they can’t make recommendations, they are generally very good at helping people understand the issues, options and any additional help available through their state and federal government. In NC & FL it’s called SHIP. The Medicare website can usually provide links and information about how to contact these services providers in your state. I’ve used them repeatedly and they provided a great wealth of information, services and assistance in even applying for some of the benefits for my Mom.

May I ask you why "Member Pays" Cell in December shows $300 when it should be zero as it is already over $8000 in Catastrophic Coverage?

Hello Alexander - The top row shows the Actual Drug Costs for the Consumer which is showing $0 for December. The 6th row down just factors into the Initial Coverage Level and TROOP (True Out of Pocket) figures.

Hello - thanks for your feedback. We would need the list of medications with dosage /frequency, to complete a part D analysis /recommendation. Generic drugs but fall into different tiers/costs, depending on drug. You can give us a call at 908-272-1970.

Those that are expensive drugs you will need prior approvals with inferior drugs because they wont approve the drug you need.. this isnt like regular insurance out of pocket. Most elderly dont get the meds they really need because they cant afford them.

Hi Lou, I believe you are referencing Step Therapy, rather than prior approvals. Step therapy is where the insurance company may request you to take a lower cost drug first. And just to clarify, most Medicare beneficiaries actually have very inexpensive medications. Only about 15% reach the coverage gap. For those it can be very expensive; but there will be a cap in 2024 of about $3,500 and a cap of $2,000 in out of pocket drug costs in 2025 on Part D plans.

That would depend on the medication.

Yes, most drugs are available on most plans for 90 days supply. There are certain exceptions for very high cost medications.

Thank you for doing this video explaining the way the 4 stages of part D work. I haven’t seen anyone else explain this so clearly. I’ve always been on tier 1 generics, so it’s never been an issue. But I will be starting a very expensive new drug and will be in stage 4 by March. The drug is not even in most formularies yet although FDA approved, so it is hard to use the Medicare plan finder tool to find the best plan for 2024. Otherwise, my current plan works the best for my other meds. Maybe the best option is to stick with the same plan since I will be in stage 4 regardless of the plan.

Hi Paul - thanks for watching and sharing your feedback. We recommend you confirm the new drug is covered on the Part D Plan. If so , you will likely reach the Catastrophic Phase after about $3,300-$3,500 of your spend on drugs. Then, you will be 100% covered the rest of the year. Best of luck with everything!

@@SeniorAdvisorsMedicare it was not covered, but it was added as a special add to my personal formulary. That’s why I may stick with my current plan for 2024. If I switch plans, it will have to be approved all over again and I don’t know how the other insurance companies are about approving these types of drugs. It’s all uncharted territory for me.

There's a problem with the language which has confused many people, including customer representatives from Part D insurance plans. The deductible DOESN'T apply to the tier 1 and tier 2 drugs in some plans, BUT the cost of the tier 1 and tier 2 DOES apply to the deductible. The cost of ALL drugs in all tiers is the "TOTAL cost of drugs". That total is what is used to determine whether the cost of tier 3,4,5 drugs is covered.

@@SeniorAdvisorsMedicare Thanks, I'm not begging for things to be free but, to just make sense. My wife at 76 takes Nothing but I understand that I will need to sign her up to avoid penalities later if necessary, I expect to sign myself up just for my BP meds and then just pay cash as my scrip is only 35 dollars every 90 days. The system is a minefield and it seems the mines are undetectable until you encounter one.

Yes. There is a 1% penalty for every month you don’t have a Part D plan when you are supposed to have one. For example, if you go 3 years without a Part D Plan, there will be a 36% penalty (based on the average Part D premium) for the rest of your life.

@@johnurban7333 - No, it's a great thing to be healthy. There are Part D plans that have very low premiums (some as low as $0.00/month in 2024 in certain states).

I didn't have Medicare drug until I was 70, because I was using the drug coverage through Walgreens. They finally realized it and I had to back pay for the five years I missed!! Not too good!

Senior Advisors calls me 5 times a day and won't stop. Innappropriately harrasing and lack integrity. Should be illegal. No amount of asking them to stop calling has worked. I am not doing business with an unethical group who's relentlessly harrassing seniors.
