I think a great follow-up to this would be to talk about how people would get off of sleeping medication they may have been taking every night for years. Depending on the medication there may be significant rebound insomnia as you mentioned. So if you made a video abouthow to transition into getting the body to once again become drowsy and sleep without that assistance I think a lot of people might find that helpful.

I have been taking 1 mg of lorezepam to sleep for the last couple of years (the nightmares, night terrors, and the night time panic attacks they caused led my doctor to this conclusion). I am stable on my meds and am ready to take a break from therapy as agreed upon by both myself and my therapist. We talked and I am cutting the pills in half and taking some valerian (approved by the doctor) to help me rest. I will probably use a half dose for a 2 or 3 weeks before I cut down to valerian only. Then I hope to reduce the amount of valerian I use as time progresses over a period of weeks and my body gets used to sleeping on it's own again. I chose valerian as melatonin seems to stir up my anxiety dreams.

Have been an insomniac my entire life due to trauma; it’s horrific. Have been prescribed trazodone, ambien, Lunesta, Belsomra, Klonopin, & seroquel. I never took them every night & all failed eventually. Seroquel had awful side effects for me & at that point I wasn’t trusting my psychiatrist anymore… I was also on EMSAM at the time which has amphetamine metabolites, definitely counterproductive to getting some healthy sleep. When COVID hit, my insurance coverage changed & EMSAM was no longer affordable. I carefully weaned myself off entirely & for the past 2 years have been on NO prescription medications. Cannabis helps a little & since I’m retired & spouse is working from home I simply welcome sleep whenever it may come. Death is welcome too; not suicidal; simply exhausted. 😢

Dr. Marks, this is off topic, but I would like to suggest a video on depression/ anxiety in the older population. Could different vitamin deficiencies in the over 60 crowd cause depression/anxiety? (as opposed to their younger, maybe healthier counterparts) Are different antidepressants and anti-anxiety medications and doses different? Is psychedelic type micro dosing off limits for them? Does the fact that over 60's don't absorb vitamins/medications well, mean they need more? Do hormonal factors play more of a role? Such as underactive thyroid, low estrogen/progesterone, testosterone? Would Selenium or Zinc help? I'm trying to help my mother, who started being depressed about a year 1/2 ago. Nothing had changed in her life, and she was always a happy person. Antidepressants aren't helping, and so much is attributed to her 'age.' Surely, she isn't the only older person dealing with this. Why do some of doctors go right to the 'old people problems' as a reason for all of their ills? Surely, something else is going on. Would you consider addressing this? Also, if anyone else has noticed this problem or a similar one, I'd be glad to hear what it is, and how you handled it. Thank you. ❤
@lazydoctorr

Absolutely! Vitamins play a very important role in our emotions, and usually that's one of the first few things a doctor checks for prior to providing medication. I recommend making sure you take Vitamin B2, B6, B12 and Vitamin D, along with magnesium/fish oil, etc as those are usually vital to our emotional play. Hope this helps! If you'd like to know some natural anxiety remedies that work, I could recommend a few that work :D

yeah the sleep quality on all these is crap as they interfere with with REM/deep sleep. A friend was on Zyrem (GHB) for 2 years it worked wonders compared with all the 7 or 8 he tried before. only problem is that it was prohibitively expensive so it was stopped. I think its the only known sleep drug that improves sleep architecture. its a real shame as this was sold over the counter up till the mid 90s when it became highly controlled because it has abuse potential. have you herd about this Dr? any thoughts?

one thing id like to add when ghb was banned it had more than 10 indications (like sleep, fibromyalgia, longevity supplement, anesthesia) now it has one indication for people with excessive day time sleepiness that happen to suffer from cataplexy ( sudden loss of control muscles) . i think it works by enabling the patents to get into deep sleep which they never could without it

Insomniac here and have anxiety as well. I've been taking Ambien for well over 10 years. It helps me sleep at least 5 hours per night. I'll take that as opposed to no sleep at all. The doctor is spot on with this video. I wake up groggy, irritable and unable to focus. Lately, my anxiety has been exacerbated by the dry mouth, loss of words and delayed speech. I'm literally at a loss for words which is very unlike me. I speak for a living so this is not good. It's gotten so bad that I'm self conscious when speaking to people and avoid having conversations when i can. I feel like people think I'm having a stroke or something. This is not good.🥺 I'm thinking of going cold turkey off the ambien but I'd like to know if the decline in my cognitive function is reversible? Will the dry mouth go away? Will I ever be able to find my words again?

I hate sleeping pills. But one thing that saved me was Quetiapine. I take 200-300mg (depending on my cycle) for BP2. And it just fixed my sleeping issues. Now I usually get around seven hours of sleep at night, and I usually fall asleep within half an hour. I still wake up during the night, but I fall right back asleep. However sometimes if I take my pills and then start doing things and suddenly it's one am I will have trouble falling asleep and sometimes stay up all night. I seem to have to go to bed before midnight for it to work well.

@LevelheadedMind 2 года назад
I worked night shift for many years and used sleep medication such as trazodone, and slow-release melatonin as she mentioned. I found the slowed released- melatonin to be most useful and less side effects, as I did not tolerate the trazodone. Also, you need to change your circadian rhythm, so wearing blue light blocking glasses 2 hours before bed is a great way for your own melatonin to start releasing naturally. Also making sure that when you first wake up you expose yourself to day light (you can use a 1,000 LUV lightbox to mimic sunlight), this will also help to set your circadian rhythm to be backwards, if that makes any sense, that way your melatonin production can be responsive to your pattern of sleeping.

I use Benzodiazepines way more than what's recommended for several years and can say that they are a blessing! It's not great but you know what else isn't great? No sleep! Recently I managed to reduce my intake to twice a week only (although I go through phases where I take them for 10 days in a row). If your dosage isn't high and you wanna get off and are finding it difficult, all you gotta do is ask for a prescription of Valium (Diazepam) and you can taper yourself off (the condition here being you know how to taper off). Technically you need a doctor, but honestly the tapering process is not rocket science so doing it yourself is possible. Not a doctor here, but did it way too many times that I now know my shit lol

I needed this. I knew it but needed to hear it again. I suffer from severe insomnia at times during my life. I take meds for a few months and I'm able to get back on track. Fall of 2021 I struggled to stay awake until 9pm. Beginning of winter I wouldn't fall asleep until 3 to 5 am. I was getting 2 to 4 hours of sleep and that wasn't a deep sleep. Happens in my life when I notice I'm stressed the most. But right now I take 3x ambien a night (15mg) by 7 pm and I won't fall asleep until 1 am. My anxiety is so bad I'm now up to 3x Clonazepam a day. I really take 3 ambien and 1 or 2 clolonazepam a day. 1 am beats the 3 or 5 am and some nights id be awake all night. There's nights where I get home late from work and don't take them and I pull an all nighter. Not quite there yet but my body is definitely getting back to a normal so I can pull back on the ambien.

Love this! I started on Ambien nightly after issues with PTSD. Of course, this was after trying melatonin and histamines which stopped working. I stayed on Ambien nightly for 8 years! I tried to stop so many times, but couldn’t function through the rebound insomnia. Tapering never helped. I finally stopped taking Ambien cold-turkey about three months ago. I spent 8 weeks sleeping only 2-4 hours per night. I thought for sure I would wreck my car, hallucinate, or fall asleep at work, but now I’m sleeping closer to 5 hours per night. My next plan is sleep restriction. I’m convinced that long-term use of a sleep aid made my body forget its natural circadian rhythm. The hardest part is learning how to slow my thoughts and awareness enough to fall asleep on my own. Believe it or not, it doesn’t matter how tired you are, if you’ve forgotten how to enter the mental realm of sleep, you won’t be sleeping. Addition: I’ve been joking with my coworkers about my “early-onset dementia” for about two years. I started forgetting everything, which I attributed to stress. However, over the last few weeks I’ve not had any trouble with forgetfulness. Makes me wonder…

You should definitely see a psychologist who specializes in cognitive behavioral therapy for insomnia (CBT-I.) The one I saw had helped many people who were long time ambien users sleep naturally again. The sleep restriction is not all that effective on your own, but with support from someone who can coach you after bad nights and challenge your thinking, the sleep restriction begins working at multiple levels. Happy to point you in the right direction if you're curious - best of luck.

Other important note. In case of chronic insomnia sleeping pills are not the final answer. Instead you should go to your psychiatrist who prescribes micro doses of antidepressants. This is something you can take for a couple of months(as it`s not as strong as the sleeping pill), lean off and then hopefully your insomnia will be cured. Sleeping pills are almost like a band-aid for insomnia not the cure.

I currently live in a poorly built apartment with a lot of noise outside, and shaking from a large fan underneath my bedroom that makes it impossible for me to sleep through the entire night. I tried everything (blackout curtains, white noise, mass loaded vinyl, expensive well built bed frame, memory foam mattress, thick carpet with carpet padding, Loop earplugs, eye mask, etc.) and it all helped a little, but I still only get about 4-5 hours of sleep off and on. I take various things to help me sleep when I haven’t slept in a few nights - Tylenol PM, Benadryl, and/or Zyrtec. And I even have a Xanax Rx. But even then I still can’t sleep some nights. And forget melatonin. I have noticed that cutting out coffee entirely, and cutting out chocolate before close to bed time, really helps. This video was really helpful. My Dad was a chronic insomniac and he got Alzheimer’s. I’ve read that there’s a tie between insomnia and Alzheimer’s (lack of sleep) and I’m very worried about it because I’ve noticed that when I go 3 days in a row of more without adequate sleep I can’t remember common words for objects or people’s names.

He basically said you’ve been sleeping OK? Having problems waking up in the morning? “leading questions“ I’m gonna prescribe you Lexapro… literally nothing on my end to suggest I needed or was seeking help for depression or had any issues in that realm. I was actually going through something completely different but the doctor never even got around to finding out why I came in he just wanted to prescribe me that and high blood pressure medication. My blood pressure was high but he made absolutely no suggestions about diet or lifestyle or anything. He basically saw my numbers and said he’s going to prescribe me this and that. I would like to report him.

I'd love to see a video on the POSITIVE symptoms of metal health conditions in general and/or maybe follow up videos delving more into specific ones? Its easy to get bogged down in the negatives but there are some positive to neuroatypical thinking. Personally, I my ADHD, autism and BPD mean that; all emotions are very intense but in the case of positive ones it CAN be very rewarding, hyper-focus/ obsessions have made me very skilled/ knowledgeable in some areas, increases in empathy and abstract thinking often means people around me feel understood, an ability to be manipulative can be the best course for a positive outcome in many situations, and the ADHD or BPD stereotype of being "quirky" or "fun" does hold SOME merit. Other conditions also can have positives, some schizophrenic patients may live in a delusion that's peaceful and joyous. Autism can be linked to very high intelligence or proficiency in the case of savant syndrome. Neuroatypical minds tend to be more creative. And without psychotic symptoms many artists would not have had the inspiration to fuel their art, such as van gogh. I would love to see a video explaining why and how these positives can occur (perferably for each one [more perferably for each of mine]) And I think many people would find information on how to reframe their diagnosis' and utilize the tools that they can provide, to compensate for all the crap that they cause. (E.g. the percentage of ADHD in those that start their own business is higher than in the general population)

That comment is interesting which I typically do not say about many and you bring up some awesome outside the norm thinking. I think it still rides on where does the person fall on the graph...the straight line. If you can function in society well to say... very poor than you will not maybe fall into society norms (whatever the hell that is in 2022) or you will be high functioning and do well. I think most meaning all humans are just doing the best they can in their own world. Not easy at any age. You sound pretty together but you may infact be full blown psychotic...lol. Tried physical exhaustion, meditation, warm light reading, self-storytelling, chamomile, melatonin (even though it makes me feel awful and has never worked), binge drinking, cold medicine, pot and pot brownies, none of which helped, most of which made my psychosis worse. I later was prescribed Quetiapine, and even a nibble of 1/6 of a tablet could put me out, which seemed like a godsend, but even the tiniest dosage would feel like a 20s rager hangover the next day, so a double-edged sword. I eventually evened out a bit though I wake several times a night still (down from a dozen+) but I still Never wake up feeling refreshed. Cherish your boring sleep; some people just don't get it.

Hi Dr. Marks! Ive been watching your channel for a while now as I'm on my newly diagnosed bipolar journey. I originally had a diagnosis of depression when I was younger (18ish), then last year I was re-diagnosed with depression along with GAD - I'm 28 now. My psychologist officially diagnosed me as bipolar as she's been working with me for a year as she confirmed that I was in a hypomanic state and recommended I talk to my psychiatrist about starting a mood stabilizer. I'm currently on lamotrigine and it's doing great but they're removing me off of Lexapro as my whole treatment team has agreed that the Lexapro is what is throwing me into manic - however they've also confirmed that I have ADHD. My psychiatrist informed me that SSRI's can cause a lot of issues with patients who are diagnosed as bipolar often causing them to go into manic - which a lot of this stems from being bipolar hard to diagnose. Is this something you've seen in your practice? Could you do a video about the reasons why misdiagnoses happen so often for those who are Bipolar and why its so hard to Diagnose? Secondly, why some mental health medications can put you into a full-blown manic?

Hi Sultan. I can’t give you specific recommendations without doing my own assessment of you. In general though, if someone has a partial response to something, I augment usually with antispychotics like Rexulti or Abilify, maybe lithium. I also have the person get thyroid checked, vitamin D levels and make sure there are no other medical issues that could interfere (testosterone in men). ECT is more effective than TMS. Then there’s Ketamine. I would consider Ketamine a last resort though because there’s a lot of controls around taking it and we don’t know for how long it works
