Hey Dr. D, is there anyway you could make a video on how to track your ovulation without a device such as a thermometer? I have a copper IUD and I know only 6/1000 women get pregnant on it but I’d just rather have a second back up to make sure my partner and I use a condom when I’m ovulating. Thanks so much for the information!

I have a liletta iud and I love it! No period equals paradise,lol. The only possible side effect that I have is mild acne but my dermatologist does not think the iud caused my hormonal acne. Acne can be controlled with medication so I am not going to have my liletta iud removed. I am keeping my iud in for the full 7 years. 😀I edited my comment so I could say a few more things. Not every woman on a iud will get acne. Every woman's body reacts differently. I believe my dermatologist when she says that my acne is not caused by the iud because I have struggled with acne since I was a teenager. I had acne as a adult too. When I use acne medication my acne gets better.

My OBGYN checked my Kyleena placement after only 1 week after insertion. Every place I’ve read said it should be at least a month

See video #13 “IUD Removal”. Heavy bleeding after removal is unusual- I would advise anyone with heavy bleeding after removal to see their doctor for further evaluation. -Dr. Dervaitis

Hi Dr. Dervaitis, firstly I love your videos and they have been so helpful! Would it be possible for you to make a video about cramping please? I had Mirena put in 2 weeks ago and have had on and off cramping since, but I'm not sure what's considered normal and what isn't. Thank you! :)

Hi Nicole- I’ve not done a specific video on cramping because pain is so much more subjective (compared to say, bleeding for example) so it’s harder to quantify, and harder to make generalizations. It’s a challenge to provide information like this, as I never want to falsely reassure someone who may have a significant issue that requires medical attention (I do explain in my channel info and to viewers who comment that this channel is not intended to provide personalized medical advice). Generally speaking however- menstrual like cramps (which could last up to a week or longer) after IUD insertion, are fairly common. I generally recommend regular dosages of anti-inflammatory medications for these cramps (for patients without allergies or contraindications). -Dr. Dervaitis

Please answer! I got my iud today. It’s Kyleena. I went there with positive vibes I was trying not to think about the pain. I got there and at first I was going to get Mirena. There were two doctors. I knew how the process was going to go. So they first tried to measure my uterus. The thing to measure couldn’t pass through my cervix. They said it was too tight and they would have to bring a tool to dilate my cervix. I didn’t know that tool was going to hurt me so much. After that they said that they would have to do it 2 more times or try to insert the iud so I said that they can try to insert the iud so I can be done with it. Well fail! One of the doctors put the iud in and I asked it is it working? She said yes! I was like ok. Then he dragged the iud out he didn’t insert it right. Then I asked them what happened? And they said that they “arms” didn’t open. I was upset because after all the pain they couldn’t even put it right. After 5 attempts I was done, and I told them that this is the last try. The doctor went to bring another iud (the third iud) she brought Kyleena because it was smaller. She did it really fast and then she said ok, it’s done. I don’t know if I should be worried after all the attempts I don’t know if they hurt my uterus. They tried to insert it at least 10 times. I’m 18, and I don’t know but the tool to dilate my cervix hurt me so much. Now i tried to feel my strings, I can only feel one. Is that normal, should I be worried?

Hello Paula- I’m so sorry to hear that you had this experience, it is certainly not typical (the insertion process can vary depending on the experience of the inserter). Dilating a narrow cervix can be very uncomfortable (the fact that you experienced pain does not mean something is wrong). Multiple attempts at dilation would not usually cause any damage to the uterus or have any impact on your future reproductive health (or the effectiveness of the IUD). Bottom line- as long as the last insertion of the Kyleena was successful, it should be effective (2/1000 chance of pregnancy). It’s common for the strings to be so close together that they feel like just one string. I would be sure to attend any follow up appointments that were scheduled (don’t worry- they will just do a check and it won’t be anything like what you’ve just gone through!) and I usually counsel my own patients to use back up contraception until that visit (because of the small chance if the IUD falling out). Now it’s was more and the cramps got worse. Also I’ve been checking the strings, and the feel was longer, I’m going to schedule an appointment but should I be worried?

Can I immediately have an IUD inserted after nexplanon removal? Also how are they different? I know nexplanon is one hormone & the hormonal IUD is a combination of two but why is one of the side effects of nexplanon prolonged bleeding while the hormonal IUD decreases bleeding? (Bleeding = menstrual bleeding)

Hello- Yes, an IUD can be inserted at the same time as IUD removal. To correct you, both Nexplanon and the Hormonal IUD contain only one hormone (both are a form of progesterone only contraception). The difference is that with the hormonal IUD, the progesterone is delivered locally into the uterus to thin the lining, while with Nexplanon, it acts systemically and needs to travel through the blood steam to the uterus. They contain different versions of progesterone (IUD=levonorgestrel, implant= etonogeetrel). -Dr. Dervaitis

I have a question. I was recently diagnosed with endometriosis by laparoscopy. I was advised by my surgeon to get the mirena coil fitted while i was under general anaesthesia. I agreed and got the mirena coil fitted. Two general questions: I have read some worrysome comments that the mirena coil can cause pelvic inflammatory disease. Is this true? Also, can pelvic inflammatory disease be identified through a laparoscopy as well or is it just endometriosis? (im just concerned in case i already had PID and it wasn't picked up on). Thanks.

Hello- While there is a small risk of infection associated with IUD insertion (about 0.5%, and usually within the first 3 weeks after insertion) there is not otherwise any significant risk for PID associated with IUD use in the long term. Some signs of PID might be visible at laparoscopy, but it is not a definitive “test” to rule in, or rule out, the presence of PID. If you have concerns, talk to your doctor. -Dr. Dervaitis

My mother had breast cancer at age 40 and therefore I am at increased risk-- can you tell me if there are any dangers of having a hormonal IUD with this increased risk of breast cancer. Would a copper IUD be safer?

There is not consistent or compelling evidence to link the levonorgestrel IUD with breast cancer. Your mother’s history would not be a contraindication to using the hormonal IUD- even BRCA + patients (at known increased risk) are able to consider progesterone containing contraception (only in patients who they themselves have been diagnosed with breast cancer would we recommend against hormonal - while copper IUD would still be an option)

Not everyone will have “adjustment phase” bleeding, and some may enjoy actually very little bleeding or cramping after insertion. -Dr. Dervaitis

Hi doctor! I'm trying to find out if you can still see slippery, egg-whitish, fertile cervical mucus whilst on the mirena, or if that's a warning sign that it's no longer having effect? I binged watched all your videos and couldn't find the answer to that! Is it a concern if I start seeing fertile cervical mucus around ovulation time???

The majority of patients using Mirena still ovulate- so ovulation related changes to secretions can still be seen (it prevents pregnancy by a mechanism that has nothing to do with ovulation- it thickens mucous at the cervical opening and thins the lining of the uterus- see video 9). -Dr. Dervaitis

Hi, I was wondering if the IUD is supposed to regulate your period? I am on the pill and have regular 28 day cycles. I’m looking to switch to an IUD but wondering if it will cause irregular periods

The hormonal IUD tends to lessen the overall amount of menstrual bleeding, but it may not necessarily “regulate” that bleeding. Most women using the hormonal IUD still ovulate, and may still have regular menstrual cycles. Up to 30% of patients using the hormonal IUD will have almost no bleeding (they may experience spotting or discharge that is infrequent and unpredictable). It will depend also on what that patient’s cycles were like in the past (in terms of regularity...) when they were not taking the pill. -Dr. Dervaitis

Dr, I have a genuine question. With the chances of pregnancy being less than 1% with iuds, what in your honest opinion on all the RU-vid videos of women telling stories on how they conceived while on an iud?

They are the less than 1%. The chance of pregnancy is 2-8/1000 (depending on if using the hormonal IUD or copper) For every 2 (to 8…) women on RU-vid reporting their pregnancy with an IUD, there are 998 (or 992) who had a pregnancy prevented thanks to their IUD (but those women don’t happen to be posting that information on RU-vid ). Further more, if there are 2 women posting about their pregnancy with their hormonal IUD… There should also be 90 posting about their pregnancy on the pill. (9% chance of pregnancy with typical use) There should be 180 posting about their pregnancy despite condom use. (18% chance of pregnancy with typical use). Many patients are under the false impression that an IUD is guaranteed to prevent pregnancy 100% of the time (false- there is no 100% perfect method) and so they are shocked (and understandably upset) and possibly more motivated to share their experience online. -Dr. Dervaitis

I recently took vitamin bcomplex a week before my expected period to help with my menstrual pain I am on copper iud. Does bcomplex make my period come late? I usually on 24-26 days cycle. Its the 30th today, my breast feel tender.

I just have my Merina IUD last September 17,they said expect the bleeding but I still don’t have it until now..should I be worried?? We take the pregnancy test before inserting and it was negative..
