Thanks for posting this...really well said. I had ICL about 19 months ago and I was about -10 prior. My pupils were too large for Lasik. Turns out, they are probably too large for ICL also so I'm dealing with the halos and glare like you describe. I'm only free of the effects outside in sunlight. Otherwise, the light produced by a lamp or overhead light gives me the ghosting/glare effect. It goes away using the brominidine drops, but as you found, you don't want to use them every day. I just have to keep telling myself that it is a tradeoff.

i had icl surgery back around 2014ish. recovery was fast only a couple of days. the biggest side effect for me was seeing halos around lights. they would bother me when night driving and with the on/off indicator on many devices at home. i ended up putting masking tape or bandaids on the indicator lights. halos bother me on some tv screens but not all. no halos on tablets or cell phones. i still see halos now but not as bad as the first couple of years. night driving can be irritating but doable.

Hey Jeremy, thanks for taking the time to do these update vids. I should have watched them before i got evo+ surgery this week. My eye doctor said i‘d receive the perfect product to get rid of my astigmatism contacts but he never lost a word about the central placed hole and that it can lead to such nasty circle reflections. It will be perfect he said… Now 3 days after surgery my vision is already ultra sharp which is huge trade off now to weigh compared to the halo rings. Also getting up without bothering to put in contacts is super. Unfortunately cinema is my nr 1 hobby and i built myself 2 years ago a home theater just for that, the room is totally dark and thus i see currently around the large screen a circular glare shape - its not as bad as the LED headlights of cars, but visually a situation i can‘t accept. Also these circles of light are ‚wobbling‘ / ‚shimmering‘ at the edge if the light source moves, drives me crazy and i cant image how the brain can filter this out. In my home all ceiling spots are LED , and especially now where days are short, i see rings everywhere coming up. :-( I found a study on the web that said the halo rings wil get less noticeable after the 3 month mark but it was not assessed if its because of the brain or if theres is an other process responsible for the reduction (like the lens settling down more so that the edge of the reflection causing hole is less pronounced). I really dont understand that this disadvantage is played down by doctors / producers , especially for cases where you basically want perfect eye sight and get rid of contacts as main reason. I give some months time but highly doubt that it will get a lot better in my ‚cinema situations‘. The worst is me testing for circles with the iphone led backlight…. There i see 2 inner rings and at visual end a faint ring. If i move the smartphone it looks like an UFO approaching… omg Right now my pupil is still very tiny because the doc gave me some drops at the end of the surgery to contract the iris , so even in low lights the pupil is way too small. I‘ll def wait to see how the rings change if the pupil / iris work normal again, maybe it has a positive effect… I had to drop this frustration somewhere and hope you dont mind! I feel more people should know in advance what to expect. Have a merry christmas! Best, Andre

Totally get how you’re feeling with that! Once you’re no longer on post surgery it will become much easier to deal with the visual side effects. My brain has learned to filter out most of the noise, especially the rings in my vision. I mocked up a pic of what my vision looks like in low light : imgur.io/a/qTK8CK0 It’s become easier and easier with time. But the first 3 or 4 months was rough. And occasionally it can still be frustrating when watching some movies. I don’t regret the surgery, and I still prefer these side effects over something like the dreaded lasik dry eye. But it can still be annoying. I can relate though. I bought a Sony OLED the month before my surgery. Was loving how incredibly low light looked. And this completely gutted the magic. 😪 Merry Christmas! Hope things improve for you!!

@Xenom0rph
@Xenom0rph 2 месяца назад
@@deendeen9584 After about 5 - 6 months i got really used to the circle halos and even under dim light conditions my brain kind of adjusted and it's not noticeable anymore. If i sit in a complete dark room and there are only some light sources, then the halos will be very noticeable in the first 1 - 2 mins, until the pupil adjusts and then it fades away. I went to the cinema several times and i could totally accomodate, chill and enjoy. Personally i come to the conclusion that I'm satisfied and i would do it again as the positives outweigh the negatives. Also the post 1-year checkup with the eye doctor was spot on, he said it's a perfect fit but he also admitted that - my surgery being his first Evo survery - he had some lessons learned that he will inform patients first about the halo situation. So in a nutshell: Some halos in dim light as a tradeoff for full Comfort and perfect eyesight (100 / 100) is fine for me.

I'm now 16 months post op, and with each month that passes I think less and less about my vision or the side effects. I think my brain has nearly fully adapted to everything. It still is there, but I no longer think about it or notice it. Even at my 8 month mark I was still experiencing a steady improvement with each month that passes. The brain is a very interesting tool! Hopefully yours will continue to filter out more and more as mine has.

Hi, thank you for sharing your experience. I had the older visian ICL surgery back in 2018 when they did the iridotomy holes. That part was the worst. My vision was amazing after surgery but I ended up having a lot of inflammation in my eyes and eye pressure was up so my surgeon took the ICL out. I was so extremely grateful that it was reversible. I have been waiting for the EVO Visian to come out with the central hole to see if I would do better with the lens. I am -14 and -15 severely nearsighted. I am back in glasses and contacts and thinking of getting a consult again for EVO. I also noticed a lot of glare at night halos when driving… I hate driving at night now so I think that your eyes will eventually adjust or if it’s so bad you can take them out. 😊

Ah, I’m sorry you had to get your older ICLs out. :( I’ve heard stories that the iridotomy holes can heal up, causing issues with fluid circulation. So far, in about 5 months of Evo, I’ve had no pressure issues whatsoever… so I do think the design with the holes in it really helps a lot. Hopefully if you go for it again things turn out much better this time around. -14 and -15 sounds like you have some pretty thick glasses!

I do know I got a discount on my procedure because I was the first person in my state to get the surgery (after FDA approval) and they shared my story for marketing purposes. I got my ICL done 3 weeks ago. I got the glare and multiple vision when it's dim. When my kitchen light is off, the oven clock becomes 5: one original image, 2 additional in the right eye, 2 additional in the left eye. But when the kitchen light is on, it's only one image. With TV, dark scene does look less clear, but no 5 images unless it is very very dark. So I am still mostly satisfy with the lens. My glasses are so thick and heavy, and they hurt my noses and ears and I could not take them off.

I had just commented on one of your older videos and then just noticed this more recent update of yours. I got my ICL surgery in July of 2021 and I am still suffering with the complications. It's super unfortunate because like you say vision during the day is fantastic. But the halos and glares in low light are just brutal to deal with haha. I have the exact same thing with my pupils being dilated more than the lenses. I'm more than likely getting the lenses removed because after more than a year I just can't get past the vision disturbances. Especially after taking a flight to Salt Lake City UT to see some ICL specialists at the eye institute of UT. (they were highly recommended by several professionals) Basically what they said is there isn't much to be done to fix it other than getting them removed. I also had them explain to me about how the vision hinderances are causing my eyes more than normal strain and I have certainly noticed an increase in headaches and eye pain at the year point I am at. The hardest part in the decision is exactly what you say though. It is nice to not be fully reliant on glasses or contacts. But to me after a year of hoping for improvement I do believe that the vision complications are worse than where I was before.
@JeremyJensenTheOriginal
@JeremyJensenTheOriginal Год назад
I’m sorry that yours aren’t working out. I don’t blame you for considering the idea of removing them. If the glare I was experiencing was any worse than it currently is, then I’d be considering that also. My doctor, Dr Moshirfar at Hoopes Vision in SLC, has told me that the glare I’m experiencing will likely continue and my brain will just get better at filtering it out. And one of my doctors assistants told me that as I get older my pupils will not dilate as much as they do at my current age, so that could help decrease the issue. But given how bad yours sounds, I’d probably opt to lose them also. The glare annoys me, but it’s not nearly as annoying to me as having to clean my glasses several times a day, or dealing with contacts. The biggest thing for me is that the glare is annoying, but not a negative impact on my vision clarity.

Both in your vision stabilizing, and your brain adjusting. :) The LED camera lights that I’m using in this video were actually very hard for me to use during the first month. 13:57 This is what happened to me. I have glare from the iridotomy procedure, and you are correct that it is permanent. The glare is distressing enough for me that I decided to cut my losses and cancel the ICL surgery so as not to risk further side effects. I consider it the worst decision of my life. Others may feel differently even if they have the same outcome, but that's my story.

Dang, that really sucks. The side effects of Evo+ have been of little to no regret for me. The few things that annoy me are outweighed by the benefits. But the idea of a permanent hole creating glare for the rest of my life would be a huge concern also. It seems like that side effect isn't stressed enough by many surgeons.

@@JeremyJensenTheOriginal Yeah, it unfortunately was not mentioned as a possible outcome by any of the surgeons I spoke to. They all told me that the ICL was reversible without ever cautioning that the associated iris procedure was not reversible, which honestly seems like malpractice to me but what’s done is done. I also forgot to mention that I was only eligible for the Toric ICL due to my astigmatism, and there is no Evo version of the Toric lens as far as I’m aware.

You are literarily me lol. Same risk because of keratoconus, same vision issue of -7, -6.5. I also went for the surgery because I didn't want to feel helpless without eye glasses or contact in case of emergencies- not a very good feeling to be dependent on a tool for your vision. I went to a clinic at first who didn't do ICL and they recommended PRK and cross-linking. But then I sought the opinion of a clinic that did ALL types of eye correction surgeries and they recommended ICL. I do recommend for people who read this comment to go see a clinic that does ALL the surgeries so you can get a differential diagnosis instead of a place that will recommend the limited procedures they offer and sell you those.. I just had my ICL done this past Tuesday and yesterday. So far I do see the hallows, which is annoying and I feel like my eye sight is more blurry. I think this has to do with the steroid eye drops. Did you notice the same thing? Did your vision become crisper with time?

Just to anyone who sees this and wants as much reference as possible before deciding on surgery: I had a higher prescription (with astigmatism transposition my ICLs were -12 and -14), I got the normal 4.9-5.8mm optical zone EVO lenses (apparently the optical zone depends on the power). My maximal pupil size with eye drop dilation was 6.5 mm. I only get that sort of glare in very low light situations (the normal low beams on my car is enough to completely eliminate it while driving for example, even on an otherwise dark country road) and 99% I have no issues at all, but i notice like I said at the very low light end it's borderline. Otherwise interesting to follow your progress.

@@JeremyJensenTheOriginal Yeah it's great. Vision is better than I can remember ever having, by a big margin. Wore contacts for a while, but didn't tolerate them so well, and couldn't get astigmatism correction to work, terrible sticky eyes night vision, and glasses in that high prescription just suck. I haven't had a full visual acuity test since surgery, they only showed me the 20/20 (or 1.0 since I'm in Europe) line, and I had zero problem. Overall my vision feels like 4k now compared to before, and I'm blown away. Hope you get used to the ICLs more (and pupils tend to shrink a little with age, as you probably know.) Hello! I think you will probably have some of the same vision issues I've described in this video. No way to know for sure until after you get surgery. The good news is that my brain has learned to better filter out a lot of the noise, so it has been less and less of an issue. But the amount of filtering your brain does is the one thing you can't predict. It took about 6 months for me to be comfortable with the glare. And now 12 months later it barely bothers me at all. Usually a dimmed duplicate of the light source shows up directly below the main light source. 6 months after my surgery, still see them... though I'm used to it now and it rarely catches my attention like it used to. Best I can describe it is that you'll learn to live with the defects as trade-offs for the upside to your surgery, or you'll hate it so much that you'll get them removed. @JeremyJensenTheOriginal
@JeremyJensenTheOriginal Год назад
 @Razear no problem! And yeah, those things were discussed. After reviewing all risks this was what lead me to wait for the Evo lens specifically -- as the risk of complications seemed lower due to the better design for eye fluid circulation. And the Evo+ lens size would better help visual quality with my large pupils. All of the risks were low enough that I felt comfortable taking the chance with surgery, especially with a surgeon as qualified as the one I went with. And most importantly to me, assuming no surgical errors, Evo lenses can be removed without any long term consequences or permanent changes to the eye. PRK/LASIK are a one way street, and that honestly scared me more than anything. The risks are something every person has to consider on their own, and their unique eye structure. But my eye structure put me at very low risk of complication due to the amount of space inside of my eye to have room for such a lens.

This would be life changing, I can't drive because I can't see with my vision, doc says I can drive with my vision but I really can't see clearly, with evo it'll be life changing. @JeremyJensenTheOriginal Год назад
The 'lasik' subreddit is a pretty good community for Evo discussion. I don't have a Facebook, so joining/starting a group isn't something I'd do. My guess is that your Evo ICL will get you much closer to 20/20 than you've ever had, if not all the way to 20/20. The exam you have in prep for this surgery is the most comprehensive one you'll probably ever experience. There's always the possibility they don't get it completely perfect, and if that does happen you can still wear glasses (at a much lower Rx). The glare caused by Evo lenses when you have large pupils is a worthwhile tradeoff for how good your vision is going to likely be!

@JeremyJensenTheOriginal
@JeremyJensenTheOriginal Год назад
Basically, as my doc put it… these lenses will stay in my eyes until the day comes that I may require cataract surgery. So until then, if my vision were to get worse or change, glasses may be required. If my vision changes too much then they can just remove the lenses at that time.
@perfectomunoz

While you're on steroid drops it's gonna be super intense glare. Glare should get easier and easier as your eyes heal and you no longer require eyedrops. After that your brain should learn to filter out most of the noise. When I first got surgery the glare was something I thought about almost nonstop. By 6 months I barely thought about it at all. And now, after a year it crosses my mind at most for 5-10 minutes a day. It's incredible how much the brain filters out... I can't even see the circles anymore, even if I try to look for them, where immediately after surgery they would be visible on nearly all light sources. Now the circle's are only visible under very specific light sources. And double vision/glare only impacts me in certain low light scenarios, but is fine 99% of the time. Things will likely get better! It just requires patience, and doing your best to not think about it so that your brain can learn to adapt.
