The seven key trichoscopy features of trichotillomania. This video summerises the most common and the most specific trichoscopy features of trichotillomania (hair pulling).
It adresses the newly deacribed trichotillomania incognito.
The distinction between automatic and focused trichotillomania is discussed. I share my point of view, about trichotillomania which is often treated by the dermatologists and the type of trichotillomania which requires psychiatric treatment.

Hello professor and thank you for educating us in all possible ways:) I would like to ask you - sometimes patients come and say that they have itchy scalp and they scratch themself . In the examination I see alopecic plaques with signs of TTM. How do you approach these patients? Would you do some more examination (biopsy, patch tests, else?) to find out if there is really a reason for their scratching? Thank you:)

Thank you ;-) My first idea would be scalp dysethesia, usually also with brom hairs; also trichoteiromannia (hair rubbing) may give a similar trichoscopy image as trichotillomania ; in the first situation I would consider triamcinolone infections

I think that the terminology is still not perfect, because most persons with trichotillomania do not have a "mania". Thanks a lot for your comment!

Dear prof. Lidia Rudnicka! I have beard alopecia areata for 3 years. My doctor recommends tonsil surgery because pus comes out of my tonsils when she pushes it. My tonsils are bad for 3 years too. Plus I have a constant ear inflammation for 2 years. I have never heard about a tonsil surgery that solved alopecia areata. Do you know a case when a tonsil surgery solved alopecia areata? What do you think? Thank you! : )

This is complex issue. Any inflammatory disease (such as bacterial tonsillitis) may decrease or inhibit the efficacy of treatment. Tonsillitis may also be a contraindication for some treatments. However, treating tonsillitis alone is not likely to help the hair disease.

I have an office in Warsaw, but waiting time is over 1 year. I suggest to look up a dermatologist and ask at the front desk whether she/he is treating hair diseases. If not try another dermatologists. Dermatology is a very wide field and some dermatologists focus on being rather super-experts in a limited area rather than treating every dermatological disease

I have suffered from trichotillomania for 35yrs. Significant hair loss and worn wigs for last 20yrs. My scalp is so itchy! Pulling helps relieve this. Is there any relief for itchy scalp like this?

Maybe you have a disease that causes the itch (lichen planopilaris? scalp disesthesia etc). This should be diagnosis and treated.

I have very annoying wiry, pubic-like hairs at the top of my scalp. I play with them involuntarily. It is not due to damage as i never use heat. Do you know why they are like that and if I can do anything about it?

There are many hair structure abnormalites, so it is impossible to say, but playing with hair weakens the hair shaft and may influence the hair appearance

If possible, I suggest to try taking every dermoscope in your hand (or at least the one that you plan to buy) and check whether it is comfortable FOR YOU and you see well the whole field of view. In regard to handheld dermoscopes I was using Heine and Dermlite and I was happy with the models I purchased. My most recent one is Heine DeltaOne and I am very happy with it, but again, I recomend checking yourself before you decide

@@profLidiaRudnicka i’m sorry i meant does traction alopecia cause miniaturization? And if there is miniaturization due to traction alopecia is the miniaturization because of scarring that is happening or is the miniaturization process in traction alopecia a sign that hair is still alive and can be thickened again if traction is stopped ? (To sum it up is miniaturized hairs in traction an early sign before scarring happens completely?)
