Either rinse out then lightly brush new paste on, or floss and rinse first then brush without rinsing after. The flouride in the paste is crucial (even more so than tap water). @elhuesote @Monica Myers failed to mention that dentist rinse, yes THEN give you a fluoride treatment after. Mouthwash is not really needed if you have a decent paste. The dentist might give it to you to sanitize or neutralize your breath for procedure. I think people just have a BIG adversity to change, or to actual credibility that tests their habit they'd hate to break.
