we use epinephrine to prolong duration of action of local anesthesia as a sympathomimetic it acts on alpha-1 causing vasoconstriction which lead to decrease drainage of local anesthesia, and so decrease systemic side effects of local anesthesia as CNS&CVS side effects, and also used for decrease bleeding during surgery. this side effect is due to acting on beta-1 also beside alpha-1, and this can be reduced by using Levonordefrin which has little beta-1 acting or Phenylephrine which is pure alpha-1 agonist. Cocaine as Local anesthetic has exception for using vasoconstrictor as beside its anesthetic effect it prevents reuptake of NE and thus increasing NE by itself so no need for vasoconstrictor.

We add epinephrine as a vasoconstrictor with local anesthetics to increase their duration by delaying their entry to the circulation to be metabolised. Plus local anesthetics (except cocaine) act as vasodilators so their action would be brief therefore a vasoconstrictor like ephedrine would increase their duration. Thank you dr jae!!

hey, @medicosis, although you're great (I mean, spectacularly great), I rely very much on books too because, yeah, it is the right thing to do. Nevertheless, I am having a very hard time with the books and with a not so great professor about "pain", neuropathic and neurogenic. I wonder, do you have any video related to this topic? Disappointed there is no mention of the mother of anesthesia who worked with the Mayo brothers, Alice Magaw. Her research, clinical findings, and published work provided a new standard for safer anesthesia practices so that patients were not dying from anesthesia delivery. Before these standards were researched and developed, patients would die from anesthesia delivery which renders it pointless.
