Depression remains the leading cause of disability worldwide, affecting almost 300 million people, half of whom can't find lasting relief from drugs or therapy. But a new experimental treatment using a fast-acting approach with targeted magnetic stimulation, called SAINT (Stanford Accelerated Intelligent Neuromodulation Therapy), has achieved significant success in trials. Correspondent Lee Cowan reports.

“The chatter.” Ah yes - that pesky chatter. It wasn’t until a few years ago that I suddenly decided to change the phrasing of how I acknowledged my depression. I stopped saying “I am” depressed, and started saying “I have” depression. I have an illness, in the same way someone has diabetes or Parkinson’s. However, no one says they “are” diabetes, so why should I say I am my disease. Once I stopped recognizing my depression as a personality trait and more of a genetic disorder, I took away some of its power and also some of my shame. When I feel depressed now, I can treat the symptoms of the depression without allowing “the chatter” to convince me that I am broken as a person and need to be punished.

When I hear chronic depression from a young age to age 61, suicide attempts (suicidal ideation), and anorexia (eating disorder), I hear underlying Cluster B disorder. Have you ever had Dialectical Behavior Therapy (DBT)? Have you ever heard of a treatment center called Harmony Place in Monterey, CA? Dr. Schwarz specializes in treating eating disorders, and talks about how some of his anorexic patients ask him to please not take away their anorexia. People forget the "nervosa" part of anorexia and that it's an anxiety-based issue. If I had to randomly assume/guess, I'd be willing to bet you were raised by a narcissist(s), and that because of your parent(s) you developed an attachment disorder that is the root cause of all this suffering, including your depression. Now, full disclosure, I didn't come up with that myself, it's something I learned about these "disorders" when reading some of the research and looking for specialists with success rates, as so many "experts" fail their patients, which must make them more woeful, hopeless, and/or anxious. Harmony place specializes in treating eating disorders (and many other similar and/or related disorders), but they focus on the underlying attachment disorder with the parent that causes the disorder to develop in the first place, not the anorexia nervosa....or depression....or suicidal ideation....or if there's any chemical dependency, etc. (or whatever the issue may be). Almost no one focuses on the root cause, instead they just write prescriptions for people that keep them dependent (and sometimes broke) on pharmaceuticals. Dr. Schwarz has been working in this field for decades and was most frustrated by the rate of relapse he was seeing, which is what prompted him to look for another way to approach this (and that is addressing the root cause). People "relapse" because no one is treating the root cause. If you have any interest in listening to him talk about this, here's a link to just one of his videos on eating disorder treatment harmonyplacemonterey.com/treatment/eating-disorder-treatment Here's a video of him talking about his approach to treating depression (unipolar treatment) harmonyplacemonterey.com/treatment/unipolar-depression/ Please feel free to ignore this entire post if you want to, I just saw your comment and wondered if you've ever really encountered a specialist who treats the root cause of these issues instead of just writing you a prescription and tells you to be content with that. Wishing you well on your path to finding a treatment that works for you. No one should have to live like you have for 60+ years.

I worry that, for many patients, expensive treatments will remain out of reach. I know that when I suffered from Depression, years ago, my downward spiral led to job loss, which led to a loss of insurance, which, eventually, led to great difficulty in receiving treatment and affording the medication that, eventually, really worked to ease my symptoms. The condition had a cascade of effects across my entire life, but it was my finances, perhaps, that were hit the hardest. It completely derailed my career trajectory and prospects. Depression isn’t just mental. It’s physical . It drains the life out of me. I would have no energy to even get up to shower. I can’t look at anyone, talk to anyone. I would just want tp hide under the cover in my bed for days. And for those days, the chatter would convince me I’m worthless and I’m a failure and this world is better without me. It’s a horrible horrible condition to have. I don’t wish it on my worst enemies. The last thing you want to hear is “ cheer up, just don’t think about it”. Or “ there’s tons of ppl worse than you “. If I have a penny for every time I’ve heard that, I’d be rich by now. But it truly doesn’t make someone with depression feel any better.

@@Chad_Max Clinicall depression is a medical condition, it can literally be seen on scans (as shown in this video as well). You have a right to decide not to be with someone who has it, but please don’t lump all people with depression together. Some can be helped by changing their lifestyles, values etc., but clinical depression needs actual medical brain treatment, whether through medication or procedures such as these. Also it’s rude to call people with mental health issues “whackjobs”, because it can easily happen to all of us. For instance, severe depression can be a result of traumatic brain injury, which you can get by slipping and falling on the street, being in a car accident, etc. So be mindful that you could be a “whackjob” needing this treatment one day

I am stressed out from all the mental and pain that my parents have given me, living with depression, I have came to know the difference between sadness and depression, depression doesn't only last for a couple of days, it's like a feeling that is very different from sadness, in depression, you don't want to die, but you just want to like "unexist" you want yourself to be erased from everyone memories and the existence itself, you think "what is the purpose of me, what am I supposed to do with this life", I am still trying to not "suicide" due to religious reasons although I get many suicidal thoughts. To all of my brothers and sisters suffering from depression, stay strong don't ever, I said don't ever think of suicide, you have to overcome it, you are a warrior. You are not just a "regular human" you are a warrior suffering from this disease that doesn't exist but you suffer from it. Stay strong.

@catc8927
@catc8927 2 года назад
The pulses in TMS are far stronger than anything your phone or other electronics can generate. TMS pulses are calibrated by seeing the level that causes the patient’s thumb to twitch. Most of our devices can’t emit enough power to do that. If sending a text doesn’t make your thumb twitch, you’re not getting enough electromagnetic energy from its transmissions to seriously affect your neurons.

This is a wonderful breakthrough and I'm so happy for these people. I have never been diagnosed with depression, but rather OCD, and it has been at times quite debilitating. I think I'd give almost anything to be free of it, but then I also fear I'd lose something; creativity, ability to remember things...stuff like that. For me, a treatment is almost "be careful what you wish for," but at least I'm lucky enough to not have it so bad where I can actually consider these things. My heart goes out to everyone suffering from any kind of brain disorder. It SUCKS, and I hope we all get better someday soon.

Maybe it is approved and considered for ocd too by now, or at least also in studies. Is ocd being located in certain brain structural locations according to brain activity by now? Though they are also putting hoarding disorder into a separate category and maybe more than one discrete type of ocd. NYStatePsychiatricHospital was doing trials with the transcranial magnetic treatment some years ago and recruiting patients with OCD spectrum diagnoses under anxiety disorder clinic there. I read the material for informed consent, but decided not to participate because of taking CAT scan for baseline readings. I thought of cost benefit ratio with unknown benefit. Think if in extremis of imminent life threatening suicidality it would have been more clear with "what have you got to lose?"

I too have OCD, along with depression/anxiety i manage basically on my own… I’ve found NAC (N-acetyl cysteine) to be very helpful for my form of OCD (dermatillomania). I take a high dose, 2-3000mg/day and i try to be consistent but i miss days sometimes & do feel a difference. Ive also found Inositol in higher doses than the label recommends, to be helpful for anxiety… which is just so exhausting to live with i think it can trigger/exacerbate depression, it can also cause insomnia & sleep disturbance or deprivation is veeeeeery similar feeling to depression so its also not helpful! I rely on a LOT of herbs really, cannabis too, i think i used to rely on that too much but diversifying with other herbs & supplements helps me manage & keep things more in balance these days. Valerian is fantastic for sleep. Knocks me out! Lol. EFT (emotional freedom technique) aka tapping therapy has also really helped me for acute anxiety or stress in the moment. I also just invested in some Loop earplugs, to help me with noise sensitivity/anxiety/sleep. The “experience” ones are great for the car to cut down on the tiring road noise but still be able to hear important louder sounds like horns. They’ve also discovered there is a definite link between the microbiome/our gut bacteria, and our mood & mental health. Its important to get both Pre* & Pro- biotic foods/drinks in your diet. My personal favorite is Kombucha, Aqua Kefir, or Jun Kombucha, they’re like natural anti-depressants, and they’re fun to drink so it helps give me an alternative to wine 😉which I do love, but you reeeeeally have to be careful & alcohol is NOT recommended for depression, a hangover is like giving yourself a mild case of depression lol. There is also hard kombucha with a slightly higher alcohol content I find such a nice in-between, its relaxing, but not so strong it gives me a hangover or exacerbates depression, it’s really nice for summertime. I also randomly wish Bars would make it just normal to have Kombucha and/or hard kombucha on tap- for the non-alcoholic friends & to just give people a healthier alternative that doesn’t harm mental or physical health while still allowing people to be social. Anyway, i hope maybe some of these tips help, i believe it was a youtube comment section on some mental health/OCD related vid where I originally learned about NAC & the Yale study on people with trichtotillomania (basically the sister OCD to what I have, dermatillomania). You can look up ‘Yale/NAC/trichtotillomania’ study & surely find it, it should pop up 👍 good luck! 💜 🙏✌️

@Roxanne Mitchell According to published studies in scientific journals, transcendental meditation seems to have the same mechanism of action as this device (PFC and ACC stimulation). Transcendental meditation is sitting down with eyes closed and repeating a mantra for 20 minutes twice a day. One of the common mantras I believe is "Raam", which in traditional Indian cultures is used as the name of God. I mention that in case you want to use a more culturally appropriate mantra for your culture. I believe with practice you can use the mantra for all your waking period, what is referred to in Indian text as "every breath and bite". Both TM and this seem to have the same mechanism of action, the difference is this SAINT therapy has a bigger carbon footprint, requires no effort on the part of the patient, is relatively instantaneous, and is price prohibitive for poorer regions of non first world countries. In the tm when you notice a thought, over time you start to recognize it as just a thought and not a reality, and drift back to repetition of your mantra, your tool against depression. The mantra is designed to eventually replace your thoughts during meditation. The paper on tm is Mahone (2018) "fMRI during transcendental meditation practice." The paper on SAINT therapy in this video is Cole (2021) "Stanford Neuromodulation Therapy: a Double-Blind Trial" I don't wantbto sound like I'm trying to diminish their great work, because it does seem like a really great, lifesaving invention!

This made me cry so much. Im recently in therapy and i've basically been almost always if nit always sad all mh life. Mh therapist Said that i probably been deling with chronic depression and just knowing finally that theres nothing wrong with me as a person. Trauma and life circunstances change and alter the brain and body is such a relieff to know that there is nothing wrong with u, your brain unfortunally was developed differently and it is not your fault, there is hope. There is always hope and i believe and i will continue to get better ♥️ i wish u all the best from the bottom of my heart, i really do. We got this ♥️🌸

I don't want to hold up this avatar that you've created in the world. It's too much for me." Deep rest. Your body needs to be depressed. It needs deep rest from the character that you've been trying to play... My father just lighting up the room or wrapping everyone in a story and going, oh, that's the thing I want to do.. My father could have been a great comedian, but he didn't believe that that was possible for him, and so he made a conservative choice. He got a safe job. When I was 12 years old, he was let go from that safe job. And we had do everything we could to survive.. I saw his love and humor and how it altered the world around me, and I thought - that's something to do, that's something worth my time.. one of the worst parts of being bipolar is that you never know when you are going to fall into the bipole hole [of depression] again. you know that you can easily fall into that depression hole again at anytime. even when you’re having “good” moments, you don’t know how long it’s going to last. you also don’t know how long or short your depression episode or those types of thoughts are going to last. it can be anywhere from seconds or minutes to months, years, or decades.

The fact that it requires an MRI prior to treatment, means a couple of things. First it won't be cheap. Even with good insurance an MRI can cost you. Second unless you are suicidal and getting this treatment in a hospital setting, means that it will require a specialist with a ton of consults before its considered, which also means it won't be cheap and so if treatment is successful then you will be happier but now ridiculously in debt. This will get incredible pushback from big pharma that rely on medication to treat depression and other mental health disorders, so you won't see this in your lifetime as a option for less wealthy people. Lastly based on the way people and government view mental health, this treatment will be far down the road, and the likelihood that the average person who needs it will get it is unlikely, unless you are affluent like the women of the study were or knows someone who can get you into the study. Lastly most insurance plans for average folk don't cover mental health needs sufficiently anyways, and with the cost of this kind of equipment, there's no way the average insurance is going to cover this. Lastly at least for one woman of the study required follow up treatment? Even more cost. While there's no hope for me or any of you watching this in our lifetimes, I do hope at some point in the future this can help to catch depression early and save people's lives.

This is not an argument against what your reply but here is my experience: I am a Vet with Depression+PTSD. First I have Medicare and have had several MRIs the co-pay has been between $60-80. I have participated in several studies a couple of which are FDA approved treatment. I am currently in another clinical trial. The only way I could get Medicare to pay for the treatment was to enroll in the clinical trial. If I had sought out and paid out of pocket for the treatment it would have been 15K-20K excluding monthly adjustments. I say all this to get across to people who feel like they want to try a treatment or feel like the treatment they are currently on isn’t effective. When you have the energy (seek help) there are many people out there who will help get you where you want to go. Don’t let anyone discourage you, even your doctors. I found all those clinical trials by myself, my Psychiatrist did not suggest but neither did he discourage me from enrolling. Please stay alive your cure is either out there to it’s on the way.

@@Dharmarenee I understand what you are saying and agree. For some (Vets) and people who manage to get a good job with good insurance, then it might be manageable. In my 30 years in the workforce I have never had insurance that covered or reduced the cost sufficiently that I was able to afford it. Only when I have been unemployed and was able to get on state coverage was I able to get the treatment I needed (not specialists but basic therapy and some limited access to meds.), but the moment I get a job I am unable to keep state insurance and its a giant seesaw. I'm glad you are able to get the care you feel you need. I cannot and its killing me.
@Dharmarenee
@Dharmarenee 2 года назад
I maybe left out a vital part even though I am a disabled Veteran I do not seek care in the VA system. I can no longer work so that makes me eligible for Medicare. The VA is helpful if you are in a crisis but long term follow-up and management is sorely lacking. I promise you I am no way wealthy and all the studies were free and most provided an stipend. There are a lot of things happening in mental illness treatment right now, there are treatments in clinical trials(I won’t discuss them in this forum)that are literally mind blowing. The concern with big pharma may be valid but “everything old is new again” and what was once outlawed is now being used again. You seem like a very savvy and determined fellow I bet you could help in the struggle for people to find and afford the treatment they need. I will cling to hope for my part and share my experiences with others and never accept that something is off limits to me because I am “too” this or that. Like I said I have tons of experience in a lot of treatments, I don’t dwell on hearsay and conspiracy theories.

It is alright to be concern or not know about what happens after this life. Fortunately there's a way to not suffer, not be in danger, not be poor, not get hurt, not have to work in vain, not have to feel any kind of negative vibe, and not to get tired anymore. Imagine you will be invincible, immortal, and holy with a new, upgraded body to the core! Imagine everyone around you will be the same as you. Isn't that a delightful thought? It's true you or anyone can have eternal life. All you got to do is to accept Jesus Christ 🙏 as your Savior! Once you do, He will let you into heaven during your day when you finally see Him!
