How I Do it.
Not all ACJoint separations require reconstructive surgery. In many cases high-grade AC Joint separations will cause abnormal clavicle (collarbone) motion as well as an unsightly bump. These injuries are due to falling onto the shoulder. Very common in cyclists and in mixed martial arts athletes. This is one of many methods used to reconstruct a chronic AC Joint separation. In most cases the patient can expect an excellent outcome both from the functional and cosmetic standpoint. ________________________________

My surgery was pointless. It popped back out when I returned to full activity. If you can live with the pain do it. Surgery only makes it worse.

Im a kitesurfer with a bad separation. How strong is this long term. I also work physical labor with my arms every day. There doesn’t seem to be an easy solution. Injured two days ago…

Hey Dr. Mora, nice drawing! You’re an artist! I had a question for you. I incurred a bad grade 3 separation back in june of 2012 and a family friend of mine referred me to an orthopedist who had performed rotator cuff surgery on their daughter and ACL surgery on their son. Unfortunately, I now know that the doctor was not an AC joint specialist. He strongly urged me to undergo a modified weaver Dunn surgery with CA ligament transfer and clavicle resection (8mm). Being as how I was 23 at the time, extremely active and knew nothing about the AC joint, I naively agreed. Had I known that newer, better surgeries existed, I definitely would have opted for one of those or would have just gone the conservative route. I was operated on just 5 days after my injury so I never had a chance to try PT first. Benign as how I was really young, strong and fit pre-injury, I now think I would have healed nicely without surgery. I did extensive PT for many years after the surgery and, although my shoulder is functional, it’s far from perfect and I cannot engage in the same sports and activities that I used to (I.e. water polo, volleyball, pull-ups, etc.) I was wondering if you think a revision surgery is possible and if it could improve my shoulder strength and function? My surgery only involved reconstruction of the CC ligaments. Is it possible to reconstruct the AC ligaments and capsule after resection has already occurred? Any info or advice you can give me is much appreciated. Thanks for your time, Cameron

Cameron Brinkman Hello. Revision might be possible but it’s more important to understand reason for dysfunction. If ACJ unstable than a revision is an option. I would start with comprehensive evaluation. Don’t assume ACJ instability.

Devansh not sure. My xrays and MRIs show a number of the things that could be causing pain and dysfunction

Can sutures break over time as you age wear and tear and if you get into a car wreck could it stick up again and break?

Is any of the clavicle sawn off in any type of surgery's? And what if it is a grade 2/3 separation

Does the clavicle migrate up or does the scapula droop due to the loss of the supporting AC/ CC ligaments? As well, if you had a DCR, what effect does the lack of bone on the clavicle (another support point for the joint) have on the efficacy of this procedure? Asking for a friend...
