I'm sorry I can't discharge the patient because beneficiary travel wont respond to my consult after 3pm. Also I have no idea how to get the patient's insurance to approve for a medical bed

This proves that there's plenty of drama and tension without the need for OTT unrealistic events, inappropriate staff conduct being normalised, or any amount of surprise lupis. A+, would watch a real show like this and love every second

...no. You're not special because you see that a bad thing is bad. Everyone even vaguely aware of how hospitals work in the US knows that it's bad. And just in general, every thought you've ever had, likely, has been had by at least 100 other people.

My medical transportation requires two business days' notices as well, but they make exceptions for urgent care, hospital discharge, and they'll make special exceptions for other appointments that aren't specifically to urgent care but are deemed urgent.

Reminds me of watching the TV 1:30 show ER where every emergency was smooth and carefully coordinated. When in my reality we were all internally panicking and dropping stuff. I really enjoyed the boring days when not a lot happened.

My mom does ultrasound and took call for years and sometimes woke me up at 3am to go do echos in the ER. We couldn’t do anything too far away or go to movies in case she got called in.

When I had a central line infection a few years ago, I checked into the ER at midnight on July 1st. I love being a patient at a teaching hospital... but not on July 1st 😂 it didn't help that that night coincided with the once-every-five-years hospital-wide computer shutdown!

this is why i love scrubs. the show def has its problems, but its drama isnt so much within patients with super dramatic conditions and more within the politics of the hospital and it's staff. although the sleeping with people in the on-call rooms is still ever present no matter what show u watch lol

This is the reality that ER and Grey's Anatomy and all the others miss! 🤣🤣 Just a note - the preferred nomenclature for an echo tech is a technologist, not a technician. At least in the US, the preferred term is technologist for radiology techs, as they have the education and expertise to use the machinery on patients. Technician is used for those who work on the machines themselves. This distinction came about in the mid-to-late 1900s as the field grew and developed into what it is now. There are still places who use the term "technician" for an echo/x-ray/US/CT/etc tech, but that is old-fashioned. It is a sticking point for those of us in the field and many do not appreciate the out-of-date term.

I don’t think it’s going to go over lol But tech is the term so u can pretend they’re meaning technologist lol Why does a nurse get to be nurse And physical therapy assistant get to be assistant when they have no more study time than u- all 2 yr degrees Someone needs to standardize healthcare. But it won’t matter, the new terms would never stick lol

Could be an honest difference in terminology between specialties. Technician and technologist are the difference between an associates and a bachelors degree in the clinical lab science. Field technicians repair machines. Lab technicians are still lab specialists, just like a nurse with a two year degree is still a nurse, if they're certified.

@eininw It's not, though. As I explained, it is the difference between an out-of-date term and the proper one, in the field. Source - I am a radiologic technologist and intimately involved with our professional societies who emphasize the nomenclature for the profession.

I understand that it's not the same meaning and that it's worth informing the content creator. It is definitely an honest mistake though, i.e. a mistake that any average person could make. Annoying, but unsurprising.

I remember when I was a new nurse and how my nerves would be calling the overnight resident at 3am for something.. serious but not life-threatening..... Then, watching the resident have to call the attending and wake him up at 4am😂

The hospital cafeteria closes at midnight! This must be pre-Vovid ... our cafeteria, along with all the restaurants in town are only open minimal hours ... like 1.5 hrs at Lunch...and 1 hour for Dinner!!

Back in the stone age, a rotation through Phila. General meant hot food 24/7. But getting to the cafeteria could mean having to take the underground train that ran among the building.

80 hours a week is fine, but 36 hours at a time is not in the U.S. Longest allowable by ACGME is 24 hours with up to 4 hours afterwards for transition of care or resident education.

​​​@@rambam23Is that a recent change? I'm pretty sure it was 36 hours not that many years ago. Personally, I'd be delighted if they made it sane hours! They have proven over and over that being sleep deprived is equivalent to being somewhat drunk. (I think it was about equivalent to the legal limit, but I'm not positive. It was several years ago that I heard about it.)

Dumb question: isn’t it kind of a relief to have a patient who can’t get home due to the discharge thing? They dont need care anymore technically so it’s like you have 1 less patient for the rest of the day and less work

I don't know the whole picture but the patient is taking up a bed, so another patient has to stay in ER for an extra day with no sleep and uncomfortable stretcher. The patient in the unit has to pay for an extra day, more fight with a insurance company. Extended hospital stay looks bad on papers and decreases patient's healing and morale

They're your patient till they leave the hospital even if they cant discharge till the next day. You still have to care for them.

@reginabillotti if anything happens to the patient, it's hospital's liability and it would be looked as abandoning a patient. So no. If the patient is alert and oriented and can walk on their own, they can leave as soon as they are discharged

So, I carry a pager (it's not a real pager, it's an app, but we call it "a pager" informally), but I'm nowhere near the medical field. When I get paged, it's because [employer] is about to lose a bunch of money, not because anyone is at risk of dying. Serious question for y'all doctors: Do you have standards for what is and is not a pageable event? Our standard is "customers are having a noticeably bad experience and/or [company] is losing a lot of money really quickly." Obviously you can't just translate that directly into medicine, but you don't want people getting paged for every little thing. By our reckoning, getting paged more than a few times a week is considered really bad. But I know most US hospitals have a serious staffing problem, especially for nurses, so I imagine your standards are going to look different from ours.
