Previously submitted category certificate was on the basis of correspondence I was unaware of the fact that it should be made on the basis of permanent address..can it be amended now??

Mam , in the attestation form, question no. 12 k - mentions either withdrawn from any training institution under GOVT. or otherwise? I have left pg training in govt hospital last year due to personal reasons, so must I mention here in this column or it is not needed? Plz reply mam, Thankyou

Because In warning on first page, it is written as *suppression of any factual information in the Attestation form would be a disqualification and is likely to render the candidate unfit for employment under the government.* So I am scared, Please Guide Me , ThankYou mam!
