On April 11, 2018, Boston firefighters operating at a second-alarm fire in Mattapan had a scare as the ladder on the spare unit Tower Ladder 10 was running failed. Companies were on scene for over four hours after the all out working to bring the ladder down.

It was the second time something like this had happened. I believe Demonracer2 got a New Jersey Pierce that had a similar issue.

Aerial Ladder and Tower Ladders are cantilevered structures, and are NEVER supposed to make contact with a building. In my opinion, this incident was caused by either hydraulic fluid system failure, or, I hate to say it, firefighter error.

The department where i live had the same thing happen to there Pierce 100' rear mount platform but it was from operator error during a drill

Just by looking at it, I'd say it happened because someone lowered the end down onto the roof. There's really no other way that 2 sections would pop up like that.

Cylinder seals, fitting leaks, or the line to the control valve leak would all result in the possibility of such a failure. However if it was a leak-down issue which isn't uncommon in hydraulics, it may have just sat itself on the building. The ladder is essentially a giant lever and fully extended like that it probably only put about 1 ton of force on the building (not a big deal for most structures like that). I don't know about those particular hydraulics, but unless the unit could go directly to 90 degrees, there'd be no reason for the system to pressurize the lowering functionality of the boom unless it's capable of going straight up which in light wind it'd need to be forced down ever so slightly for gravity to do its thing. Knowing practically nothing about that unit, I'd assume that it was capable of pressurizing the lowing section of the cylinder and breaking / de-tracking the ladder just as we saw it on video. I know the newer units are significantly smarter than the older ones and would most likely prevent such a thing from occurring. Seeing how this was a spare and due for retirement, I'd put my money under operator error. W/ out a computer and sensors it was more than happy to comply with an un-safe user command.

My armchair investigator's eye sees that the lower section of the ladder is at a low angle, pointing to the middle of the building, while the telescopic sections are are at progressively steeper angles up to the roof, suggesting that either the hydraulic rams holding the ladder up failed or the ladder was lowered while the tip of the ladder was obstructed by the building, lifting the telescopic sections out of their tracks. Lucky they didn't lift out completely and slide all the way down.

FNG got his fall protect hooked on the boom lever. That ladder did not fail. He raised it as the crane operator was hoisting, and lowered it as they went to the ground.

They said peirce is one the best and strongest long lasting truck manufactured. So why did passaic tower ladder did the exact same thing at a fire last year ?

Josh Smith Hi Josh. Pierce does have a strong reputation, but I can't speak for the recent failures. I'm on a truck company and Pierce has been reputable in our department. These trucks will only be as good as the operator who runs them and the preventive maintenance they receive. I imagine with city budgets being what they are, this spare probably didn't get the attention it needed. Once again, it could have been the operator as well. We rear-mounted turntables, so I don't have much info on mid-mounts. I hope this isn't a known defect.

Piece ladders are not the best or the safest. You want a strong, safe dependable ladder, get an E-ONE. They have never had a ladder failure.

@@williammcclendon9248 I’ve learned a long time ago, never say never! This wasn’t an hydraulic failure. This was a human error! It’s fully extended it snapped welds that normally protect it from bending. And when it falls fully extended, it’s going to snap welds! It should have no issues with extending the ladder and bucket. But look at farm implements, used every day, and they’re replacing hoses every year on some equipment part of is because they let the hydraulics down. Hoses get old, and wear out. There’s a point that should never get past, but I’m surprised they were able to get it up and off the roof. I guarantee someone had to change their shorts after this! I’m betting it was human error or the hose blew out, which would put xtra force on the welds. I also know those end buckets they’re heavier than hell! Add two people and things will break. Do ya see these end buckets in the FDNY? Not a chance in hell. It’s because your putting over 1500#s on an end of a ladder. What could go wrong with that? Everything!!!

I was at Pierce a few years ago and saw a ladder truck from BFD that had broken. It was BFD operator error on that one. This one looks like the same type of incident. Tower came down while bucket was on the roof. Why? We'll see.

Just think about the firefighter in the basket when the ladder separated. At least it was leaning against the building and didn't fold over.

Unless there was some sort of of way to manually unextend the ladder portion of it, what they did have to do after the video ended, take the sections apart??????

I believe it was able to be relinked and retracted, whether by hand or by hydraulics I'm not sure.

The question I have is...why did they not remove power from the tower ladder's boom? Or is it in a protected covering? I ask this because of pinch point training I have had at a lumber mill.

It was a two-alarm fire, out relatively quick. This was definitely the bigger issue. Yes, they changed over to a spare KME ladder on the scene. Thanks for watching!

Was an older model Pierce Dash Mid Mount Tower That failed in Passaic Nj believe it was also the first ever delivered in Nj Just Replaced by a Spartan ERV Mid Mount Tower Ladder

I wonder though, if someone pushed the lever to lower the tower and it hit the building. The ladder might have succumbed to the forces of it's own hydraulic system. Then again, this is only speculation. Thankfully nobody was hurt!!!

Actually they do, one failed in the UK some years ago at a fire station and killed some on lookers, also the make "Merryweather" had to issue a notice to all fire brigades to stop using their turntable ladders as during an inspection 1 was found to have a crack in it. Now if you really want to look at junk try the Camiva ladder, one I knew of failed often and spent more time off the run than one it also liked to "Walk" when the ladder was fully extended....

And also Metz ladders aint as good as they seem, seen one where the ladder hit the building and get stuck in the window it broke, the anti impact device failed on it then a few months after same appliance the ladder fully retreated whilst on a job, luckily no one was on the ladder at the time....

When they swung the ladder after the crane was attached, did the truck and crane have to coordinate the movement, or was it all crane and the pivot point on the truck was free to rotate?

The BFD Motor Squad looked like they were inserting something into the turntable whenever they moved it, so I'm thinking there was coordination. I was mostly focused on watching the ladder snap back into place though.

Makes you wonder how the ladder "collapsed" in the first place. If the sections snapped back into place, perhaps as suggested in other posts it could have been operator error; pushing the controls to lower the ladder instead of raising it. Perhaps the basket was resting on the building and the operator manipulated the controls to lower the ladder thinking he was raising it. When the ladder didn't move, perhaps the operator kept manipulating the controls because he wasn't getting the desired result. Kept pushing down until the hydraulic pressure caused the ladder to buckle. Speculation of course. Just interesting how the sections snapped back into place as the ladder was being lowered.

Pierce gets the booms from another party. Pierce just does the bodies and installs equipment and mechanicals

I was on scene where it happened to one of our leaders. Our operator who was a bit on the bigger side manage by lining towards move the control in the down position. Massive dent in the building roof and wall and the leader was looking exactly like that. Ohh ohh

Everybody is fine; there was a firefighter in the bucket from what I've heard, but they hopped onto the roof.

NOBODY WAS HURT BECAUSE IT WAS SET UP! it was a set up to get a new machine, that's all... THESE ARE MAINTAINED DAILY AND SHOULD NEVER HAD ANYTHING LIKE THIS HAPPEN TO IT.....

Tower ladders are extremely heavy beasts and after working with a couple in the area I can find no reason to have one. They are HEAVY, OVERBUILT, EXCESSIVELY LONG, EXCESSIVE REAR OVERHANG, and SHORT in terms of a good working length. Without being on the fireground and directly involved in the investigation, just looking at the wreckage it appears to be Inattention to the proper setting of the aerial device by the operator. Probably trying to firmly test the structure while lowering the ladder assembly. And the ladder did what ladders do when the tip is set but the rest of the ladder continues down, it started to disassemble itself. Now, from a fire mechanics point of view, if the ff had stopped immediately when the disassembly began and reversed to begin lifting the ladder , the situation would not be the state it is. Technically, the main section of the ladder should be able to lift and cautiously gather the ladder sections back in place more or less. Retracting the aerial could be a challenge BUT the ladder could be carefully swung away and lowered to the ground for intentional disassembly and return to the shops. Most ff's dont realise that the ladder sections are raised and lowered by a wire rope system that the spool which does the raising/lowering is what they take their first step over climbing the ladder. There are no tracks or clips to hold the sections down. EXCEPT on some of the Tower Ladders, its the nature of the beasts manufacturing. This is a Pierce unit and isnt built to severe use as others are..

It's all good. However I think this serves as a good object lesson on the importance of clarity when commenting, and even more so in an emergency situation. Not being negative here simply taking advantage of the situation to address a point many RU-vid commenters miss.

I have to say this but BFD has a horrible maintenance record mostly due to not having enough trained mechanics on staff to maintain their equipment. A few years back a ladder truck had break failure and cost a leutenant his life. And that's not the first incident they have had either. The city council repeatedly screwed the BFD with budget cuts and constant contract BS negotiations. I used to see their apparatus being serviced at a commercial mechanics shop on Comm ave and would often wonder when was the last time that truck was in for even basic maintenance. Felt bad for the FF's forced to rely on equipment that might fail them. That's not the FF'S fault, that's the division captains and fire chiefs responsibility, and the cities.

This truck was a workhorse before it was placed in the spare pool, and got a good amount of front-line work even after that as the only spare tower in the city.
