APGAR scores are given on all babies at 1 and 5 minutes of life. Each baby can get any score between 0 and 10. But what do the scores mean? What are the categories included? We go over pulse (heart rate), color, breathing, grimace and tone in this video, and how the points are awarded. What do you do if a baby has a low APGAR?

This is a pretty good summery. I love it. I've mostly had issues with the obs concerning APGAR scoring but being a neonatal nurse I guess we can be quick in those things.

Ha! Yes very true! OBs can take it personally- especially if they disagree on our numbers! Given that the vast majority of the time apgars don’t really mean a lot- I’ll round up for the parents and the OB- as it were!!!

Great review for newer providers!! Weird fact that some of the posh pre-schools ask about apagar scores on the application, I have had a few families who were concerned because they would have to report a low Apgar scores.

OMG are you joking??? That’s madness!!!!! Actually wanted to do a more depth apgar thing for the rest of us- like what do they really mean! I’m totally gonna use that anecdote if ok?!!

@TalaTalksNICU I too, thought it was a joke until I saw the application. I am not sure if it was being used for research purposes to establish the studies suggesting lower graduation rates, learning and cognitive issues, etc or to determine admission. I'm hoping this isn't a common occurrence. The families asking were sending their small humans to schools far above my pay grade, 😆 Would hate for someone's potential to be limited by a low score at birth.

How about those babies who cry after extraction but then go into an apnoic episode soon after?

True- totally happens- so baby comes out looking okish- but by 1 minute is doing nothing. We still give that (bad) one minute apgar- we just need to document well what happened!!!

I went to CS to catch a baby. The baby did not cry at birth, color was 1, heart rate was 2, respiration was 2 (no flaring, retractions, grunting, lungs cleared after several minutes), tone was 2. Baby was just wide eyed and looking around. We tried to stimulate the baby to cry but he won't. The OB doctor was mad that we can't make the baby cry. Do we have to make this type of babies cry? I think it's cruel to aggressively rub the back and rub/flick the soles of the feet.

Agreed- you can get 2 out of 2 on the Apgar score for breathing adequately- a cry is not needed. Some babies are just quiet! Sounds like you did a great job. (It must be SO hard for the OBs- they are the mother's doctor first, and they want the mother to be happy. Often that's by hearing the babies cry!)
