In the world of physical therapy and sports medicine, every patient presents a unique challenge. Recently, we encountered a particularly perplexing case involving a highly competitive powerlifter who arrived at our clinic in Tulsa with severe knee pain. What made her case so intriguing was that there was no apparent cause for her discomfort-no traumatic injury, no overtraining, just sudden, unexplained pain.
Our patient, a state and world record holder in various powerlifting events, walked into our clinic on crutches, unable to bear weight on her leg. Naturally, our first assumption was that she had injured herself during a heavy lift. However, as we delved deeper into her history, it became clear that her pain had an elusive origin. She simply woke up one day with excruciating knee pain.
We began with the usual assessments-movement work, exercises, and rehabilitation techniques. We even resorted to needling her knee, a favorite technique of mine for addressing knee issues. Despite our best efforts, her pain persisted. We considered the possibility of a meniscus tear and were on the verge of recommending an MRI when we decided to explore another avenue.
It was during this exploratory phase that we discovered the root cause of her pain: her feet. Our patient had extremely flat feet, which caused excessive pronation, especially when lifting heavy weights. This abnormal foot mechanics was putting undue stress on her knees. It was a revelation that should have come earlier, but sometimes the simplest solutions are the most overlooked.
We tried various interventions-rehabilitation exercises, supportive footwear, and ankle adjustments-but nothing seemed to alleviate her pain. In a moment of frustration, I jokingly suggested she try toe spacers. When she returned to the clinic, she reported zero pain. The toe spacers had created a wider foot base, allowing her foot to utilize the entire surface of her lifting shoes. This simple adjustment corrected her foot and ankle mechanics, eliminating the stress on her knee.
This case was a powerful reminder of the importance of considering all factors in a patient's biomechanics. Foot mechanics play a crucial role in overall body function, especially for athletes who put immense pressure on their bodies. Since then, we have made it a point to always check the feet and ankles of patients presenting with knee or hip pain.
This experience not only provided relief for our patient but also enriched our understanding of the interconnectedness of the body. It was a rewarding journey that underscored the value of looking beyond the obvious and considering holistic solutions.
