Just reading this comment sent chills down my spine.

It being protocal also protects the doctors, takes away the responsibility of making the decision so they can focus on saving the most likly to live. If anyone does come after them its hard to argue against somone following protocal, especilly when the protocal is to preserve the living.

@Filip Gasic If you or a family member got shot in a mass shooting and you or the family member flatlined you'd atleast want them to try to save you. I understand why they do it, but it still sends chills down my spine knowing that this happens in the real world and that there are people that could have been saved.

@@danielshalyapin348 You're right: it does send cholls down the spine hearing that, in the event of a mass casualty event, patients are triaged according to clinical need. Trouble is, such clinical need results in cold, almost brutal decisions; with patients coming in fast, especially with serious to severe injuries, the famous 'Golden Hour' collapses to minutes, or even seconds. On any other day, you're right; a trauma team will expend as much effort in resusitating a single patient as possible. Trouble is, even with a Major Trauma Centre, they face two hard problems: even if they call in all staff that is off-duty, but still available, in the face God only knows how many seriously injured, it still not be enough. On top of which, time also becomes a stressor; if a patient comes in so severely injured that their chances of being resuscitated are low, then the seemingly brutal decision has to made. It is tragic, and it is horrific, especially for a loved one, if they get to know what happened. But, sadly, in a situation like this, it no longer becomes a question of could they have been saved, but rather if they may have been able to be saved ...

Once the adrenaline kicks in the body goes into full survival mode and just follows routine. It's what happens when the adrenaline goes down that causes most stress. Most people in emergency situation don't remember a lot of what they did to help, because their frontal cortex (the decision part that's basically you being able to think about yourself) gets overrun by the more primitive brain areas to make quick snapshot decisions, but as soon as you start reflecting again you notice the immense stress you just withheld. It is quite common for people to break down once the action is over as compared to in the middle of it.

@@pandatrip5516 this happens with physical attributes too- when things finally settled after I was in the hospital for a couple months my mom's body seemed to just crash for a while because it was no longer in preservation /fear mode

I have a friend who’s training as a doctor now and when she was assigned in the ER they had car crash victims come in. 5 people. She said it was a mess. At 3am every ounce of drowsiness flies out the window and people just came in and out past curtains. 2/5 managed to survive

I know someone who was next to the shooting in Tunisia and watched the woman in front of her get blown up. She still can’t go to beaches or wear sun cream as it brings back too many memories.

I cried. I'm sensitive as is and I'm told I'm very empathetic. I was almost crying but managing to hold it in then she came on screen panicking and I lost it.

The protocol is necessary because there are high chances they can't be saved. So the resources should be used on the ones with their hearts till responding to treatment

They may seem used to it, but in reality they are working on adrenaline. As soon as everything calms down they'll feel guilt and regret, especially when they learn who the patients were.

Erika V They have to get used to the protocol, or they don’t last long in a large triage situation. Being an ER doctor can be insanely stressful, and if they can’t force themselves to stay in a logical, calm and collected state of mind, a lot of bad things can happen; such as mistakes in treatment that can cause the death of a patient. It may seem cold, but when you have so many people dying around you, you have to chose to let one dead stay dead, or let two other people die while you spend the next five minutes trying to resuscitate one person. Especially since you don’t even know if that one dead person can still be saved; in some cases of severe injury and blood loss it just can’t be done, and ER doctors and nurses have to make a difficult call at a time where every second wasted could loose another patient. It’s not often an ER gets swamped with triage, but I can tell you that I’ve heard most ER doctors and nurses refer to it as “Hell”, and they usually mourn the patients they lost as soon as things calm down.

In a Mass causulty incident you need to prioritise on who you treat, you never have enough doctors so if someone flatlines you take manpower away from people who still have a chance to survive he said it in a rush because there was another critical patient he had to attend to.

steve phnx I work in a hospital in England and it depends on circumstances - amount of resus bays, cubicles, resources available, medical staff, and amount of patients. However the patients welfare is paramount and we try and save patients wherever possible however in some situations it isn’t always worth it due to the quality of life which they may end up with. For uk guidelines go to resus council uk of nice guidelines

In the time it takes to restart one person's heart and get them stable. 2 people who are more stable could be saved. they have to save as many as they can sadly x

Yes it’s so sad, It wasn’t their fault at all! They got hurt by someone who wanted to kill. And the hospital doesn’t have enough capacity to help everyone. I understand it if they are on the scene, like a big car crash they need to prioritize because of the driving distance and number of Ambon. But man, this is a hospital they can take beds from another department. And since this is such a big city they have multiple hospitals. But sometimes even if there is not a big bunch coming in, they can’t do everything they can because of the stupid healthcare systematic in USA and they don’t know if they have insurance or not. And you have a big chance getting shot because of the crazzzzy gun laws.

I know, it sucks. 😢 However cardiac arrest has a very poor survival rate and when you've lost that amount of blood it makes it even less likely. That guy who died at 14:38, who was left to due so they could work on the other girl was: A)A cancer patient (already reducing the chances of survival) and B) his heart had gone into asystole. (This kind of cardiac arrest legit has a 2% survival rate) So he was doomed anyway. 😢

Only mentally well adjusted people would be able possess guns not serious criminals and lunatics. Actually all it does is feed into the narrative that assault weapons are somehow weapons of mass destruction... I mean i watch the episode and the man had one gun with one magazine that's 30 rounds and i counted 15 gunshot wounds in the first two cars, at most 6 victims. assuming he hit every single shot, in the end there were more dead bodies then he actually had bullets, if he actually killed everyone with one shot.
