In this interview with leading mindbody physician Dr. Howard Schubiner, we discuss his 20+ years of experience working with conditions like PPPD, MdDS, vestibular migraine, post-COVID dizziness and other chronic dizziness. We talk about what the conventional medical messaging surrounding these symptoms gets wrong, and how the messaging can even cause chronic symptoms. We also talk about emotional awareness and expression, a treatment approach for which Dr. Schubiner and colleagues were recently awarded a several million dollar National Institute of Health research grant to study.
You can learn more about Dr. Schubiner and his long and impactful career in mindbody medicine on his website, unlearnyourpain.com, where you can also find more information about mindbody conditions, his books, research, programs and more.
To learn more about me, please visit my website at thesteadycoach.com. You can also take my course (a MAJOR update with elements of emotional awareness and expression as discussed in this video is coming Jan 2023) entirely for free by signing up at thesteadycoach.com/free-course
The checklist referenced in this video can be found on the ppdassociation.org website.
00:00 Intro
03:58 What is mindbody medicine?
06:01 Chronic dizziness disorders are curable mindbody conditions, as are many other medically unexplained chronic symptoms
07:50 Predictive processing explains medically unexplained symptoms like chronic dizziness
10:18 Dr. Schubiner confirms having seen people recover from PPPD, MdDS, vestibular migraine in his practice
11:10 We treat all "primary" symptom-based conditions in the same way, regardless of the type of symptoms
12:41 Doctors often don't know how to diagnose and treat neuroplastic conditions because there is no structural problem to fix
14:44 Post-COVID dizziness is a mindbody condition that can be treated and cured using mindbody methods; it is NOT incurable
17:40 The "nocebo effect" refers to when people get worse due to receiving the message that they cannot recover
20:14 People often have a hard time believing their symptoms are mindbody because they are told by experts that their conditions are incurable
21:50 Some ways to figure out that you have a neural circuit condition
24:00 Injuries heal and the brain is plastic; if symptoms persist, it is often a brain error
25:24 Everyone knows that emotions cause physical symptoms!
26:09 When in danger, the brain sets off an alarm- physical symptoms are an indication of this alarm
27:50 Symptoms persist when the neural circuit is reinforced by fear, worry and despair
28:50 There are two ways dealing with symptoms: dealing with the emotions, and lowering the fear keeping the circuit going
30:04 How to start when symptoms are present all the time, such as with MdDS or certain types of PPPD or vestibular migraine
31:45 It is normal for us to learn to make unpleasant sensations less unpleasant (e.g. drinking coffee or having a deep tissue massage)
33:29 Another technique for "turning toward" and not fearing sensations
36:41 How people can tell if emotional awareness and expression might help
39:40 Why being unaware of emotions in situations in our lives can cause symptoms
42:50 Dr. Schubiner describes how emotional awareness and expression helps people with chronic dizziness
46:00 Where CBT and talk therapy sometimes miss the mark and don't lead to true healing
48:00 Dr. Schubiner makes some recommendations for what you can do what right now
51:24 It may seem hard to believe, but as they recover, people will often see their symptoms as a gift
54:49 Dr. Schubiner's message to you: you can recover, don't give up hope! In my opinion, you and Dr. Schubiner are angels here on earth. His work amazes me, and listening to him so wisely explain about our symptoms, the connection to our brain/sub conscious brings me to tears. I had surgery in July to correct hyper parathyroidism… One of my most prominent symptoms was lightheadedness/dizziness. It’s only been in the past three months, that I have discovered Dr. Sarno’s work, along with Dan, at pain free you… And of course yourself, Dr. Y. Now I have the reassurance, and the knowledge to carry me forward. I no longer have to sit on my bed propped up all day with my eyes closed most of the time. That is long gone! Now I am working on these lightheaded spells… I can’t thank you enough for taking the time to post these amazing videos. You are a godsend!

A few years ago, I would get these occasional dizzy spells. Over time they got worse. Finally, one Sunday evening I had one that was so bad I had my wife take me to the hospital. Of course, they could find nothing wrong 😊and released me; suggesting I follow up with my personal physician, which I did. He had me get a couple tests, ct scan, fmri. When I went for my follow up, he said nothing was wrong with me. To his credit he didn’t suggest more tests. He just said I was okay. I haven’t had a dizziness episode since then. It is sooo clear to me now that it was TMS, or physical anxiety or mind/body or whatever you would like to call it.

Thank you Dr. Yonit for doing this interview with the amazing Dr. Shubiner. I haven’t seen much material up until now that dizziness is nueroplastic so this is very helpful for my brain’s belief in it. I appreciate you also asking him how we can take steps right now. I feel like I’ve done a lot of emotional work over the years but this reminds me of the various emotions to express that I may not have done much with and to try speaking them out loud then releasing them and living differently after that. I tend to journal mostly but there are multiple ways for self expression. Thank you for your advocacy for patience and true healing!

Hi Sr. YO, I recently stumbled upon your channel and have listened to a few of your videos. It has been so helpful for me emotionally to hear this information and the ice cream on the cake is listening to your intervew with Dr. Schubner. As you both stated, I have practitioners, both Neurologist and ENT, who have suggested CBT, SSRI, and VRT. I have had VRT, which helped but only for a tine and has no interest i. an SSRI that will only bandaid the problem at best or cause worse side effects, no thank you! I have Vestibular Migraine so my symptoms are largely dizziness and other things versus headaches(sometimes). I also have RA. The last 2 1/2 years have been spent attending a cycle of doctor's appointments and being unsble to have much of a social life or ho to the gym becsuse of the oftentimes debilitating symptoms. Thank you and Dr. Schubner so much. This gives me hope. However, where do I begin to find a practice like this. I live in Philadelphia, PA.

Thanks so much for again a nice video. For me it for sure started because of stress. The stress caused burnout symptoms and later bppv. After BPPV i developed the PPPD. The stress that caused burnout stuff is long gone... My life is quiet and i really do almost everything in life. Horse riding/ driving and more. BUT.. i still feel pppd symptoms all day long. Some days its more my eyes, some days its more just out of balance etc. I do not have anything that worries me other than the symptoms from pppd that i feel all the time. I really try to do what you say and also this doctor..keep going with my life, try to not be scared and have trust...but...because it takes so much time...i keep falling back in feeling scared that i just wont be able to resolve it...🙈The most i worry is when i speak or am with more people around me...i feel so out of balance than. And than shaky....is that weird? It worries me that i feel like that...🥰

Yes, recognizing that stress is a very important part! It sounds like the fear is still strong and there are times when you avoid things because they're scary (this is totally understandable), That is probably a part of why the symptoms are still going. It does take time and I wish it did not. I know it's awful and you're suffering. Sometimes I find that even when the initial stress is gone, there are still some things that are "sticky" in life that are reminding people of difficult times from the past. This can also make the fear worse- anxiety is often a response to emotions that are uncomfortable.

Thank you for this very informative interview Dr. Yo. I’m aware that everyone has been warned to not discuss Covid vaccine injury but my question is if this is the cause of symptoms, can these people be healed as well.
@TheSteadyCoach
@TheSteadyCoach Год назад
If there is actual tissue damage from any vaccine, then it is not really a mindbody issue. However, in most cases in which people report issues after vaccines, there is no evidence of tissue damage, and in those cases, they can be treated as mindbody issues. Anecdotally- I've worked with many whose symptoms came on after illness or vaccine. None had actual tissue damage, and all got better with mindbody methods.

I have used the ‘Journal Speak’ program she emphasizes the need to use feeling words when expressing feelings- meaning “I feel unheard”, “I feel alone”, rather than “ I feel I like I’m alone”. Otherwise you are doing more of a narrative than expressing your feelings. Googling ‘feeling word list’ can help. It’s hard but much more helpful.

Hi there, I am so sorry you’ve been suffering for so long! occipital neuralgia is another one of those symptom-based or primary diagnoses that Dr. Schubiner refers to in this video. It doesn’t necessarily mean there is tissue damage that explains the symptoms.

Can you or the doctor recommend a good vestibular rehab therapist in the San Diego area, please. I have had Menierre's and VM vertigo for 33 years off and on. Now long term Covid MECFS so I need more support. Also covered in first time psoriasis. A lot on my plate but the dizziness is the most difficult at the moment. Excellent video. Actually all of them are. So so helpful. But I can honestly say that for Menierre's sufferers, sometimes the brain doesn't seem to compensate. The deafness, tinnitus, pressure, and vertigo are deeply disabling. Pushing through for decades has taken its toll. I would love it if someone could tell me, "You will never be dizzy again." Would be a release from this prison.

Also, I go to PRN physical therapy, which is covered by my insurance, and they have some excellent therapist that know about VOR, etc. There’s also a place in Missionvalley called Occulus which is for dizziness. And there’s also the dizzy doctor. Just look him up under the busy doctor. I haven’t done the last 2 things listed, but this is all in San Diego.

I am listening to this as a patient who was a medical professional. I think we are being told we are not to castrofize the symptoms but acknowledge by Journaling, lean into emotion to get to bottom. Of it and brain will follow. I had( have) chronic pain , 35 yrs then 25 yrs of vestibular sx. Recently migraines reemerging while adding a med to help pain. This while removing what I was compensating the vertigo . Awaiting a vertigi check for 7mos while migraines coming morefrequently. One happened and in June and i say it broke my brain. Ent says its A thing .I'm like your are kidding right! My brain is irritated. I know one of the newer sensors are. My husband's son who emerged into my life after being homeless , raised his daughter w my husband and she was a difficult child. Not at first. Life happens and we are caring for loved ones. Now I see I am pushing myself to to cope and have the best life I can have but my brain is foggy and It's not working as I would like. I have stopped doing more as since covid hit I had an amping of cardiac again. Have not had issues in 7 years. ? Guess my brain is calling the shots .

This also sounds like anxiety disorder where you have all kinds of symptoms such as dizziness or vertigo and the fear of those sensations causes a cascade of others and induce more fear and the believe something is structurally wrong.
@TheSteadyCoach
@TheSteadyCoach Год назад
Very true, there is a strong overlap between them. One reason I don't call it an anxiety disorder is that many people who have physical symptoms don't have classic symptoms of anxiety and/or don't classify themselves as anxious.

If you published a lot of those episodes with a successful finish, you would sell more books and increase the knowledge of those who try to implement what you have essentially taught them to do. Thanks for your help and considering my suggestion to help others learn from your knowledge.

He did say that if there is some sort of damage in the inner ear would lead to dizziness. So if you do have that it’s not mind/body??
@TheSteadyCoach
@TheSteadyCoach Год назад
As we discussed a few minutes later, damage to the inner ear heals or the brain adapts to it. When this does not happen, inner ear damage does not explain the symptoms.

These medicines relax you (can take away the fear of the dizziness) but I’ve also heard they are not good for neuroplasticity. I sometimes take a very small dose when I’m super anxious but also started healing from PPPD with all the tips on this channel so I wouldn’t want to jeopardize or slow down the process of retraining the brain.

How does one check for structural damage related to dizziness?

Workup with ENT and/or neurologist and vestibular testing. Sometimes includes imaging and various lab tests, hearing tests and specific tests to check the function of the eyes and inner ear.

I have been dealing with dizziness for the past 3 years. I was taking medication for anxiety and was dizzy while taking it. I quit 1year ago and got tinnitus in my right ear 4 days after I stopped taking the medication. I have had MRI'S, CT scans with and without contrast from neurologist, seen ENT and had VNG test and they say it's not my ears and they says everything is fine. I have pain in my neck and shoulders and I'm tense all the time from worrying about this. I have good day's and not so good days. The lights inside stores make the dizziness worse sometimes but I can be outside and it's fine. Is it me causing the dizziness for thinking about it all the time? I try not to but it's there. There have been times when it's not there for a few days but then it's back. Is there still hope to get rid of the dizziness? Is this all just mental?

Everything i think! Neruo, ent, er, opthalmologist, pcp… etc etc all tests over past 5-7 yrs i think. Is this covering everything to say somatic dizziness? Anything im possibly missing? Doubtful lol but i need to let the checking go and check fully so my brain lets go of that part!

How do you know if your dizziness is primary or not?
@TheSteadyCoach

When there is no medical reason for the symptoms to be there- aka tissue damage is not there OR there was tissue damage but the brain has not adapted.

Also symptom based diagnoses like post Covid syndrome or PPPD are primary diagnosis, aka the symptoms themselves are the diagnosis, this means they are mindbody issues.

Hi Kimberly, it’s hard to answer that- all conditions are biopsychosocial but some have more of a biological contribution than others. This would be a good question to ask your doctor: “do you think my dizziness symptoms are fully explained by my tumor?” And “would vestibular rehabilitation improve my symptoms and/or functioning?”
