Introductory video: • Why you have PPPD, MdD...
The only predictable thing about recovery from PPPD, MdDS, vestibular migraine and other medically unexplained causes of chronic dizziness symptoms is that it's unpredictable. One very common source of this unpredictability is that symptoms have a tendency to morph, shift, change and swap throughout the recovery process. In this video, we explore why, and I provide a general framework for how you should approach symptom drift to leverage it to your advantage.
00:00 Intro and what symptom drift means
02:23 What symptom drift and stack look like for different people
04:50 Your brain is misinterpreting triggers (physical and emotional) as dangerous
08:08 Your brain's prediction errors can create sensations
10:50 Danger mode causes temporary body dysfunction
12:27 The key to dealing with symptom changes
14:47 Don't focus on the symptoms themselves; focus on your nervous system

This was so encouraging . I have MDDS/PPPD/ and been told vm all from different docs . My symptoms of rocking have gone down significantly but it’s turned into this visual battle with visual snow, light sensitivity(glares with lights) and a feeling of just floaty and intense anxiety . Of course all MRIS and tests are negative . I appreciate you stating the positive tips on how to recover for life thank you!

So amazing and edifying. What a gift at a time I need it. The hands thing was something new to me. As I have been improving, and drifting over the past two weekends, I was astounded with how I felt after installing the xmas lights (on the roof:)), and washing my van yesterday. For some reason, both activities somehow made my symptoms much improve even despite maintaining activities. These hand eye coordination things made me feel much better. This video articulated so much of that for me. Thank you so much.

This is great information. After dealing with dizziness for several years I started seeing and understanding what was happening but I didn't have a name or a clear pathway to healing. Finding you in April of this year and listening to your videos along with the TMS drs I'm following has made a huge difference for me. I can look back and see symptoms that I had through all of this that are now gone so symptom drift is definitely happening in me but I now know how to deal with it. Somehow you always manage to answer my most burning questions in your new videos. 😊 Thank you for this one, I feel like the dizziness got better, then I had burning skin sensations (it was a huge issue for me a few years ago, I don’t fear that anymore so it went away in 2 days) and now crazy visual symptoms (I’ve experienced these when the dizziness started two years ago, but the volume turned up on them). I feel like it’s a “game”, like I’m being tested, it’s like my brain want to find the sensation that’s gonna really freak me out, if its guess is wrong, it changes it in a few days. 😅 A few years back I could never imagined that these stuff work like this, it would be really amusing without the suffering, but getting there. P.S. A few years back I ended 8 years of severe and 24/7 vaginal pain by laughing at it when I really had enough, I just physically, mentally couldn’t care anymore, so that’s a great advice.

Wow, you've really been through this! I really do find that framing helpful- "My brain is just trying to get my attention by throwing new sensations at me." I totally get the sentiment about the suffering. I wish that for you too, but I know you've got this, and I know you'll get there.

This made me cry. I am going through one of the hardest times I’ve ever faced since being diagnosed with vestibular migraine and chronic dizziness back in August. I have also been recently diagnosed with a PFO and extremely low iron saturation. While I do know most certainly that I am dealing with neural circuit dizziness, the symptoms I’m feeling from the low iron and the fear/uncertainty with the PFO are exaggerating things to an uncomfortable extent. I am currently wearing a heart monitor and that in itself is a constant reminder that I may have something really wrong. I am struggling to feel safe when there is so much uncertainty and so many issues to address. It was one thing when it was just the dizziness, but now I am completely overwhelmed. I can’t figure out what is causing what and the whole crew of doctors I have can’t seem to figure it out either. All this aside, I want to thank you so much for all the good you do and all the hope you bring to everyone who needs it. You are amazing Dr. Yo! ❤

You are safe, you are strong. Time is the best healer, there is light at the end of the tunnel and you will get there and figure it all out. Maybe try some meditation, or just relax with some music. Give your body and mind a break and some space from the worry and the stress, it will allow you to feel less overwhelmed and you will start noticing what could be causing each thing.....one step at a time. Good luck 🙂

My heart goes out to you. There are others here who are dealing with similar medical uncertainties and of COURSE your nervous system is responding with alarm and is being extra sensitive. Your brain can pick up on any change in physiology as a sign of danger, even when it's just... wrong. This can activate symptoms. It's similar to when people find they get headaches from changes in weather- the weather change isn't dangerous, but the brain is interpreting it as a threat. That would be a potential explanation for what is happening.

Thank you so much for this video! I found your coverage of symptom shift very encouraging, because it reflects what I learned about psychosomatic phenomena from the late Dr. John Sarno. (I was a patient of his in the 1990s). He termed this shift the "symptom imperative." The subconscious is extremely crafty at coming up with scores of these substitutions out of left field, in body parts and systems you have never consciously thought about. These symptoms need to be fresh and original in order to scare you so that you focus on them, and not the big, scary life issues that your subconscious wants to help you avoid. I highly recommend Sarno's book, "The Divided Mind." It touches on all of this. It changed my life.

This video was a breakthrough for a few reasons. I suffered with bppv and long residual dizziness last year. I recovered by overcoming the fear and had a pretty good time until Jan this year when it hit again. It took many maneuvers, too many if you ask me and still have some residuak dizziness but this time my brain is in the "well if it happened twice ot is bound to happen again and again and again" and this is the loop i find impossible to shift. If i can catch and ride the wave of "well if it happens again it is what it is, but for now let's get over this one" i feel i could feel more safe in my body. I just find it very difficult

Thank you very much for your description of symptom drift. I have spent a year recovering from a concussion resulting from a fall on ice, and are puzzled by what you so call symptom drift. you comment on brain/emotion/fear state. My initial treatment protocols emphasized performing the VOR as quickly as possible. While that may have been initially appropriate, that may not be true a year later in for the reasons you described. Repeatedly and vigorously performing VOR disruptive exercises probably causes fear and confusion that my brain cannot handle, and may thus cause symptom drift. I will experiment with performing the VOR exercises at a more reasonable pace that enhances the VOR reflexes but does not frighten or unreasonably confuse my brain.
@TheSteadyCoach
@TheSteadyCoach 6 месяцев назад
BRILLIANT. Yes. I have seen others with post concussion syndrome endure the same thing, and yes, exercises performed with a lot of intensity and urgency can increase fear signals to the brain and inadvertently perpetuate symptoms. Thank you so much for sharing your experience.

I have experienced dizziness for a year. I found you the end of June. The dizziness started a couple of months following a sinus surgery to cure migraines. Total failure. Migraines increased. So my fear evaporated in a couple of weeks of listening to you. But I went to an optometrist that told me my eyes are not working together and that’s why I’m dizzy. She gave me a new prescription with a prism. But the dizziness did not stop. I really feel like it’s TMS from the surgery and increase in migraines. But after months of TMS work, I’m still dizzy-although I’ve completely lost the fear and so grateful. So is it a structural problem or TMS?I’m at a loss.

Dizziness is a tricky little beast in my experience. It is less cut and dry than pain for many people. It’s wonderful you’ve lost the fear. That indicates to me that what is still driving the symptoms is ongoing unresolved stress. For many people, this comes down to internal stressors like the stress they (consciously or unconsciously) put on themselves. Outward signs include perfectionism, people pleasing, etc.

Hi, in the end you talked a bit about how it's good to focus on activities where you dont think about the issues. I noticed however that my copingsmechanism since i was 12 or so (16years ago now) has been to push away these feelings, weird sensations and thoughts. This because they seemed to complex, too much and i didnt know how to deal with them. How do i focus on other activities without pushing my issues away? On the one hand i should do exercises and learn to be self aware, but on the other hand i should also do fun stuff and try to let my brain focus on other things than these issues. But when i try to focus on activities i just fall back into old habits of quicker, no contact with body, pushing all the issues away. Maybe this question got too long😅 but it boils down to: what does it look like to let myself enjoy activities without it feeding my tendency to push away thoughts and feelings and get overstimulated?

Dr Yonit, can i ask u this? I do almost everything again. But, with feeling off balance or feeling different sensations. Some things got better over time finally. But...the thing that i find most difficult, is having people around me. Talking to people around me, interaction makes me feel so off balance all the time. I really miss feeling normal when being with people around me. I notice my brain just cant keep up with all that happens around me than. Is this common? And. .will this also get better again over time? I so much miss doing stuff with friends...🥺

The example you used of seeing things out of the corner of your eye when nothings there has been a new symptom for me recently. Exactly like you described, its when I'm distracted. It's kinda scared me a bit but I feel like my dizziness has shifted to being more vision related. I even notice recently that mirrors are a dizzy trigger. Is seeing shadows or movement in your peripheral a common symptom of danger mode dizziness?

Hi Dr. Yo, I’m currently working my way through your free course alongside your workbook that I purchased. I’ve noticed my dizziness symptoms are shifting, and i’m getting more of a weak noodle feeling in my legs. I have been making myself going on daily walks like I used to, but it’s almost like my legs are getting super fatigued. It levels out at various times throughout the walk but overall it feels very strange. I also sometimes get super cold feet. These symptoms usually appear when dizziness does not, they sort of switch off. But usually more dizziness. Until recently when I started your course and now it’s shifting. Can the weak feeling in my legs be from NCD? I’ve had every test in the book I should mention (muscular neurologist, MRI, etc etc). I feel like I can’t find anything about this on your channel so it’s continuing my fear that something is wrong.

Today when I tripped I got that sensation where the body heats up quickly to brace for a fall. Although usually when this happens I get that warm sensation in my head. Today I got some dizziness instead. For some background information I’ve been struggling with general dizziness and vertigo episodes since October of 2023. Is this an example of symptom drift?

Hello Dr. Yo, first of all thanks for your helping videos. I have recently been diagnosed with PPPD and have some questions that you may be able to answer. Is it normal for my symptoms to improve when I drink a glass of wine? Alcohol is not the solution, but it gives me some relaxation every now and then. I also wonder if you can go skiing with PPPD? I am going on a skiing holiday and my symptoms increase when I exercise/go for a walk. I've heard that avoiding activity isn't helpful so I'm wondering if skiing is actually a good thing for my brain to recover faster? Thanks for all your help!

Hey Dr. Yonit, I believe that my dizziness may be caused from a histamine problem. Also, I got bit by a tick in July and my symptoms happened August. When I drink beer now all the sudden I get sick and dizzy. Do you have any idea about whether a tick could cause histamine intolerance? Been to so many doctors. I guess I need to go to one and ask for a specific blood test. But I have these PPPD symptoms. Please help if you can. Thank you!
@TheSteadyCoach
@TheSteadyCoach 6 месяцев назад
Hi Shane, re: the histamine intolerance, that's out of my area of expertise so that's a question best posted to a physician. However, I have seen many food intolerances be the result of mind body issues. You might consider consulting with one who specializes in mindbody stuff because those folks are going to take a holistic approach like mine. There is a big directory at pddassociation.org

Hi dr. yonit . I am part of your members on the steady coach app . I had a question on unilateral hypofunction . If I had a VNG which showed some weakness , and then I did (vrt + brain exercises) for a month, would a 2nd VNG detect any improvements , or is a VNG test just to detect inner ear issues without considering the brain adaptation? Thank you .
@TheSteadyCoach
@TheSteadyCoach 6 месяцев назад
Hi, this is a great question. Sometimes ears spontaneously get better over time, so there may be a change on a repeat VNG. However, that's not the goal of VRT/brain exercises- they can help your brain adapt regardless of whether the ear function changes. In many cases, after VRT, the VNG results would be the same. In other words, a VNG just detects inner ear issues without considering the brain adaptation.

It's wonderful to hear about your progress! And yes, "danger mode" can be active/cause prediction errors even when you're not actually in danger. My guess about this kind of thing is that you are going through a biological transition (hormones or otherwise) at this time and your brain is mistakenly identifying it as a source of danger.
@tlynnven
@tlynnven 6 месяцев назад
Wondering if this danger mode in my brain and nervous system can affect my heart specifically heart skips because this is the symptom that started a few months ago and I’m fixing to go see a cardiologist about it just to check on things but I just feel like it’s all related to the nervous system. I noticed it more when I sit down I’m in front the computer or lying down for the night so things like that that are kind of situational. I do notice it sometimes whenever I’m standing or doing something from time to time but most of the time when I’m sitting

I went through so much! I bawled after we ended our call. After a year, I finally transitioned off my walker! Then a cane, now only a cane when I need it.
