Chronic lymphocytic leukemia (CLL) is a type of cancer that affects the white blood cells, specifically the lymphocytes. It is considered chronic because it progresses slowly over several years, often without causing noticeable symptoms in its early stages. In CLL, an abnormal population of lymphocytes, known as clonal cells, accumulates in the blood and bone marrow, eventually crowding out normal blood cells and impairing their production. This video addresses the mechanism, diagnostics, symptoms and treatment guidelines of chronic lymphocytic leukemia.

Ibrutinib, venetoclax, and obinutuzumab have revolutionized the treatment landscape for chronic lymphocytic leukemia (CLL) by significantly improving outcomes for patients. These agents are now considered first-line therapies due to their high efficacy and favorable safety profiles. However, the high cost of these agents presents a significant challenge for their widespread use. Ibrutinib, for example, can cost upwards of $140,000 per year, while venetoclax and obinutuzumab are similarly priced. In many parts of the world, the high cost of these agents makes it difficult for patients to access them, particularly in low- and middle-income countries. while Ibrutinib, venetoclax, and obinutuzumab represent significant advances in the treatment of CLL, their high cost and variable availability present significant challenges for their widespread use. It is crucial that efforts are made to ensure that these agents are accessible to all patients who could benefit from them, regardless of their geographic location or financial resources.

I am a cll patient. I am eating capsule ibrutinib 140 mg twice daily as per direction of my physician. I am eating it since 2022. I am fine but feeling much weak. Please help with suggestion.

It is important to communicate with your physician about any experiences of arthralgia and myalgia (Joint and muscle pain) while taking ibrutinib. These symptoms are common side effects of the medication, and your physician is the best person to assess drug toxicity and potentially adjust dosages or medications. It's crucial to be open and honest with your healthcare provider about your symptoms to ensure the best possible management. If there are signs of drug toxicity your physician may reduce or hold a dose in order to manage these symptoms. It is also important to review your other medications with your physician as drugs such as a statin may also worsen the these specific side effects. I recommend that you kindly reach out to your physician for further guidance and support in managing your health concerns effectively. Your physician can provide you with personalized care and tailored treatment options that may be beneficial for your specific needs. Wish you all the best.

I just started Obintuzamab last week continuing and will start Venetoclax in 2 weeks. Will side effects increase when starting Venetoclax?

First, I want to say you are already a hero for taking charge of your health and fighting against this disease. Starting treatment is a courageous step! I would like to encourage that patients often do not experience side effects in the same way when combining these agents. That is to say patient A may experience mild side effects compared to patient B that experiences severe side effects on the same dose or as you move from one cycle to another. That said, Venetoclax will be started at a low dose for example 20 mg/day and increased as the cycle progresses. Your doctor is expected to guide and monitor closely your experiences if there is the need for a dose reduction, the need for regimen switch or delay. Regarding Venetoclax you should stay very well hydrated (Drink lots of water) even 2 days prior to the first dose to avoid tumor lysis syndrome of course if there is no contraindication to that. Side effects may vary but about slightly more than half of the patients may have a reduced neutrophil. Your doctor is expected to monitor your CBC. I wish you all the best! Thank you.
