Anterior Pelvic Tilt is a very complex condition and can be caused by many different factors (forward head posture, pronation of the feet, fallen arches, contracted/shortened hip flexors). Finding the underlying cause is crucial in the correction of this postural deficiency.

You can see by his facial reaction at the start of the video how motivated he is and how upset he is by the state of health of people sitting all day and doing jobs and activitiea that ruin posture

You just made my day Doc. This stuff is just SO important, and I feel most of us have postural issues which are not allowing us to lead great lives. I have had 2 knee surgeries on each knee, and now am having issues with the right one again. I am gonna go on a limb and bet that ALL of what you talked about has created major imbalances in my body, and as a result, I keep getting hurt. Thank you so much. You just won a FAN for life. This is an excellent video. I have this condition. I have done some of these exercises before but the standing postural changes are great and make me feel better already. It's hard to retrain your body to correct itself but this video makes it easy to understand how to do this. It's not easy but worth it in the long run if you want the pain to go away.

Agree with you, it's a head tilting forward as I saw myself in a shop window and I was horrified. No wonder I have back pain , neck pain and a headache but since noticing I did some pelvis exercising abd it made such a difference. I know when I walk crooked that my head start to ache so I can correct my body stance again. Thank you for this, you are the best

Omkar Bhatkar That depends on how dedicated you are to correcting your posture. Work on it daily, from head to toe. Nobody can say exactly how long it will take, but you will feel a lot better if you dedicate time daily to work on it, and always be aware of your sitting and standing posture, from head to toe, just like the doctor says in his video. If your posture has been off for a long time, it could take several months to correct. The best thing is to start taking care to do it right away, and daily, and to always be aware of it

Do heavy deadlifts with super good form hinging at the hip, beleive me you will thank me later. Gives a solid punch to glutes and hams, all else remaining is just the stretch for hip flexors.

Frank, As in every profession, you get the ones who are highly and profoundly compassionate to learn ever more to enhance their skills forever and apply the best knowledge they ever can, simply to help mankind to improve the quality of their life Then, you get the others who get "some" training and then that's enough for them, and then they cruise along, for quite some time, producing unsatisfactory, unprofessional, and even unethical work. Then, sooner or later, they crash. These are the guys to avoid at any rate and you can feel it how their mind works. They have complaints, they are unhappy with their lot, they are very negative. Obviously, it's not their fault because in most cases, these guys have suffered various traumatic experiences, often in their childhood. Without knowing that they carry this toxic load as memories in their body's tissues and actually block proper functioning of all their senses, any sense of healing themselves is without reach. So, they are just trudging on with that heavy burden of abuse and hurt as if that's the life they are destined to follow forever.

Very comprehensive. Addressed all my issues, anterior pelvic tilt and it's causes, forward head, weak abs, hyper lordosis. I've been doing exercises for my posture to stretch my tight chest muscles, and I'll be adding the exercises you demonstrated here. Thanks!

the closest info I've gotten to this very accurate break-down was from a PT, not a doctor. and I don't know if that is b'cause the "doctors" (plural) that I have seen didn't know or just wasn't honest to my face about the condition of my physical stance....you are "the best" in every aspect, Dr. Mandell. You can take people's advice as much as you want, but only you know your body the best. The first thing someone needs to do is realize what is causing them to have postural issues in their day to day lives. What this guy says won't help you if you have an issue from training, but if you have issues from being sedentary then yeah get active and strengthen your body. When you train these muscles, train them with proper form from the start or you will suffer imbalances. Another issue is people feeling inadequate about themselves, which causes tension to build up in their bodies. If you can't relax your body completely, then I would try to work on mediation. There's thousands of possibilities for that dull pain you feel from being inactive for so long. Take everything slow and listen to your body.

Most Insurances cover Physical Therapy and it can be a great help. "Forever Fit" is an example of PT's who teach as well as train. I see the Physical Therapists at Forever Fit in Wheaton, Maryland. They are very helpful. But I need to study and understand the total problem and think through every step. Dr. Mandell explains the entire problem. He puts all the loose ends together. Sunny, I love your way of thinking. Today, stress is very serious and in fact, it is a real killer; it creeps into your life without you realizing it. Listening to one's body is the first cardinal rule. When any pain crops up, find out what could be the source of this pain. Go the natural way;; do not rush to get your medication!! Medication, in most cases, does not work - you simply enrich the pharma greedy individuals. The second one is to become aware that both our body and mind are connected and need regular movement or exercise, but with moderation. Any excess is harmful. The third rule is to accept that we all endure stress: some is good, and some is very damaging. Therefore, it is important to learn how to control it before it controls us.

Great video Doctor. Can an anterior pelvic tilt cause my feet to point outward when I'm standing? Duck feet? I have a pretty aggressive anterior pelvic tilt, and also extremely tight quads. Not sure which is to blame, but the cause if coming from above my knees...as my kneecaps points outwards with my feet, so it is not a lower leg problem. I constantly get shin splints when trying to run, and lower back pain when I'm hiking from the curved back.

Hey Doc, I know you were mentioning all the possible causes, but is there any way you can write them down along with the ways to fix them (in a response to me?) So far I got 1. Weak hip flexors (stretch with lunge stretch) 2. Forward head (chin tuck) 3. Pronated feet? (Idk what to do for this) Anything else? I really wanna fix this issue. It’s keeping me from seeing my abs even though I try so hard in the gym. Thanks a ton!

Thank you for such and informative video. My ENTIRE family has what I now know is an anterior pelvic tilt. It was even coined the tway-sway (our last name is tway). We have all suffered from many of the issues you mentioned and you are the first that not only named but explained the causation and multiple ways to not just alleviate symptoms but combat the root problem. motivationaldoc @motivationaldoc Hello, doc. I have a lot of problems with my spine. First anterior pelvic tilt and then hatchback. I cant move my neck backside and also cant rotate. Feels my shoulders are frozen. Now in the starting i felt, i have pain in chest either side and was very high at a point of time instant and now i am feeling headache also. Because i try to move my neck in all directions. My neck also crackles that sound when i turn the neck. Can you recommend me something. Doctors here heavily rely on drugs and pain killer.

Thank You!... wish I know now, previously. Years back received spinal compressed fractures resulting in mild scoliosis... years later noticed lower back pain/weakness due pelvis tilt... given few helpful suggestions due complexities... resulting in severe pelvis misalignment, in pelvic arthritis and worn hips... NO PAIN... refused surgery... relearned to walk... learning to do stairs, and working on abdominal muscles... your video gives me additional hope... I need pelvis straightening... creating a stronger balance and stronger gait. Thank you, again!

Ive been lifting weights for years and have been neglecting the root of the problem with my APT. This is very helpful. my right leg goes inward, hip flexors tight etc all the above. I have been thinking it was other problems and im relieved its all connected, and that i know what to do now. I want to add that engaging pelvic floor is helpful too, to correct this issue.

IF YOU ARE FIGHTING THIS AND NOT WINNING PLEASE READ.... I had both of these and continued to train for MMA twice a day. My body got so twisted up it almost killed me. Multiple herniated discs due to over active muscles. In my experience, my weak muscles were so stretched out that I could not get them to activate, and the tight muscles were on a constant activation, in massive knots. I wasn’t sure I was doing the right exercises because I got zero results, just constant pain. You may have to start with very subtle activation techniques to get your brain reconnected with the unused muscles. Be patient, and consistent and you will slowly start to win.

Question - it's confirmed by a podiatrist I severely overpronate, and by my chiropractor that I have hyperlordosis. I also have osteoarthritis in my mid back. How do I know what is causing the problem - the overpronation or the APT? Thank you for your incredibly informative videos, your exercises that you've recommended have helped me tremendously!

hi, I've watched slot of vids on this subject, but this one was very informative, giving me some new tips. I'm generally pretty well trained, have a pretty stable looking physic, but probably have. anterior pelvic tilt, I've been working on excerises such as yours for a while but have also internally rotated knees and fallen/weak arches and flat feet. can you recommend any excerises to correct these other conditions?

@@siobhancapell start with just learning to suck in your stomach and breath through your nose and not your mouth while curling light weight dumbbells. Then do squats and push ups with the same breathing. Its all about training your core to engage properly. I hope this can help you.

Pushing abs out while lifting weights is absolutely the correct way of core engagement for weight lifting. Technically, the stomach moves in compared to completely relaxed state, but the abs muscles push out compared to sucked in state. Do NOT suck in your stomach (using exhalation muscles, i.e. obliques) during this type of movement! You need increased stability in lumbar region while lifting weights, and this is afforded by increasing abdominal pressure. This is also known as the valsalva maneuver and is routinely used by powerlifters. Also check out Simon Borg-Oliver (a guy with 50 years of yoga expertise) who addresses this topic directly.

Thank you sooooooo much. I just turned 31 and have allowed life situations to get in the way of working out even though I eat wholesome and very clean (mostly). I experience all of the problems you have touched on. A new issue I feel in my neck is sometimes I feel like my head is too heavy when I aim to stand and sit upright (the correct way). Any feedback?

Comment For ME For These Stretches: 12:28 Stretch So Keep Chin Level and just nod head forward and back like you would with keeping two fingers on chin; 2 Sets Of 30 Seconds Only Do These Next Two When Hip Flexor Is Tight: 13:54 Simple hip flexor stretch while one knee is behind the other; 1 Set Of 30 Seconds For Each Side Only Do These Next One When Hip Flexor Is Tight: 14:35 Again, another hip flexor where one knee is behind the other but this time on a pillow; 1 Set Of 30 Seconds For Each Side 15:26 Lastly a butt bridge but one leg is lifted straight into the air; 2 Sets Of 30 Seconds Alternately switching both legs

Wow, thanks a lot for the exercises. I am wrecked with a pain in my back for 2 years. I didn't know what to do. I spent thousands of pounds consulting this pro and that pro, but no one told me how to do the exercises.

Even when your abs and butt muscles are very strong (let's say amateur gymnast/intermediate yoga level), you can still experience the suffering from anterior pelvic tilt, forward head etc. Here is how: load said muscles to exhaustion. Then see how your posture will work the next day when the muscles feel sore and knocked out. In other words, what the doctor says here is very true, just take note that especially if you exercise hard the condition of muscles and your posture are going to vary greatly day to day. Even catching a cold will have detrimental effects on muscle condition! And in such cases a short REST will bring you back in form rather than extra strengthening exercise - you definitely can have too much of a good thing.

At 13:36-38, the word "outside" should be "inside:" "... make sure you're not wearing the *inside* of your shoes out near the heels faster than you should be. That would be an indication of pronation, or inward tilting of the foot." The image shown is incorrect: it depicts supination (outward tilting of the foot) with excessive wearing on the outside of the heel of the shoe. Arch supports help with pronated flat feet (fallen arches with inward tilting of the feet), not with supination (outward tilting of the feet). .Tu10-22-19-1917E

Dr. Mandell, I've been doing research on and working on imbalances in my body for about 18 months. I've gotten bits and pieces from different areas of practice. Physical therapists, chiropractors, medical doctors, and trainers. As well as the science of the brain that ties it all together. I've been working on these exercises for a few weeks. I accidentally tense my neck muscles when I contract my glutes and abs. I'm guessing this has to do with fibromyalgia and also not having a good mind body connection yet, but hopefully over time my muscles will learn how to work properly.

Probably the single most important physical exercise regime to straighten you up and straighten you out. Every few sentences every exercise a gem and turning in from pretzel the flat. I've seen progress after just a week or two sessions and I'm 75. Wow! Goodnight part is many of these exercises can be done with very little equipment

Sir i have all the problems discussed here.. Pelvic tilt Hunchback Knock knee Knock elbow Inward feet.. But probably u shown pevic tilt exercise only... Anyway loved the way u explained.. want to know how many months will it requires to cure normally if a person do ur exercises regularly

thank you so much, Dr. Mandell! I'm really happy to have found you. I'm following you for about two weeks and I really find help in your exercises! Pity it couldn't happen earlier. I already have serious neck and head pains. I doubt this anterior tilt is one of my issues. Beginning with a difference in leg's length subsequent double scoliosis and all damages caused to discs all over the spine...now I often get up or awake with a pain on top of the head! Doing every day your exercises may help me? Thank you again.

John Merven - The doctor would need to physically examine you so you would have to be seen by him, at his location, before he would be able to make any recommendations. He would not be able to do that over the internet. If you read the words at the beginning of the videos, they state that he is not diagnosing you. The videos are meant to be for your information, and to help you. You could check out medical and chiropractic care, and physical therapists where you are located. Your primary medical physician should be able to help direct you.

This is an awesome video, so much info and many exercises. After almost a year in pain and no definitive diagnosis, I am to start therapy again addressing glute weakness. If therapy does not take care of it, I will immediately start with this routine. I do have a question though, Doc Mandell: would breast stroke swimming be okay while working with these exercises? What would you recommend for cardio ?

Awesome, I have been in the Chiropractor since 6 months ago regards to this issue. I work sitting for 8 hours and that's part of the cause. Thank you for sharing this, I know now what I was doing in the GYM was wrong. I was using the pelvis during doing the abs and putting my legs too high. Sometimes I notice the tightness in the lower back avoiding the energy to flow. Thanks again, Sandra

One video was stating this is simply caused by sitting too much hmmm yes maybe for some , but my situation did not occur until abdominal sugeries and a violent attack that broke several major bones and that treatment caused worse problems and they were never affected Lord help me this PAIN

Nope. But again I've never been very rigorous about the exercises. If I can't see any difference after two weeks I lose the motivation. I guess I'll just live with it. Perhaps my case is too serious to fix myself

It works!! One other thing to add is when you sleep, try to sleep facing up with a pillow or bolster either under the back of your upper thighs or just above your knees (depending on how bad you APT is) and it'll relief you while you sleep, I'm a side sleeper but this has done wonders along with the exercises. If you're lazy you can do this to relief your muscles and do the squeeze buttocks and abs while standing as well as before you sleep in the position I just mentioned :-)

This video was posted in 2016, but after watching the video for the first time, 3/14/2021 and reading several hundred comments, not one person stated that they tried the exercises, and they worked. Most would state that the information is great and informative, which I agree with. I will start the exercises within the next week, and will report my progress. I did not know that I had issues with APT until I ran into an old girlfriend and she stated to me that I had shrank. I said to her at the time that people don't shrink. After that meeting, I started to think about what she said, and she was right. I always wondered why my clothes were not fitting me right. By the way, I was abut 6'2 and my ex was about 5'9. At the time of the meeting, we could see eye to eye. Days later, I measured myself to be 6'. Hopefully with these exercises. I will be able to correct my posture and restore some if not all of my lost height.

I have crippling lower back pain. The pain has cost me my job and any kind of quality of life. My pain is so great that it has somehow caused my right leg to actually become significantly shorter. Is there any way possible I can fix this at home? I lost my insurance and I don't have the hundreds of dollars my chiropractor said it would take to fix it. I am honestly running out of options. I can't stand for more than a couple minutes, I can't walk more than 15 to 20 feet. I've also gained significant weight from not being able to move. Any help would be greatly appreciated. Thank you for your time.

research, research, research n then test out what you've learnt on yourself ... you know ur body n ur history best .. eat an anti-inflammatory diet too, maybe go the whole hog like i have (for energy reasons as i have me/cfs) n go paleo-keto -at 36, i've had my first 2 periods where i wasnt reduced to hogging the bathroom, hot water bottle, the bed n eating co-codamol like it was goin outta fashion XD (ive also lost my excess weight i gained last sedentary 3 years in 3 months) so i now believe the hype that its not only anti-inflammatory, bone and joint protective/healing, but also reduces the intensity of pain (plus too many other benefits to mention -ive read peer-reviewed stuff written by scientists, not just the 'look how much weight ive lost' fashionista crap XD sarah myhill is a good place to start for the science n practical stuff or dr eric berg re the individual specifics/reasons why people doing the keto diet (someone with dementia/diabetes/heart disease/leaky gut/low thyroid etc will each take a slightly different approach due to how those each affect the body systems) hoping you've already found your 'cure' and living in less pain these last 12 months, but if not, this may help you or others

Mike D I’m sorry to hear. I have to use a lift in my left shoe, and a lower back brace. I have so much pain walking/ standing. I’m alone, and I couldn’t stand long enough to make food, so I lost over 30 lbs. to 105. My whole back is messed up. I haven’t been able to enjoy my life for 10 years now. My Chiropractor couldn’t do anything for me, but cause more pain ,for a week, after every visit. A lot of people take their good quality of life, and health for granted. I’d trade them in a heartbeat.

I have been doing quite a lot of research about fixing APT and by far this is the greatest video I have found so far. The human body is a chain, and after this video I have understood that I dont have just to do exercises and stretches to fix my APT but also my upper back syndrome and my Head forward position. I need to fix my body as a whole. Thank you very much, and greetings from Spain. Keep it up with this vids, please. Have a great day. Note: I would be really thankfull if you can explain how people with APT should properly sit in a chair. As far as i know, we should create a Posterior Pelvic Tilt embracing the abs. Correct me if Im wrong. Thanks again

I treated for LBP for over 3 years. the whole time I knew my arch was excessive. I layed on the floor my daughter could put her whole arm under my lower back without touching it. I recently (6 years later) started treating for apt and have the best improvement ever. I went through 6 physical therapists and 3 doctors who thought i was drug seeking. the pt never informed me what I was doing or why. none of them mentioned apt. just a little research and I can clearly see it's been apt all along. never anything but generic core exercises that never helped. thank you and others for your videos.
