I've had my left knee replaced 3x's for various reasons. These surgeries have caused loss of flexion and extension in my knee. The left crank arm shortener is the only reason I can still ride my recumbent trike today. The shortener is a godsend!

A crank arm shortener allows me to comfortably pedal with my messed up knee.

And, another thing RE: foot angle on the pedal itself. What difference does it make, if you pedal with your feet perpendicular to the pedal plane versus, say, 30-to-45 degrees "pigeon-toed" to the pedals? I found my knees hurt more with my feet maintained at an angle with those web straps to avoid "leg suck". Ergonomics become a lot more relevant with Age & Gravity, apparently.

It sounds like you are positioning your feet at a diagonal angle to align your feet with the straps of the pedals which makes sense why your having knee problems. For those strap pedals you will want to put your feet in at an angle through the strap and then straighten your feet to a perpendicular angle like you would for standard bike riding. This will tighten the straps and have them lay diagonally across your feet, this is to create a stronger hold on your feet to the pedals.

Just another query. Does the distance between your feet, i.e. the width of the bottom bracket plus distance to the center of the pedal platform make a difference? Like, if your feet are farther apart, do you lose some power and put more torque and strain on your knees & hips? Also, what is the performance differential to relative height of the seat to the height of the pedals? If you're sitting more above the pedals, do you lose some leg power to your crank?

For your first question. The Bottom Bracket width does make a small difference, but its not something you would really take into account due to the fact that it isn't really something you can adjust or swap out, because the size is set by the manufacturer. So without welding a new bottom bracket in you can't really change that fact and at that point its not really worth the effort for the minimal change it will make for your ride. For your second question. When it comes to pedals above or below the seat there are tons of factors involved that go into making more or less force like; angle of the backrest, angle of the bottom seat, orientation of the pedals , etc. But mainly what is involved is the proper engagement of the back muscles into a pedal stroke.
