Over 20 years ago, the IOM report “To Err Is Human” set forth a national agenda aimed to reduce medical errors and improve patient safety. The report clearly stated that:
“The problem is not bad people in health care--it is that good people are working in bad systems that need to be made safer.”
Hospitals across the country began engaging in patient safety initiatives promoting “just culture” and “HRO strategies”. These ideals recognize that punishing people without changing the system perpetuates the problem instead of solving it. Honest disclosure without fear of retribution is an important characteristic of a just culture.
Where are we today?
This session will discuss the mistakes we have learned from and how to ensure accountability is appropriate, including:
-Review history of healthcare and standards of care
-Understand current climate in healthcare
-Discuss characteristics of High Reliability Organizations (HROs) and “Just -Culture”
-Learn strategies being utilized in hospitals to improve patient safety
