Like shortness of breath, weakness etc. Something like cardiac arrest doesn’t come out of nowhere except for an arrhythmia which is notoriously void of other symptoms till you have a bad arrhythmia. You can have v tachycardia, ventricular fibrillation without the things that give noticeable symptoms. Does he have cardiomyopathy , myocarditis, NNo I’ve seen people have ventricular tachycardia from electrical problem in heart. I’ve seen fine one minute and them go out flat line. I’ve watched it. Then they Are resuscitated and fine. If it’s an electrical intrinsic problem they are put on arrhythmia medication to control the electrical system in the heart. They have testing called electrophysiology studies. It diagnoses faulty cardiac electricity. Look it up. So yes it does happen. Just cause you never heard of it. I can tell you don’t have a medical degree .
