Join me for a day in the life of a doctor in clinic! Today I'm working in the general internal medicine rapid assessment clinic and you will get to meet other members of the team - occupational therapist (Famie), pharmacist (Alice), a junior resident (Wendy) and our staff physician (Dr. Panju). We will be working together as a team to see patients who were sent from the emergency department or family doctor's office for a specialists opinion.
ALSO you will get to see what it's like when doctors talk to each other... the real medical jargon!! Let me know what you think about that haha!

Rogue Ranger she’s done and is an internal med doctor, unless she wants to do a sub specialty which can be another 1-3 years as a fellow. I think she mentioned that she wanted to do one, not sure.

I loved the little shout out to Occupational Therapy as part of the interdisciplinary team!! I feel like we’re the “Canadians” of the health care world because we’re kind of humble and agreeable with everything - often in the background, but such a pivotal role on the team 😊

With the older woman who was dizzy, can you tell us why your first instinct was to go to what medications she was on and not something like cardiac or diabetes? Thanks :)

Hi Robyn - great question! My differential was still very brought at that point (including cardiac, neurological etc.). I have an order of things I typically ask when I first meet a patient and medications is one of them... that's how it came up so quickly. But we did still order some testing to make sure it's nothing else underlying her symptoms. hope that makes sense!

If I had to guess, I would say she did that for a couple of reasons. (I'm an EMT-B not a doctor, so I could be off) 1. Older people tend to be on multiple meds just because they tend to have more medical issues, which you'd expect from a geriatric patient. So for older people, medication reactions aren't that uncommon. 2. Her symptoms could be the result of a number of things like you stated, but you'd want to start with the least invasive and cheapest possibilities first which would almost always be history, allergies, and medications. That would be my best guess for the approach to that patient.

Asking existing medications is pretty standard, especially if some drugs have the possibility of interacting with something that she's planning on giving

I'm continually impressed by how you're able to give us awesome informative content, give the viewers the basic idea of what a particular patient you're seeing is going through without giving us too much information about them. After digging into the internet I found that Canadian Healthcare abides by the Personal Information Protection and Electronic Documents Act (PIPEDA) which is similar to The Health Insurance Portability and Accountability Act (HIPPA) in the U.S. On another note: Congratulations on starting your 3rd year Medical Residency. I became a fan during the beginning of your 2nd year and immediately knew I needed to watch the backlog of videos of your 1st year and have enjoyed every minute of it.

I don't usually post comments. But, decided i would here. I was originally drawn to your videos because of believe it or not, your violin playing in the opener (which sounds good!)(my middle daughter plays)..... and since i used to be a paramedic, the medical cases you see and share with your viewers. I haven't been watching long but really like how you put the videos together and the medical content while protecting the patients privacy. I even laugh at some of the subtle edit things like the whoosh sound when you bring up the clock, etc.! I also challenge my diagnosing skills and try to figure out the patient problems from the presenting symptoms you describe, before you share the diagnosis and treatment plan...... please don't bill me for the training! Anyway, thank you for sharing the videos and putting in the extra work to edit and post these videos in addition to your tough work schedule.

For the first time ever, RU-vid recommended something to me that was a perfect fit!! Your videos are not only fun to watch but I also find them incredibly educational. I'm not a med student or anything, in fact I'm a pre-vet Major, but I can appreciate this so much. You probably have tons of medical/pre-Med students who watch and get inspired/pumped about their profession. I know for a fact that if a vet student were to make these videos it would be a life changer for me. Maybe I'll be that vet student who makes these. I'm just really inspired by you and your videos. If a doctor is undergoing a fellowship in cardiology or other subspecialty, can he or she still practice Internal medicine while on fellowship training?

Great question! Short answer is yes - after you are done writing your royal college exam for internal medicine. The exam has just recently been moved up to 3rd year now (before it was written in your 4th year) so I'm not sure if the rules will change at all. I certainly hope to do that!

I feel like your naive little sister. So excited and almost teary eyed when I heard third year medical resident. The work you do is incredible, I really like how chill everybody in the clinic was with filming. It's sort of clear some hospitals you've been at have been more open than others but you are giving us educational medical content that is in the perfect realm to be so interesting and relatable but still educational

I’ve watched you since the middle/end of your first year it’s crazy to see how far you’ve come and accomplished Even though I’m not going into the medicine field myself I’m absolutely in love your videos you’re always so positive and a great motivator for good study habits and self care, also I feel like your videos are honestly such a great resource for people who are in medical school. I really appreciated how you included that clip of the physician talking to the first year about expectations and plans for when you’d all go see the patient I thought that was so interesting to see the beginnings of how residents learn their skills and get practice. QUESTION? I am trying to look into careers in business or medical (if I take business it would be in the hospital). One thing that I do not hear often is how people who are not doctors do their jobs alongside doctors and the variety of jobs in the hospital all happening at once. OMG third year?!?! I've been watching since you were a first year medical student. Congratulations on starting year three! How does it feel to be a third year? Are you feeling more confident in what you are doing? Do you feel better equipped to take on challenging causes without feeling like you need lots of checking in with others? Hope you're getting to enjoy your summer a bit too! Congrats again on starting year three!

Oh man, 3rd year! Congrats! I really love seeing how many different people at your hospital are actually directly involved in creating a treatment plan for patients. We can definitely learn something from you here in Germany.

I want to know what happened to the patient with the high liver enzymes. I too always have high liver enzymes in my blood. Negative to hepatitis, no alcohol consumption, no Tylenol use ever. However I found out I’m allergic to yeast, and I believe it is affecting my liver. I am no mean a specialist, but I think I have auto brewery syndrome because In my lab notes at the bottom it said some chemical(don’t remember the name) that is found is alcohol but I don’t drink ever!!
