Join me and my husband, Mark, for another day in the life of a doctor. Today we're seeing patients in the hospital. You'll learn all about liver cirrhosis, the fluid that builds up in the abdomen and the procedure to remove it. Plus learn about the many conditions that can cause high blood pressure and hypertensive emergencies!

Siobhan, I was having high blood pressure at 15. Thank goodness they did all the tests that you mentioned. I was diagnosed with Chronic Kidney Disease at 15. I have been living with the disease for 5 years.

I feel for you. I was diagnosed with CKD at 27, but it was suspected when I was 22. I've now been living with it for 26 years and have had a successful (live donor) transplant. There is light at the end of the tunnel, even though it may not feel like it at times. Ruth

The kidneys regulate blood pressure so CKD can lead to blood pressure changes or fluid retention

One of my university professors (I finished 10 years ago) was from the Sault. I also remember when my grandfather had cancer (he passed away over 19 years ago), he had fluid on his belly too.

I was in the hospital last year after getting quite sick. I was in for three weeks when I was assigned a hospitalist and I was better a week after. Hospitalists are AWESOME. Thanks for busting your buns for all of us folks Mark!

Brings me back to my 5 day stay in hospital for hypertensive crisis with end organ damage -> 225/150. Considered primary but aggravated by sleep apnea. Unfortunately, I waited too long to see a doctor, so I now have permanent issues along with tons on meds to take. All I can say, is keep an eye on how your feeling, and take your pressure from time to time.

Oh, the best husband and wife doctors in the world. Thank you for the video. Specially the liver stuff. Oh my gosh I hope he’s feeling better. That happened to me when I perforated my colon they sent me to interventional radiology within two days and then they drained me and I felt so much better. Thank you both of you for saving lives. Believe it or not, I watched your videos during my clincial pastoral education chaplain residency program last year. I learned so much about the medical side that it made my clinical hours easier. I finished my chaplain residency and now I'm a hospital chaplain.

I work as a clinical research coordinator (I'm not a doctor) and one of the studies I'm working with right now is looking at plural fluid (fluid around the lungs) that is collected from patients trying to improve the accuracy of the tests done to detect signs of cancer in that fluid. I consent patients to the study and then go pick up fluid from cytology (pathology) if there is enough left after what is needed for clinical use and take it to the research lab, so I'm familiar with those little vials of fluid. My role is a part of research most people don't realize exists, sort of a bridge between the patient and the researchers, although often the doctors the patients are seeing are the researchers as well.

it's crazy that I started watching these videos years ago, just fascinated at the medical info. now I'm in med school and could follow along in depth and understand everything! Also, what great timing, I'm in the thick of my GI unit (learning liver!) and finished learning hypertension in my cardio block weeks ago!

Where were you when I was working. Oh, yeah, in Canada. Lol. I was a nurse for 39 years. Retired two years ago. Did acute dialysis for 32 of those years in the hospital days and nights. Love your upbeat attitude. Would have like running into those smiles every now and then. 😊

Thanks for clearing up the obstetrics confusion 😂 I was one of the people who asked about it and I thought I was the only one who was confused😂 I'm an intern from India and we rotate among all the departments during this year, I thought it was different in the US and canada. I had a sleep study done because I would wake up constantly due to my mouth being wide open which dried out my throat and made me cough often. Study showed I had 69 episodes per hour! They Rx me a chin strap to keep my mouth shut and an apnea machine to ensure I continue breathing. Since Sept my readings show an avg 1.5 episodes per night - a huge difference. Those episodes also resulted in my having hypertension so I'm on an ACE Inhibitor now. I never thought how sleep can affect your BP. At least I don't wake up exhausted anymore

Hello Dr. Siobhan, This is an important video of you about treating a patient with Liver Hypertentive Disease. Actually my girlfriend had this sickness of hypertension and she had high blood pressure. Finally unfortunately she died by the effect of hypertension which caused her heart stop functioning. I was sad about her Siobhan

such an interesting video! I had pretty much the same experience with hypertension back in Feb... I'm 23 years old, i started getting headaches on and off for a couple of months and i too also experienced blurry vision. After a trip to the opticians, it was found i had papillodema, all problems caused by hypertensive emergency! Turns out i've got inflammation in my kidneys which has caused the problems (unknown as to whether this is CKD yet but it's looking extremely likely) Can totally relate to the emergency in this video! I hope everything is okay with her now :)

Crazy that I don’t have high blood pressure but I tend to have a high pulse. A few months ago I was even having mild chest pains and when I checked my pulse, it was around 150 bpm. I hadn’t been exercising or anything then but it was during a very hard time with the sudden passing of my mom. On a normal day my pulse tends to be anywhere between 95-120:bpm. I do have central sleep apnea as well. When it was first discovered in the sleep study, I stopped breathing 150x an hour. The doctor told me I was lucky to be alive because he hasn’t seen such a severe case before. I was struggling for 2 years before my primary finally put in the order for a sleep study. I was always severely tired and dosed off everywhere basically. Today, thankfully I am doing better and I have been breathing more in my sleep.

Endocrinologist comment: For hypertensive emergencies - Draw one of every color tube. There are 3 main "flavors" of hypertension: Volume (too much fluid in the "pipes"), Pressor (blood vessels are squeezing down, increasing resistance), and compliance (vessels are too stiff - think pulsatile flow through steel pipes). Then, figuring it out just requires a little deferred gratification.

Good job to yoou both. They removed fluid from my stomach but it "carmelized" soon afterwards so it couldn't be tested. 10 yrs and a few Gastrointestinal and Nephrologists later, i still have no idea what it was. You both provided some great information. Thanks
