what about if there is surenal adenoma? though acth either from adenohypophysis or other tumor organs is down surrenal will continue to produce abundant of cortisol? what test we have to do?
Is fasting recommended for dogs prior to the low dose dexamethasone test? My vet had us withhold all food for 24 hours. Her test came back negative, but I am afraid it was skewed from the extra long fasting.
Hi that was a really helpful video. Just a quick question; if I used a high dose dex test and cortisol levels was still high; instead of it being a tumour in the lungs causing an increase in ACTH, could it instead be an adrenal adenoma releasing excess cortisol; I'm assuming dex would not stop the adrenal glands from producing excess cortisol if they were the issue? Thanks
Probably late to the party. My cardiologist did blood test (I have POTS with orthostatic high blood pressure) and also a blood test of my hormones. Test results came back (I will have my cardiologist visit in 3 weeks) and my ACTH and cortisol are both over maximum limit. Can this might be Cushings?
I only heard of POTS this past year (a woman with MG was misdiagnosed with panic disorder when she moved during a procedure!) I can't diagnose online - try to ask an endocrinologist!
Hey Jen, my guess is that it makes no difference because cortisol will be suppressed either way, but I would ask an endocrinologist. Please let me know if you find out!
@@AJmonics I am here because my Endo doesn't know. I was referred to him because of low acth, dhea, and tumor on the adrenal. Last year he only did a baseline and 8mg suppression tests. Said everything is good. He said he'll do 8mg suppression test every year (no baseline) for 4 years to see if the tumor is active. This year, I questioned him about my sleep cycle and he said he'd research it. From you wonderful presenters on RU-vid, I've learned that I need acth and cortisol done and 1mg, then 8mg if necessary, correct?
High ACTH caused higher than normal cortisol which by negative feedback suppressed further more increase in cortisol. Dexamethasone administration now have negative feedback which makes the reduction from high cortisol noted before the test
