You’re more helpful than my orthopedic surgeon… he said what I have, but couldn’t explain why. Even after asking… and gave me a prescription for pain and sent me off. I had to ask several times for exercises and got very little feedback. This was two weeks ago. You’ve been very helpful in explaining the whys and the fixes.

Spinal stenosis I think, in that pain is brought on by exercise and relieved by sitting (but also stretching). Pain alternates between left and right lower back but only goes down the leg from time to time on the right side. This pattern has ben going on for years (I'm 67). Is it stenosis and what exercises should I be doing? Should I avoid the stretches even if they relieve pain?

Piriformis. What are the stretches I should do?
@PT4Everybody
@PT4Everybody 3 месяца назад
While you are in an acute flare, I would avoid stretching your piriformis. This puts increased stress on the sciatic nerve...causing pain!

My surgeon recommended a laminectomy last October based on my MRI. I had 2 injections without relief. I have been doing mostly pelvic tilt glute and abs for about 6 months, with no relief. I mostly just had sciatica pain in my left calf. Now i am feeling it in my left poterior thigh, especially after sitting in car for a period of time. It is making me very nervous, as this is the 1st pain i had in 2022, when i had debilitating pain, the worst in my life!! I had a microdiscectomy to right side L5 S1 with positive results. My most recent MRI last October showed...notable progression in L3-L4 and L4-L5 spinal canal stenosis where there is now moderate-severe spinal canal stenosis and lateral recess narrowing with contact of the respective descending L4-L5 nerve roots. Please advise!! Everyone tells you something different, even with high qualifications in this field. Also of not i had an ankle replacement in April with achillies tear after lengthening procedure. I am finally off crutches, but still in boot with wedge on Right ankle. I have continued exercises on floor throughout my recovery, like i said mostly glutes and abs, isometric and such.

Good job keeping up with your exercises with all you have been through! I would really recommend finding a Manual Physical Therapist to help with your specific situation. Particularly, how is the ankle replacement affecting how you are walking (and other such things).

Still on crutches, but almost off, had to back off a bit second to ankle pain with full weight. I do have an even up on my good foot, so I am not crooked with walking, thank goodness. I live in the mountains of Colorado and we have so many PT, with DPT. It is difficult to find the right one!!??

The videos are great! Thank you so much for sharing them and providing useful information and exercises. Does sciatica only happen down the back of the leg? My pain radiates from the left side of my buttocks, and wraps around to the front of my leg, down to my knee. I have not been able to find any relief from the pain, I get slight relief when leaning over my counter, on my elbows but I can’t find a comfortable place or way to sit. I don’t even want to think about trying to sleep!! I am beside myself and wondering if I need to go to the ER. I’m just so uncomfortable I don’t want to go and sit in an ER if there isn’t anything they can even do to help.

Yes, the sciatic nerve only goes down the back of the leg but you may have another nerve that is affecting you. I am working on some videos for that, too! For now, I would recommend seeing a healthcare professional to help you get some relief form your symptoms and figure out your root cause. Personally, I recommend a Manual Physical Therapist!

i’m confused. it falls under stenosis because my pain started at both of my glutes but when i sit down the tingling starts again in both legs down to my feet.
@PT4Everybody

You may be suffering from a central stenosis (pain in both glutes) as well as having something else going on. I would really recommend you find an awesome Manual Therapist who specializes in Low Back Pain to unravel what's going on with your body.
