Important to note that carbamoyl phosphate and citrulline are synthesized in the mitochondria of liver cells. Citrulline is transported into the cytosol, the rest of the cycle occurs, and then ornithine is transported back into the mitochondria to combine with carbamoyl phosphate (and restart the cycle). Liver damage naturally results in hyperammonemia because the urea cycle takes place in the liver. Knock out the liver, knock out the urea cycle and you're left with ammonia accumulating in the blood. This is the cause of hepatic encephalopathy.
