Hi Dr. Bolten, a fellow colleague here, i’m a military dentist from the Honduran Air Force. i have been watching some of your videos and they are very informative and helpful. I love oral surgery and some people thinks that extraction of the upper wisdom teeth are so easy to perform, they’re wrong, you have to be very careful to avoid complications like this one! It’s a a very important topic to consider prior to perform oral surgery, Pt’s clinical history is very important to know what kind of medications our patients are taking on. Thanks in advance. 👏

Thanks for the question! The best way to prevent maxillary third molar displacement is to place either a retractor or a periosteal elevator posterior and superior to the tooth to prevent it from falling into the fossa. I typically use a Minnesota retractor. It can feel very tight at times, but it is important to have a backstop. Also, when elevating, it is important to direct the tooth inferiorly and not posteriorly. Hope this helps, thanks for watching!
