Do Bunion Correctors and Non-Surgical Bunion Treatment Work?

Are you afraid to go to the doctor to get your bunion checked Because you're afraid of surgery? If, so this video is for you. My name is Dr. Andrew Schneider, and I'm a podiatrist in Houston, Texas. And bunions are one of the most common findings that I see in my patients of all ages. If a bunion isn't painful, I don't often recommend surgery. There are lots of conservative treatments to try first. In today's video, I'll discuss the nonsurgical, conservative treatments for a bunion. I'll also discuss the bunion correctors and pads that you may find in the foot care aisle of your pharmacy.
I found that whenever someone has foot pain, they turn to one of two places: Google and the pharmacy foot care aisle. Both of those places can be overwhelming with both good information and lots of misinformation. Let's start with the foot care aisle. Most people ask me about bunion correctors. These are devices that pull over the great toe from the second toe and push in on the bunion. Most are designed to be worn while you sleep at night. And while these do help this stretch out the soft tissue, it's not successful in bringing the bones back into alignment. Remember a bunion is a shifting of the metatarsal bone, not a growth of bone. So no matter what Dr. Google tells you about people claiming their bunion went away without surgery. It truly just doesn't happen.
Another product that you'll find is a bunion pad. This is a silicone or foam pad that fits over the bump of the bunion itself. If your pain is on the bump of the side of the great toe joint, this could be the cushioning you need to protect it while you're wearing shoes. However, sometimes adding a pad to something that's already painful, may fill up too much space, make your shoes more narrow, and it becomes more uncomfortable. There certainly is no harm in trying though. Another product that works very well is a toe separator or spreader. This is a silicone or a foam pad that goes between your great toe and your second toe. Now, if your great toe is leaning over too much, and the two toes are touching and that's uncomfortable, this is the right pad for you.
Just a word about shoes. A bunion makes the front part of your foot wider, and most shoes are not made like that, with a wide forefoot and a narrow heel. You either have a wide shoe or a narrow shoe. You want to try to find the widest shoe that you can find that stays on your foot when you're walking and running. That's going to help to keep the pressure off of the bunion itself. So what about conservative treatments that we do in the office? Well of course I use and recommend bunion pads and toe separators when they're appropriate, but remember a bunion starts small and that's the best time for you to get it checked.
The bunion progresses because of the foot mechanics that you inherited from your parents or grandparents. And if we can change your mechanics and remove the forces that are causing the bunion deformity, we can slow or stop the progression of your bunion. I do that with something called a custom orthotic. A custom orthotic is a prescription shoe insert that's made specifically for you. To make an orthotic, I perform an extensive biomechanical exam to learn about the range of motion on your lower extremity. I'll also watch you walk to do a gait analysis and see how everything is just working together. Ultimately, we take a mold of your feet to send to the lab with a prescription. The goal of an orthotic is to compensate for the mechanics that you inherited, and neutralize the forces that are causing the bunion deformity to progress. An orthotic should also help if you're feeling pain deep in the joint.
Unfortunately, if you're feeling the pain on the bump of the bunion, I don't find that an orthotic is very effective. If the pain that you're feeling is at the bump of the bunion, it's because it's inflamed from rubbing against the shoe. In that case, we'll try an anti-inflammatory medication. Most effective is a cortisone injection. This drops the medicine right at the site of the inflammation. It works quickly and effectively. Other treatments include oral and topical anti-inflammatory medication. We'll also try using a bunion pad to help to lessen the pressure of the bump of the bunion against the inside of your shoe.

My personal experience is, that with proper walking, streching, training, footwear, gadgets like toespreaders and anatomical information & education you definitely can reverse a forming bunion, fallen and flat aches and pain. Of course it depends on the severity of the issue but it is absolutely possible to heal your feet. Definitely.

I have normally relieved the pain with massage, aspirin, and rest. But this month I had made the mistake of wearing tight shoes for a special event and now I am paying for it. Thank you for being honest about the fixer pads not working. I had purchased a pair online that promised to correct my bunion issue. Not wearing them again.

Just got orthotics that made my gait a lot better. I should have done 30 years ago. Now have big bunion on one foot due to super flat feet. I guess no way to fix without surgery and at my age, not worth getting. Good information on the reality of a bunion.

I had bunion surgery on my right foot nearly 4 years ago. Now, the big toe has some pain in it, and it’s leaning towards the second toe. What can I use to keep it straight, stop leaning and stop the pain?

I'm in my 20s, have flat feet, and I think I've got minor, symmetrical bunions forming cuz of it. (also probably caused by me sitting crossed legged all the time lol) I tried prescription orthotics a while ago but I found them way too uncomfortable. I ended up going the other way and wore lots of barefoot and minimalist shoes. Maybe I was lucky in that my biomechanics weren't so messed up that it made things worse, but after a few years I found by making my feet work harder like that actually made my arches just a little less flat, and I didn't have to buy wide shoes anymore. I'm kinda hoping the same logic will apply to my toes, like with the right spacers and stretches I can strengthen the right muscles to keep everything from going to chaos... I mostly just don't want orthotics again, ack. They're too uncomfortable O_O

I hear what you're saying. Custom orthotics should be comfortable. If they're not, then they weren't made properly. I wonder if the mold was taken in impression foam or by using a scanner. I've seen problems with orthotics made this way. Typically the toes won't straighten, since it's a physical movement of the bones. It's good to hear that you're getting better without the orthotics.

I've had one bunionectosurgery with a Secondary shortening of my 2nd toe. But it's my right bunion that's throbbing at night. Sigh. It's really hard to do surgery after experiencing it once.

I can certainly understand that. Here's the thing, bunion surgery is different from a toe surgery. Also, if it's bothering you now, it's only going to get worse. As a bunion worsens, the procedures differ and the recovery time can be lengthened. So, I hate to say it, but in this case sooner is probably better than later.

I’ve had a few orthotics. They make the pain worse ! Cant walk a few feet with an orthotic. All pain in the arch. I have super flat feet.

Hi, I don't have bunions but the last phalanx of my big toe is curved towards the left and right in each foot respectively, probably caused by the footwear, is there anything I can do to correct that?

If it's happening on both sides, chances are it's not because of shoes. There's a condition called Hallux Interphalangeus which is exactly what you describe. If it's painful, or you just don't like it, it can be corrected surgically.

@@josuevalar6465 Yes, it's usually a condition that you're born with and it develops as you grow. Flat feet are also programmed in with your natural mechanics.

I have moderate bunions in both feet, the right will be severe soon. I believe is really late to try and correct them without surgery now. My big toe has bended so much towards the other toes, it pushes them away. I have no pains though. Should I try lapiplasty? or should I try separators, orthotics, wide shoes etc etc, like, will these methods improve the toe to come back straight from bended at all???

Once your bunion reaches the point you’re describing, only surgery will correct it. Lapiplasty is a great procedure but it’s not the only surgery available. You should discuss the options with your foot surgeon.

I just scrape my bunion with a porcelain spoon (Chinese massage) for a week. Painful! But the pain was gone completely after that 😊

@@ashu5949 you can use any other non sharp wooden equipment. Any that you can manoeuvre easily without hurting your skin. Many are sold online. I find the cheapest and most effective is that spoon 🙂 Massage in one direction all over the bunion. You can feel the difference when you massage affected areas & healthy areas. You are essentially doing lymphatic draining massage to relieve the swelling. Do it as firmly as you can to get faster healing. But if you can't stand the pain, take a break. Continue until the pain disappears. Very effective. All the best! 🙏

A bunion corrector only works in the most minor of bunions. You should get your bunion checked by a foot doctor to learn what can be done to help.

I was wearing my toe separator with a lot oof hope, I have a bunion for many years now I’m 39 but it has stayed the same size and it doesn’t, hurt but yes the orthotics I’ve read about because its cause by the way you step your foot. But I’m in Colombia let’s see. My dad has a massive bunions toe is already on top on the other finger but he is not bother by it and says it doesn’t hurt. But I truly don’t want to get to his size, and I have always wear comfortable wide shoes no high heals

Unfortunately the bunion can increase in size in your later years, due to changes in mechanics. An orthotic may be helpful to slow or stop the progression. They are genetic, but it sounds like you're doing everything you can to stop them from progressing.

What about someone who cannot wear a close-toed shoe due to a condition called erythromelalgia? Wearing a closed shoe causes severe pain and burning. My feet will get bright red and hot to the touch. So an orthotic for my bunion is impossible.

Some of my patients who want to or have to wear sandals or open shoes use a work around. They secure the orthotic in the sandal with Velcro dots or double stick tape. This could be a solution for you.

But what if i dont feel any pain (yet)? I dont have a bunion but my big toe is starting to tilt (i dont even wear pointed shoes). I've bought a toe separator, but i cant wear it all day (i do sports and stuff) and when i take it off, the toe goes back to the tilting position. And i'm afraid of doctors:(

In your case, a bunion corrector would be good to wear at night. It'll help your big toe straighten out by stretching the soft tissue. Hopefully the bunion won't form, but it's good to use a device such as this as a precaution.

Sadly, it’s unlikely to become straight without surgery. My recommendation is to leave it alone unless it is painful.

That’s a different story. Lapiplasty is a surgical procedure…not a device. As far as procedures go, it’s very effective.

If a bunion is painful on the bump on the side of your foot, then surgery is really the only option. If the pain is deeperin the joint, using a custom orthotic can be helpful. Sadly, the non-surgical correctors just don't work.

@@aprillynn6221 Yes! You definitely don't need to get surgery on both feet if one is worse than the other. Focus on the foot that hurts. Then work with your doctor to prevent the other foot from worsening.

Ryka walking shoes are wide in the toe boxes, and narrow around the heels. The brand is made for women’s feet. Buy the walking shoes only, NOT the joggers. I find the joggers don’t support the front of my feet, too flexible.

I was hoping for actually statistics. Of course a surgeon believes that non-surgical interventions only delay surgery, they are not his specialism and by definition he sees people for whom they have not worked. He's extrapolating from a self-selected group. Doesn't mean he's wrong of course, I just wanted data.

I understand what you're looking for but there really are no statistics to be given. Repetitive pressure with faulty mechanics is what causes a bunion to form. The metatarsal bone rotates out and forms the bump. There are no treatments, whether a bunion corrector, orthotic, or anything I can do in the office that will rotate the metatarsal bone back into position. I wish there was. That said, I only advocate that someone should have surgery if the bunion is painful. If not, I advise against it.

That's fantastic, but uncommon. This can happen if your bunion strictly involves soft tissue. Once the bone shifts, it doesn't shift back. Hope you get new work shoes!!

How long do you need to wear orthotics for? Is there long term damage to your foot if you use orthotics?

Adults can expect to wear orthotics indefinitely. That’s because they control the faulty mechanics when they’re being worn. The mechanics reverts when they’re not. There is absolute no damage caused by orthotics. Think of them like eyeglasses…they’re supporting a structure that has naturally weakened.

My $600 custom orthotics cause plantar fasciitis, so now I have bunions, flat feet, and painful heels. I’m just gonna kill myself, i’m over all these medical scams.

It’s very unlikely that driving a car with a clutch would be the reason for a bunion developing.

Finding the proper shoe is almost impossible….my feet hurt like hell, not specifically bunion but cannot wear anything but slip ons. Closed shoe, running shoe…forget it.

Look for a running shoe brand called Altra. It has a wide forefoot and a narrower heel. I find that it works well for my patients with bunions. Hope you find relief!

it only makes sense that if wearing shoes with too narrow of a toe box for decades will cause bunions, then doing countermeasures such as wearing spacers, etc will not be an overnight cure! But 10 years of spacers and better shoewear will make a difference

I see your logic, but shoes aren't the primary cause of bunions. Even if they were, the bunion is a shifting of the metatarsal towards the inside of the foot. Wearing spacers and good shoes, even for 10 years, won't shift the bone back into place.

@@DrAndrewSchneider Women are much more predisposed to bunions and why is this? It is because they tend to wear much more restrictive shoes than men. Therefore shoes are the primary cause of bunions. Plus people are waking up to the fact that one can never trust the medical establishment when it comes to disclosing the root cause of a disease or disorder.

I am developing bunions on both feet and they sometimes hurt. The arch of my foot has also collapsed and my feet became wide. Buying proper shoes for my feet is a problem and i always feel a relieve when i am walking barefooted. I hate wearing shoes as i am scared these bunions will get worse and make my feet to look ugly😭😭

Your shoes are probably not causing your bunions. It's the mechanics of your flat feet. If the bunions are minor, consider a custom orthotic. That will support your foot and minimize the chances of the bunions growing larger.

You should try barefoot shoes, they mimic the feeling of being barefoot by being wide, havind a wide toe box and thin sole. There are plenty infos on youtube about the benefits from them
