The doctor is assuming that we all have 'health care providers' that are actually competent and available. In the UK we have a very small chance of getting a face to face appointment or any kind of useful treatment from our 'GPs'. In addition there is a huge hierarchy of skill, you never see the same doctor twice, and many of are 'newbies; who don't have historic hands on experience on the more subtle medical conditions. Relying on my HCP being able to deal with a condition like noctoria is wishful thinking. But thanks anyway.

Getting similar in USA. Sometimes months to get appointment now. Seeing nurses when it used to be a doctor. Phone appointments when you live 10 minutes away so they can see more patients and use less office staff. As far as documenting things prior to appointment, sure show them the list you spent an hour on and they look at it for 5 seconds.
