I really appreciate the question about occasional micro-burps. I have a micro-burp every 2 months maybe, but that's it. Have had chest pressure and difficulty taking deep breaths for the last year and a half or so, and even got my heart checked, but no issues found there. No conclusion about the chest pressure. Also, bloating, which is sometimes painful. The worst episodes come after I lie down after eating, and I can barely walk. Croaking in the throat is an hourly occurrence. I'm definitely considering now that I may have R-CPD, and thanks again for the video.

I have EDS and Achalasia. I have had a Heller Myotomy Dor Fund. I am making a Peterdactyl sound that can clear a room. I have all the major hallmarks. I have an appointment with a ENT next week. I hope this can be figured out.

Any correlation between R-CPD and Barrett’s Esophagus? You said that air pressure can damage your lower sphincter. So this makes sense to me. But have you had any patients with this?

I have a question!! Can RCPD cause heartburn? I believe I have heartburn but I'm wondering is it caused by my RCPD? Ty for the helpful video

A long list of GI symptoms are possible. The big 4: 1) can't burp; 2) gurgling noises; 3) bloating/distention; and 4) excessive flatulence. Additional common symptoms: nausea after eating, heartburn, painful hiccups, a feeling of shortness of breath, hypersalivation, etc.

Sir, First of all a big appreciation for your work regarding R-CPD I'm from India, I have gone to more than 10 doctors with your videos and research papers printed None of them are ready for Botox procedure All say Botox is only used for "Voice modulation or swallowing issues" Please let me know which hospital or group in India you gave seminar presentation? So I can consult them

Hello, after strugling more than 45 years with not being able to burp ( i am 61 now) i got the botox treatment yesterday in NIMES ( south of france). The doctor told me the effect will be felt 48 hours after. Hope it worked!!!

It worked for me!!! What a bless being able to burp again !! 🙏🥰🥰 Something so insignificant for most people feels extraordinary for us. For all those people suffering of this syndrome do not hesitate to undergo the botox treatment It is just magic !!!!

QUESTION: I got Botox 6 months ago and now, I can only micro burp (on demand). So I can harness and expel air but so very little. Would a second Botox intervention help at all? It seems like I have learned how to burp, but it looks like my sphincter is either too tight or too big to fully open. Also, when the Botox was active, I could burp all the time. The third month was a golden era: I burped only when I wanted to (no more burps when laughing or singing) Lastly, when a gurgle comes up, my sphincter seems to tense even more and I can’t burp at all, I have to wait until the air bubble comes down and then I can let go of a little bit of air but it’s really not satisfying… Would a second Botox injection weaken the sphincter and allow it to stay supple? Should I do SHAKER exercises instead, as physiotherapy? Thanks for answering if you do! Oh! And when I air vomit, it’s explosive!!! So definitely not due to a lack of need to burp!

Yes, most (~80%) are "once and done", some are "twice and done," a very small number are "three times and done," etc. After two very good results and complete loss of ability to burp each time, we at least mention cricopharyngeus myotomy. I would personally probably try at least 3 times... Since you can burp a bit at 6 months, it seems hopeful that another injection might give you mastery. Using bicycling as an analogy, we consider botox to be "training wheels" for burping. It is important to find the "hook" into the burp ...usually a little "fidget" or "gesture" such as head turn, chin tuck, lowering of the larynx, etc. and to increase the number of burps you would otherwise get to practice on by drinking sparkling water, in order "botox burps" into "mademoiselleparallele" burps.

@@Laryngopedia ahah! Thank you for the reply :) So I’ll put myself on the waiting list for a second intervention then (currently 6 month wait in the Netherlands) and I’ll keep practicing in the meantime (plenty of time to do so 😄) Who knows, I may crack the code with persistence!

Dear Sir, I have an issue with my vocal cords. One of my vocal cords on the right side is partially damaged due to stomach acidity reflex. However, when the cords move/vibrate the damaged one won’t latch in order to produce a clear voice. Is there a remedy or exercise to cure the impacted/damaged cord? If R-CPD doesn't seem to be bothering you (my son says it doesn't cause him discomfort- just extra gas), is it necessary to be treated? What are the downsides of not having the botox (or other treatments)?

It is a spectrum disorder. If one has R-CPD, but a limited expression one might choose to just live with it. Just one caveat: the symptoms can progress/get worse, so some live with it for a time and then at some time in the future, decide to be treated. It will be up to him.

Hey doc, I’ve had 2 injections but symptoms returned, gastroscope found a medium sized para-oesophageal hernia and was told I might need nissen fundoplication surgery which has a side affect permanent no burp, I’m thinking about just doing another injection or myotomy instead but risky living with hernia, can i get your opinion please?

Hey doctor, i used to have a lot of gurgling but lately i have some small burps on a daily basis but i also still have the gurgling noises. Does this mean i am slowly getting the ability to burp?

It is possible for a person to have micro-burps but that are not completely relieving....

I wonder how many R-CPD sufferers also ended up with a hiatal hernia as a result of the built-up pressure on the LES for years... I have a hiatal hernia, just wondering whether the two are connected.

Informally, I think hiatal hernia is a logical consequence of all the abdominal pressure upward ... and we certainly see it in our R-CPD population.

@@Laryngopedia What's fascinating is that 10 years ago when I googled my inability to burp I found nothing. I thought I was crazy. Then yesterday I stumble into the noburp subreddit and realize that since you published that paper a few years back, there's a sizable number of people with the same issue and you're pioneering procedures to remedy the problem. It's not hard to imagine how living with unnatural pressure in your entire digestive tract could lead to a whole host of issues besides just basic discomfort. I really hope more ENTs start paying attention to your work and more research gets done; seems very important to remedy this problem for those who suffer from it, especially given how simple your solution appears to be. Thanks for your work. I'm thinking of scheduling an appointment with your practice in the next few years.
