I don't have much insight, but in nursing school we had alot of exam questions like these. Where situations with patients have very ambiguous moral decisions, but we have to pick the "most correct" interpretation when all of them can apply in some way.
Also, many of my fellow collegues would be more cautious about the psych wards versus the other sections of hospital for exactly the situation that Sam was in with that first patient. Trying to help those with mental anguish can be more dangerous than other ones at times.
I don't have any formal training in the field but I will say what they are talking about is very reminiscent of my experiences talking with the kings of professionals. It passes my sniff test at the very least.
