This is the definition of a good doctor. I am not a patient of his but the fact that he’s trying his best to educate people on people on alternatives to surgeries says a lot. It says he puts his patients best interests in mind, not his own. Surgery only when non operative treatments don’t work. I don’t know if this is his goal, but I can’t think of any other reason why he would do this.

Very informative! Professional violinist here and I have an upcoming PRP procedure with Dr. Derrington for my cervical spine. Very excited. And Dr Kim, I have heard so many great things about you! Hello Dr. can arachoiditis develop just with a disc herniation? Or do they only occur post invasive procedures? Who would be the best specialist to dx this?

Hello Dr. Kim. I have a disc bulge between L4-L5. Is there a surgery for disc bulges, or is surgery only recommended when it is herniated? The pain goes away but comes back with a vengeance if I sleep on the wrong bed or in an incorrect position.

Can you treat the annular tear without using the laser. My spine surgeon does endoscopic approach but doesn't use laser

Yes you can...but it is much better if you use the laser. The laser is one of many tools we use during endoscopic spine surgery. It is very precise and does not use electrical current to do its work, and thus it is very very safe. I have given several lectures about this issue at international spine conferences. 30 years ago I studied acupressure under an acupuncturist I learned that Lots of lower back pain was caused by adrenal exhaustion .our adrenal glands are in the lower back region and the nerves from the adrenals go directly to the spine ....with lots of adrenal stress our lower will back go out bending over just to pick up a pencil . Burning the candle at both ends Over use substances that give you energy like caffeine, mate ,energy drink juggling to many things you know the typical today life style ......

I had L4/L5 microdiscectomy due to jiujitsu injury years ago. 2 years later ruptured same disc could not walk or stand or sleep from pain. Had surgery 6 months ago, Fusion L4/L5 from the back with spacer. I am 6 months post fusion. I was off all pain meds by day 3. No tylenol nothing. Sciatic pain was gone and I could walk and stand and sleep. Now 6 months later my lower back is killing me. Especially when I try and get up from a chair or sit for a long time in a car (hour or longer) laying down no pain at all. Dr. took me off all PT and I have just been walking since day 2 of surgery at least a mile a day. But my back seems to have no stability and painful when bending forward even an inch or 2... to reach out for something. My question is what could be causing this at 6 months? I have been resigned to just sitting, sleeping walking and doing nothing since everything that requires any core or spine stability just kills my back and I am bed ridden for a week later that day. The pain is always worse as the day goes on. Thanks for any advice. I am in good shape, non smoker, non drinker, 54, 6 feet, 195, always worked out until now.

There are several possibilities, such as non-union (ie. pseudarthrosis), adjacent level degeneration, symptomatic hardware, etc. I would need to review your case and all the images. @@drchollkim Thanks for the reply, Let me ask you this what would symptoms be like of a Non-Union at L4/L5 fusion? I met with a surgeon recently who wants to go back and refuse L4/L5 (said the CT shows but sort of vague) he said sometimes he sees tons of bone growth and some times none and mine is sort of in between or vague) he said he would also recommend fusing L5/S1 at the same time, as I had a lidocaine "only" non steroid block at L5/S1 hoping to confirm L5/S1 was causing my pain and based on that shot I got about 4 hours of relief.

The spine is an electrical circuit .....a short in the electrical system will weaken the back ability to support itself causing pain from pinched nerves . What causes. A short in the nervous system ....... adrenal fatigue....
