He's saying that an event that mostly happens to under 15 year old boys playing baseball and even then is rare - around 20-30 events per year, is the cause with highest likelihood.

I saw that hit and thought it was a bad hit into his chest. Seeing him stand up and stagger nearly made my heart stop. I knew exactly what was going on and hoped someone close by would know to start CPR. I've never seen it happen before, but knew a guy who got hit in the chest with a car tire and this happened to him. He didn't make it. I've read about other cases, it's rare, but it happens. Chest compressions!

Thank you Dr. Mike for your insight into Damar’s injury and recovery! So scary! Healing energy and strength to Damar and his family! My thoughts also go to the other young man who was involved in the hit and his family. I’m sure they are hurting as well. I wish comfort and peace to both families, the players and coaches.

Definitely, the Guilt on the player and his entire family is unimaginable. The fact that Damar mother's did give him a call and the father talked strongly on those negative people blaming the player was everything that family needed. It's a long road to recovery like the specialist are indicating, please hold tight ASA family and ma God help you to pull through. Concerned South African citizen.

I was at a Little League game with a friend of mine in elementary school where a five year old boy got hit hard in the chest and his heart stopped. They started CPR immediately. But he didn't make it. He died of commotio cordis. He was on a ventilator for a few days before he was declared brain dead. Even as a kid I felt so bad for his family.

A average person has about 5 min of oxygen reserves in their body after their last breath. Just doing chest compressions will circulate the remaining oxygen until help arrives. Mouth to mouth is not needed. Hope this helps someone.

This is true, however mouth to mouth is needed if the person is a victim of drowning because you don’t know the last time they were able to take a breath.

someone in my school had a cardiac arrest during basketball tryouts, they started cpr immediately but he sadly didn't make it. Wish Damar a speedy recovery!!!❤

I’ve dealt with several patients with hypoxic brain damage secondary to cardiac arrest as a Neurology NP. Greater than 5 minutes of CPR without oxygen to the brain, I worry about hypoxic encephalopathy and brain damage. Patient can also have seizures secondary to this. He will need intense physical therapy, occupational and speech therapy if he is able to be extubated. He has to be able to follow commands while intubated before he can be extubated. The positives on his side are his young age and being physically fit in terms of recovery. Always informative Doctor Mike. Best, Melissa

WE need to bring back mouth to mouth and tech it so it works. 5 min without adequate oxygen is a fail. and he was out 9 min. Placing him on his stomach to help him breath at 50% O2... has me worried. Also, information not coming from a DR. is worse than NO information. I wish the DR. would hold a press conference. IT worries me that they haven't.

All you in the icu and neuro are weird to me as an ER RN lol. Patient being mildly aware while intubated means doc didn’t order sedation yet. The thought of a patient following directions tubed weirds me out

Took care of a 34 year old man who went into cardiac arrest. We had him on ECMO and artic sun to cool him down and try to preserve his brain function. Many pressors to keep up his blood pressure. He recovered but did suffer some brain issues where we had to put him on 1 to 1 for safety. He just would not listen and fought like crazy for everything. We eventually sent him to rehab. These situations are just sad.

There have been several professional soccer players in Europe experience a cardiac arrest on the field during a match over the past several years. None showed any pre-existing conditions, despite regular, extensive cardiac monitoring. Most never play again. However, Christian Erikson was fitted with a defibrillator and returned to the field eight months after his incident. He is currently playing for Manchester United...

Seeing him collapse like that freaked me out. The same thing happened to my brother during a basketball game, but he actually passed. It was found later that he had HCM.

i read Demar also has some lung damage sooo it has me thinking if he was injured beforehand 🤔. Football players have alot of padding in the shoulder and chest area. Have seen alot take worse hits to the chest area and still walk away like nothing. So i personally believe it was not the hit but a injury that was already bad but the blow made it worse and caused his cardiac arrest???

They have it, I think it was just a freak accident. The fact that players are involved in harder hits than that regularly and this has never happened tells us something.

No, it's just timing of the hit. Unfortunately, a hit to the chest, regardless of strength of hit can result in this if the timing of this lines up. It's possibly an event called commotio cordis, where when the chest is hit with just enough force and that force happens at the exact time the heart electrical cycle is about to begin the T-wave of that electrical rhythm. That is a critical window. If the heart is hit at that exact time in the cycle it can throw the heart's cycle/ rhythm off and the heart enters an arrhythmia, the arrhythmia then leads to cardiac arrest.

CPR = 1. Head back 2. Confirm no obstructions in mouth 3. 30 Compressions, 2"deep 4. 2 Breaths 5 Reapeat #3 & #4 until pro's arrive. (This is VERY exausting, have another person ready to switch out) Damar will recover

From what I learned years ago is you have exactly 5 min to even get that heart pumping blood. You leave the heart unattended unbeaten for longer than that your chances of survival are like 1 percent

CPR has to include oxygen. as a nurse, I disagree with new CPR protocol decreasing breaths given. The blood, or circulatory system, transport nutrients and oxygen to the body, without oxygen, what' s the point? they did cpr on the field for 9 min. before ambulance arrived and took over. I saw no breaths given, via mouth to mouth, or ambu bag. until EMT arrived. Just because I didn't see it, doesn't mean they weren't doing it. I was so impressed by that Bills med staff and trainers. They were so very good.

The new CPR protocol is for bystanders, not first responders. Of course CPR + rescue breathing is ideal, but it turns out that many bystanders didn't initiate CPR because they weren't comfortable with the idea of mouth-to-mouth. They hid the incidnet pretty well, but I would suspect that they gave rescue breaths considering they have medical staff (including an anesthesiologist) on the sidelines.

I completely agree with the newer protocol. It takes at least 30 seconds of high quaility CPR to even start getting adequate oxygen to the brain and heart. Assuming you continue giving high-quality CPR to the person for the remainder of the 2 minutes compressing, you are going to be getting enough in for the situation. Pausing for breaths out in the field is usually not effective anyway and causes longer pauses in CPR. Compression fraction and compression quality are significant when it comes to successful ROSC and not only survivability but increased *quality of life* after arrest. You may be a live, but if you dont have a functioning body its not very fun.

@Victor Also, they were probably more along the lines of ACLS rather than BLS. Especially if they had an anesthesiologist on their team. Lining, intubation, and code meds were probably vital to getting him up so quickly.

I do agree, mouth to mouth is not effective unless done properly, nor are ambu bags. and putting an O2 mask on without forcing air into the lungs is no help. So what do you suggest?

When sudden cardiac arrest occurs, uncirculated oxygen remains in the bloodstream. Research has shown that doing chest compressions, without rescue breaths, can circulate that oxygen and be as effective in doing it as traditional compression/rescue breath CPR for the first few minutes.
