I keep receiving e-mails from patients asking whether to use Dutasteride or Finasteride to get the best possible outcomes and the mails are so many that I decided on doing a video instead. DHT binds more tightly to the testosterone receptor than T does. So, the increase in T from either medicine will not improve muscle mass/performance.

I've been on finasteride since 2001. Obviously, my hair didn't stay the same within those 20 + years but I wasn't losing anything substantial. Plus, it definitely kept my problem area in a satisfactory state. However, late last year, I started to notice my scalp showing more. I waited a few months to make sure I wasn't going through a more aggressive shedding period than usual. 6 months passed and my situation had not changed or improved. I was determined to not lose what I had maintained for nearly a quarter of a century so I went on a very diligent hair boost regimen. I am now on a tablet that mix the doses of minoxidil with dutasteride. In addition to my finasteride + Nutrofol. It's s made a world of difference. Yes, it's costly and you must be diligent but for me it's worth it. Lastly, I've not had any side effects even with the considerable uptake in medication.

@@ab360 in the first couple of years I regrew some of my lost hair after that it plateaued and remained fairly steady. I also became a vegetarian around the same time I started taking finasteride. As well as eating less processed foods and I started eating a lot of herbs and spices as well.

Dutasteride blocks too much DHT and likelihood of side effects much higher as compared to Finasteride. Also the drug Dutasteride has not yet been well studied. Finasteride has been with us for 40 years or so. Thanks for watching.

@@jss.2020 Topical drugsare not yet so advanced. However Polichem has released P-3074 in the European mmarket. Better still, injectable Finasteride holds a lot of promise.

Arimidex (an AROMATASE INHIBITOR) retards breakdown of testosterone, and reduces production of estrogen. It is to be taken under medical supervision locally.

Estradiol and Testosterone are anabolic. The muscle Hypertrophy channel is easily understood by most endos and laymen. Estradiol is why women can lift more often than men. The protective effects of Estradiol may be part of why women live much longer

Thanks for watching. The culprit then is your aggressive baldness. None of these drugs completely stop hair loss in androgenetic alopecia. OM can be tried under medical supervision. You will be taking 2 potent drugs and will need to be careful.

Both have different mechanism of action and when taken together complement one another. However most benefit will be from the DHT blocker you are on.@@brusli9019

i find some errors in ur words sir, but i ll only go through one or two, while fin half life is indeed 4-6 hours, dht levels dont start raising even after 24 hours or even later, so u dont need to take fin during night at all, let alone someone whos been on treatment for months as fin has built up it takes at least 10-14 days for dht do get back to baseline if not longer. So it dont matter when u take fin , thats why people even see good results when they take it every other day at dosages as little as 0.25. 2nd dutas half life is 4-5 weeks not months. and fin blocks type 3 dht as well , making one of the side effects better hearth health, since type 3 dht is considered to damage the cardiovascular tissue.

You are amazing. I took 1 mg of finasteride every day for 25 years. Then the side effects started for me. I then started taking 1 mg of finasteride only 3 times a week. I've been doing this for 3 years now. But the effect is becoming less and less. Now I only want to take finasteride 1mg twice a week and 0.5mg dutasteride once a week. Would this dose be sufficient? I turned 50 this year.

In your case most likely your baldness is stable and you can consider going off it slowly.

Thanks for your amazing breakdown of this! Has been super helpful. I had a question. I have been on Finasteride daily for 10 months but my hair is still falling rapidly. I did a DHT test and my DHT levels were below normal but haven't had any side effects. I believe my hair loss may be aggressive so am tempted to switch my 1mg daily of Finasteride to 0.5mg Dutasteride daily to see if it can stop my hair loss and get my density back. What would you advise?

Thanks. No DHT blocker wil completely stop progression of baldness. However, it will be slower. Better would be to, in consultation with your doctor, consider once a week Dutasteride to augment your bodily response to DHT. DHT levels have very little to do with androgenic alopecia. It is the sensitivity of DHT receptors which is responsible.

Hello Doctor. Thank you for your work. I've been using Finasteride 1mg daily for over 6 years with success, but have since experienced significant loss. I have now introduced 0.5 mg Dutasteride once a week in addition to 1mg Finasteride daily. Will I experience the shedding phase with the introduction of Dutasteride, even though I am only taking it once a week?

I have an occasional patient only whom I have to put on this regimen. However shedding is not permanent on starting a hair loss medication. If that has happened in your case, perhaps it is not due to the drugs.

1 mg a day is max for premenopausal; and for post-menpausal 2.5 mg day is the dosage advised. Side effects may be decreased libido in 5% , breast swelling & tenderness, headache, irregular periods, dizziness, and at times increased body hair.

Thanks! Yes I have been taking Finasteride low dosage for the past 2 years now. In some patients the effect slows down after a few years and adjunctive once a week Dutasteride always helps this small number of patients. The efficacy of Finasteride is dependant on dosage till 1.25 mg in my experience. Taking more than this dose only increases chances of acquiring it's side effects.

I took 1mg Finasteride every day for nearly a year and it did nothing for me. Still pulled out a lot of hair when washing. Switched to Dutasteride end of March this year and my hair loss stopped. I take 0.5mg every other day though for the first 30 days I was taking it every day. I also take 12.5mg CPA every 3 days and apply 2mg E2 gel daily to the scrotum(am non binary MTF). I guess for me I was a mutant when it comes to DHT. Some Endos prescribe a small dose of 25mg Bical for managing hair loss instead of Fin and or Duta.

Hi Doc. Your videos are very informative and educational! My derm prescribed the standard Finasteride 1mg/DAILY but I think that might be too much for me to begin with. I am a 45 y/o Asian male, non smoker, in good health, who's experiencing some thinning at the crown and at NW1. I really enjoy your youtube channel! However there is one question: All Papers report a reduction in serum DHT in a relative percentage way. For example 0,5mg decreases DHT by 95%. Is this really how it works? Because if somebody has genetically high testosterone, lets say 3 times higher than normal, then taking the same dose of 0,5mg should suffice. But one the other hand if in reality 0,5mg dutasteride reduces DHT by about 3 nmol/L in absolute manner and not a relative one. Then one would need to take a much higher dose of dutasteride if one has high DHT levels.

Thanks for the input. Sadly this does not work this way. The response is not dose dependant. I have been working on this but the discrepancy in clinical response is frustrating. I go by the proclivity of a person to get ED since that is the side effect that is most feared and a reality in some individuals on Propecia. I do not prescribe it if the levels of total testosterone are less than 350 ng/dL. 350-500ng/dL is given 0.5 mg Finasteride on alternate days, 500- 750 ng/dL, 1 mg on alternate days 750-1100 ng/dL and above, 1 mg a day can be given safely.

Firstly thanks for educating us so much on this subject. I just wanted to know as you mentioned dutasteride/finasteride would increase testosterone levels and would eventually lead to increase in estradiol , is this because of aromatization. If so, can that be avoided through exercise so that the higher testosterone levels gets used to stimulate muscle synthesis.

Best not to start with a strong drug. It blocks almost all DHT. DHT has body functions like maintaining muscle mass, etc.

That s enough. Do not expect quick solutions in hair loss. Minimum wait is 3 months to see if medication is effective or not.

@@asimtahir7859 They increase by 15-20%. However it is not testosterone that is known to cause hair loss- the culprit is DHT.

The testosterone surge happens in the early morning. So if you sleep late, take the medicine just before going to bed. If an early riser of the 5AM club take it first thing when you get up.

Finasteride has such a flat dose response curve there is very little to no difference between taking 1mg 1.25mg and 1.5 mg .Dutasterides half life is dose dependent it is only 4 to 5 months when you have been taking .5mg a day for more than a few weeks if you take three .5mg per week you will block about 93% 5ar2 and the half life will be considerably less.

It is DHT and not testosterone which is bad for hair and is being controlled by Dutasteride.

Been using minox for 2 years and finasteride daily for 1 year but My hair is still thinning like crazy. Have had zero side effects so I’m thinking about dutasteride now.

Does a patient’s serum DHT level need to be zero for either medication to be therapeutic? Finally, Should a patient check LH, Estrogen and Total Testosterone every 6 months when taking either medication. When do you recommend using Finasteride for 6 days a week and Dutasteride 1 day a week. Do you ever recommend taking Dutasteride twice a week instead of every three days. DHT is required for body functions which are more important than our wish for preservation of hair. 100% DHT should never be blocked. Finasteride blocks 70% and and Dutasteride over 90% only, leaving room for benefits of DHT like muscle building, cognition, etc. Lab tests are only required if dosage has to be increased or if the patient has significant side effects and the drug needs to be reduced in dosage or completely stopped. I use Finasteride .5 mg on alternate days to start with and slowly increase dosage if required, to 1 mg a day 7 days a week. I recommend stopping it for 15 days every quarter. If after some years it is seen that Finasteride is not helpful in regning in thinning/balding, I add 0.5 mg Dutasteride once a week (that day's Finasteride is omitted). Dosages need to be forever titrated depending upon response. For there is no one dose for all men, for all situations and even in the same patient during differeing timelines, the dosages may need to be scaled up or down. I have not had to use Dutasteride for more than once a week. I am also wary of a drug that blocks so much DHT!

Avodart is used for Prostate enlargement whereas Propecia is used for cosmetic indicaions. It will always be expensive.

Hello. This is nonsense. I have been taking dutasteride once every two days with no side effects for the duration of 7 months. Until I was foolish enough to up my dose to two pills every three days. I immediatly had Side effects. So I stopped taking DUT for the duration of 6 months to make sure there was no DUT in my bloodstream. I am back on the treatment at a reasonable dose of once every two days and the side effects are back. What should I do next?

1) Both are good companies. When you start Finasteride, shedding usually happens due to old thin hair being replaced with better quality hair by the hair root under the skin. 2) More than 1 mg dosage does not have any significant benefit. 3) Dutas is what my patients use. 4) Any hair regrowth medicine will cause initial shedding if it works.

Hi doctor. Thank you for the video. I've been using finasteride 1g tablets for 6 months, started 1 week after HT (3 tablets a week in the morning - which i will start now before bed like you said in video). Every evening after shower i use minoxidil foam and everytime I still have many hair fallen out which i can see on my hands (I'd say 10-15 hair). Some people say it should be like that as 'old thin hair' need to fall out to make a space for a 'new strong hair', which could make sense. Anyway... Do you think I should take finasteride 1mg every day first instead of alternate days or should I start with dutasteride once a week? Or should I wait if falling out will slow down and hopefully stops some day? Thank you so much for answer doctor.

I have been using 1mg of finasteride daily for 16 years. It has worked well until recently in stopping the progression of my hair loss, so I think I need to add something to my regimen. I will ask my dermatologist to see what he thinks about using Propecia 6 days a week and then Avodart only once a week. My other options are to increase my daily dose of finasteride to 2.5mg or 5mg. I learned recently that finasteride is dose-dependent. Do you have any patients who benefitted more from finasteride when their dose was increased?

No, Finasteride is not dose dependant. hair loss continues with use of any DHT blockers. We need to know that Finasteride blocks 70% DHT and Dutasteride 90%.Once a week Dutasteride in cyclical fashion enhances the effect of Finasteride after 2-3 years of use when in most patients hair loss becomes brisk and there is a doubt about its continued efficacy like before.

I started using 0.5mg of dutasteride once a week in December 2020, and increased to 0.5mg every day in May 2021. It was a miracle that I found a dermatologist who prescribes off-label drugs because they saved my hair. In September 2021, I started using 2.5mg of oral minoxidil every day. The results are amazing. I had severe diffuse thinning while on finasteride, but dutasteride and oral minoxidil significantly improved my density. I never thought I would be able to use a comb again, but now I do.

Every morning, I take a 0.5mg dutasteride pill, and then a half oral minoxidil pill a few seconds later. I take a half oral minoxidil pill every evening. The oral minoxidil pills are 2.5mg, which means I get 1.25mg twelve hours apart.

The peak of testosterone happens early morning. If you are a late sleeper say sleep after midnight, take the medicine before going to bed so the peak of medicine in the blood coincides with that of testosterone surger in the blood. Otherwise take it early morning.

I took finasteride 1mg per day. However, after 2.5 months I was developing cystic acne on the back of my neck. I know for sure it's due to this drug, as I never get acne in that area. I'm not exactly sure why I developed acne from this drug. Have you seen this in any patient of yours? I've since stopped the drug after 3.5 months of taking it, allowed the acne to clear slightly, and now I'm taking dutasteride 0.5mg every 3-4 days - I'm hoping this will help.

Experience does not say so. Nor do a majority of studies. However some people have good response with smaller doses for hair loss. I would not recommend a low dose for the 1st 6 months after a hair transplant though. Thanks for watching.

A few years back, I noticed that both my hairline was receding and that the crown of my hair was thinning as well. So, I tried Rogaine, but it didn't work. Afterwards, I took 1 mg of Propecia (Finasteride) for a little over a year to at least keep the hair that I had. However, I noticed that my hair was thinning even more to the point that the dermatologist couldn't explain it himself. A year ago, I had a hair transplant in which I received 3,500 grafts. Now, I still take finasteride, biotin, multivitamin, and have had PRP a few times. The issue is that my hair is not as thick as I would like it to be, especially when my hair is short. I'm a 29 year old male. In your opinion, would it help to switch from 1 mg of Propecia to 5 mg of Proscar or 0.5 mg of Avodart in order to see hair become thicker. I've read that those who take Avodart notice shedding, but for some, new hair doesn't grow back thicker. I would really appreciate if you can tell me which would be best. Thanks a lot.

Finasteride efficacy decreases in some patients due to unknown reasons. In such cases, I exhibit 0.5 mg Dutasteride once a week. On these days, Finasteride is omitted. Most people respond. However shedding will occur but all will be well after 3 months. No, the hair that grow after shedding are not thin but much thicker.

If Finasteride has stopped having an effect or if you think its benefit has slowed down, take Tab. Dutasteride 0.5 mg just once a week in place of Finasteride for the day. This is what I recommend. Blocking 95% DHT is not a good idea.

I have been on Finasteride 1mg for 3 years with great success but I feel like I am losing a bit of ground, and am thinking of adding 0.5 dutasteride a week. I had my hormones tested a while back and they said they were very nicely balanced (testosterone: 26 nmol/L, estrogen: 97 pmol/L, free testosterone: 27.2 pg/ml). Firstly, whats the likelihood I will have a big shed if I take dutasteride once a week in addition to finasteride (From what you have seen from patients in your clinic)? And secondly what are the chances of side effects (gyno / sexual) from once a week in addition to Fin 1mg from what you have seen in your patients and considering my hormone levels. Thanks for the question.Please go ahead. This is usual dosage I prescribe to my patients in whom prgression is refractory to the dosage of Finasteride you are on. You will not have shedding. You are already on Finasteride and the mechanism of action of Dutasterde is similar.

Sir, my age is 33. Due to genetic hair loss, I have been using minoxidil every night for 7 months and I have seen saw palmetto content 2 times a day. My hair loss has not decreased. It was 60-70 a day or something like 150-200 a month ago. Now it's 60-80. But after the treatment, there was an increase in volume - thickening of the hair. I didnt under control my hair loss.When my hair loss increased 2-3 months ago, my doctor told me to continue the treatment I gave you. ( every evening minoxidil and twice a day vitamin containing saw palmetto ) I have been using this treatment since 6-7 aydır. The doctor did not tell me to use dutasteride or finasteride. Dutasteride is more effective in regrowth of hair loss in the front and top areas and in general. If I start on Dutasteride first, switch to Finasnteri after gaining as much hair as possible, won't Dutasteride protect the hair I've gained? Usually after you start DHT blockers you get shedding which can be profound and distressing. It is only sometime after 3 months that you begin to see the benefits of Dutasteride, etc.

I haven’t experienced any side effects at all and was told my a doctor that if I didn’t experience anything with finasteride most likely I won’t have any side effects with dutasteride either. does cutting proscar pills 5mg into 4ths(1.25)mg reduce the efficacy of the drug? Am wondering because of many reasons which i read online including: 1- light and moisture could damage the active ingredient when split . 2- stomach's acidity could deteriorate finasteride since its not protected by the coating. 3- you might not get equal amounts of finasteride in each piece after splitting the tablet since i heard that finasteride might not be equally distributed inside the pill. Please your thoughts on this

Yes you are right and this is explained in the video. Theoretically it can lead to less efficacy but then people are taking .25 mg in excess than what is required and have good results. As long as a tablet is cut one at a time and stored in an airtight container and finished in 4 days, I do not think it should be a significant problem.

Sir why 0.5mg fin after hair transplant is not enough?is it necessary to take 1mg only because i didn't have aggressive hair loss and was maintained on minoxidil for 7 years before ht it took me so many years to progress from grade 3 to 4,so will 0 5mg do the trick or compulsory i should take 1mg?

When you take Finasteride for thinning there is a lot of room for error. 0.5 mg may work. However, after a hair transplant a lot is at stake since planted grafts need the umbrella of a DHT blocker while they grow.

I’m about to come into taking Dut 0.5 daily without any prior FIN or DUT usage…I’d rather not waste a lot of time on a FIN trail for 12-16 months just to have it potentially not even work for me as I just turned 23 and have been very slowly noticing hair thinning and hairline receding for the past 3-4 years. But now is the time. Should I begin with a daily 1mg FIN and DUT 0.5 one day a week as a start? Or would it be better to start out on DUT 0.5 alone daily and see how things go? I just don’t wanna waste a lot of time running in circles with either drug singularly if the best approach or “most positive” approach would be to just take both from the beginning

That is not the right approach. Finasteride is safer and has many more years of usage which make it safer to prescribe. Dutasteride has been used for hair loss relatively recently. It should be used after Finasteride effectivity is on the wane. Dutasteride has a higher incidence of side effects and if they do develop, your motivation to taken even Finasteride might not be too strong.

If your baldness has not stabilised, hair so maintained will all fall off.
