People don't realise just how brutally tight a tourniquet has to be in order to be truly effective. I was attacked by a knife-wielding nutcase in the Netherlands the other year and was bleeding out; the first responders put a tourniquet on me and that honestly hurt more than being stabbed and slashed up. But, it saved my life.
I work in emergency medicine and one part of our training was actually applying CATs (combat application tourniquet) to ourselves and our colleagues to learn how much pressure we actually need to put to completely block blood flow to the selected extremity (and how much that freaking hurts). It was some time ago but the pain I felt while having the tourniquet on me is still something I really vividly remember. Tourniquets save lives. Use them if needed. Make sure to write the time of application though, it's kinda important to know how long the arm/leg has been basically dead (without blood flowing). And NEVER remove the tourniquet yourself if you didn't get it on in training purposes. It needs to be removed in the OR by the surgical team to actually make sure the clots formed don't kill you.
I wrote a really long and boring post, but shortened it and then probably still made it too long. Short short: Me scout kid mental bad but deal with ouch good, friendo hurt arm with saw oopsie, tourniquet! Nono, but I say yes, adult back with nurse, yippie, tourniquet, bad adult, bad first aid kit, you kid with mental real good, okie responsibility go me, adult ask good/bad I say, okie, but later byebye scouts. Short long: I had mental issues and experience ending up in hospitals, having cuts and just reading about things that can happen to a human body. A friend cut himself in a weird place (I don't get how he got hurt there, I mean) near his elbow and the saw went really far in. We improvised a tourniquet, but a scout team leader I asked to apply an improvised tourniquet but didn't want to, so I asked him to get the nurse got back with her and she applied a real one but said that using it earlier woulda been better, but I was getting things together and partway through tying a belt around the kid's arm when they came. The kid was airlifted and the scout team leader kicked out, cause not only did he not realize a real threatening injury occurred, but our first-aid kit lacked a tourniquet among other things (like non-alcoholic disinfectant). I got a pat on the head for suggesting a tourniquet early on but also told about the pain that entails and just in general got some slightly more advanced first aid knowledge and was like the go to "do we need to go to the hospital" guy on smaller outings that didn't have a nurse till I quit.
@ET Hardcorgamer I know! I'm older now than the scout leader that didn't want to apply a tourniquet was back then and I have no idea how the guy was entrusted with kids if he wasn't ready to help a kid in that situation. I truly don't get his thinking - we all saw the amount of blood the kid was losing. Not to mention how irresponsible it is to not have a tourniquet in a first aid kit when on an outing in the middle of the forest, with a lot of sharp objects and kids that may not realize how badly they can hurt themselves. As to HEMA, it's also important to remember that those manuals were made for knights who'd be wearing plate, grabbing a sword with a gauntlet wouldn't really hurt the hand. But I agree with you completely - sometimes you've gotta do what you've gotta do to survive. It's better to lose a limb than your life.

I don’t think there’s a single one that I haven’t enjoyed tbh. For some reason tho, I do love the sheer availability of the “doctors react” type of videos. Dr Wagner and Dr Mike do a lot of those sort of videos, and media is loaded with Grey’s Anatomy/House/Scrubs/The good doctor/er/etc etc etc. I’d like to see some officers/detectives/svu/etc explain shows like criminal minds/CSI/NCIS/etc bc the assistant scoutmaster in my scout troop was a cop and he always said he wished he knew of a department with 10% of the tech they have on those shows. Also, if you’re looking for good ones, the linguist they have explaining accents/historical figures portrayed on tv/in movies, those are my favorites.
I was thinking the same thing. I never had a shoulder dislocation but I did have a knee dislocation so I dunno if they’re similar pain/trauma-wise but it was not even the same realm of pain compared to some other pretty serious injuries I’ve had (including fracturing my femur) and I was in an immobilizer for two months. So.. “uncomfortable” is just a BIT of an understatement.

When I tore my ACL in 2017, they did the surgery pretty quick after a few weeks of physical therapy, and then within 9 months I was back to full-time training and running 5ks, but when it immediately happened, I was almost numb to the pain, and that I think was just my brain keeping me from realizing the immediate pain that I was in. The aftereffect left me feeling confused, lethargic, and even at times like I was numb all over my body.
127 hours is still an unbelievable story. The fact that he managed to cut his arm off and then survive is just mind blowing. I'd rather die under that rock than attempt this lol
@@crazynutreviews2647 It's the fact that in some interviews he's noted his biggest pieces of advice when going hiking are 'let someone know your exact route, and when you plan to be back' and 'don't just pack a cheap swiss army knife, because they make cutting off your own limbs even worse'. It takes a lot to give someone a life-or-death take of knife choice, but I guess that's one of the cases.
You forgot to take into account that the scene with Wolverine. He regenerates blood, skin, muscle tissue etc. So for him to reach under the rib cage to remove something from his heart is not going to be an issue he will heal in no time. It was easier than breaking his own ribs to access his heart
If they were going to give her a clip frim Wolverine they should have given her some context. Otherwise the scene is just absurd. Probably not a good idea to use super-powered people for a "how real is it" clip anyway. 😅
17:35 Uncomfortable is an understatement. I have Ehlers Danlos Syndrome, so I get frequent dislocations and usually reseat the joint without assistance. I've learned what works and what doesn't. But not so much in the first few minutes, but after a qtr hour or so of a joint being out of socket it starts to become really nauseating. While you're numb and in shock it's quite easy to deal with, but once the endorphins fade it's a bad place to be. It took me years to learn not to over reach and stretch my shoulders too far, just because you can doesn't mean you should lol. In my experience though, dislocations hurt considerably more than broken bones. After dislocating my wrist and fracturing my scaphoid it was the dislocation that caused 95% of the pain, the day after once some of the swelling had subsided, I managed to pull my thumb enough to reseat everything else in the wrist where it should be and the pain receded to a background murmur. Breaks ache, dislocations are much more viscerally painful. As goes the clip, when I reseat my own shoulder I lift the affected arm onto a table with my other hand, then squat down so the arm is lifted above my head, and it usually shlups back into place. It tends to stretch everything out though so for a few months it'll be prone to coming out at a sneeze. After 3-6 months it tightens up and stays in socket unless I'm stupid and reach fast without thinking. I've cracked the cartilage in the cup of the joint though, so I don't know if that's why it goes in so easily from above for me. If I can't find something to raise the arm, then I use the other hand to try and pull it over my head like I'm a ballet dancer, but it's a bit more hit and miss. I had to do it like that in the deep end of a swimming pool once and I panicked and floundered quite a bit and got a mouthful of water before I managed to get it back in and swim to the edge of the pool. Disclocations suck, and I don't go swimming much anymore as I don't like breaststroke but it's the only kind of swimming that doesn't throw out one or other of my shoulders.
For the John Wick one, I'm surprised she didn't comment on the low light. What is it with films and shows nowadays where everything indoors has to be unrealistically dark? Especially in a situation like this where you'd want all the light you can get. And what little light there is is shining on the wrong side of the patient.
agree. but, it’s a movie/show. i used to think the same until i dabbled in photography and had to learn about lighting and it actually plays a pretty big part in how you’re trying to convey a scene and set a type of mood for the shot. i’m sure there videos on youtube that explain scenes like this when it comes to lighting and when you hear why it’s lit like that and watch the scene agin you’re like “ok, i get it” and realize that realistic lighting would change the whole mood of the shot.
@@terahsavaya3688 I partly agree, the thing is, it just not realistic, and sometimes more realism is just better, plus in this case, light for surgeries are very bright and cold, and could have worked very well. It feels just unnatural and unsettling for a lot of people. So I think they could have made a better job with the light in this case.
The amount of light you see isn’t necessarily the amount they showed to be seeing in movies. Just like when they supposedly can’t see anything but you can.
I can't believe they didn't include Paul Bettany yanking a bullet out of his abdomen in "Master and Commander: the Far Side of the World"! The scene where Stephen uses a mirror to operate on himself is fantastically done.
Yes!!!! That scene always has me fascinated...because back in that time frame, as the movie shows when poor Lil Will looses his arm, they bite down on the sticks because they had nothing to numb the pain.
I like that she did no country for old men, as a former EMT I really loved that scene. And in 127 hours in real life he really did use a small multi tool knife that wasn’t sharp. I’ve never had my Shoulder dislocated, thank goodness.
Aw....I was hoping she'd rate Mark Watney's(Matt Damon) self-surgery in "The Martian." That scene was intense. Also, John Rambo's(Sylvester Stallone) field surgery in "First Blood" would've been interesting.
ill rate matt damons one. It's all feasible. He should've cut the clothes around the pole before pulling it out in case it hit some vessel, in that location may hit inferior epigastric artery if deep enough. He should use some antiseptic wash given there's clearly medical equipment. Angle of mirror needs to be better to see properly, although you could try do it by feel, sometimes we do these by feel. He should have washed the wound before closing it with staples. I'm surprised the staples hurt as it looked like the thing he injected at the start was probably local anaesthetic. So 7/10, missed a few steps but overall OK.
To be fair his injury was sustained during that martian dust storm right? If it did, that injury wouldn't have realistically happened. Dust storms on Mars are immense and massive but quite honestly it's like a weak wind storm here on earth because the gravity on Mars is so low as is its atmospheric density. The reason they look so extreme is because the whole planet is covered in nothing but dust and when those storms start that wind just keeps going, even if by earth standards is not very powerful wind.

Loved this! It would have been cool, if she had reviewed the scene in the Outlander pilot episode, where Claire puts Jamie’s shoulder back into place. It would have been a good comparison to the Buffy scene.
My grandfather always said that the first thing to do after having any kind of wound or burn was to just run it under clean water... Glad to hear such a simple thing is still considered so safe.
I would love to see her rate the scene in "Shooter" when he buys all his supplies at the store and fixes his wounds in the bathroom. And the scene from "A hitmans bodyguard" where Samuel Jackson extracts a bullet from his leg. 😁😁 Please good sir! Might we get another?? 😄🤩
A lot of these type of review shows always comment that in most cases removing the bullet isn't all that realistic unless it is causing an issue. The important thing is reduce bleeding. There are lot of people with bullets and other objects walking around with them still in them. Digging around for and pulling a bullet could cause more damage than just leaving it where it is. Not to mention without pain killers it would really painful.
As a fresh civilian medic, this was nice to watch. The TQ, we had to have them applied to use during the program so we truly understand how tight it has to be, the first few mins, it's a very solid 9/10 pain but after awhile, you stop feeling anything.
In the casino royals scene, it could be said that what he actually used was black pepper with water and that is known to cause nausea/vomiting on an empty stomach because it lowers the stomach pH even further and therefore induces retching under the right circumstances.
@@abasdarhon Black pepper is naturally acidic outside the body and, as you sarcastically imply, it does produce an alkalinizing effect inside the body once consumed…but only in small amounts…when you take an entire flask of black pepper, it overwhelms the alkalinizing properties of the pepper, trending towards acidity again and promotes stomach upset/nausea. At any rate, I could be wrong cause, last I checked, I’m just a random guy. So save the sarcastic quips for another forum.
If he grabbed pepper, it isn't shown, and the glass he drinks has no visible dark stuff in it. I think Bond was just trying anything he could think of to quickly get rid of any remaining poison.
@@freddiei.2696 First of all, black pepper (piper nigrum), is highly alkaline, not acidic. Introducing black pepper alone with or without water, whether a small amount or a flaskful, into the stomach, will absolutely not increase the acidity of the stomach; that is impossible. Now, what could happen (I can't claim certainty on this since I've not experimented with it), is that the reaction of the strongly acidic stomach juices with the strongly basic black pepper could yield a product that irritates the stomach and help induce vomiting, but this would have nothing to do with "making the stomach more acidic". For instance, every first-year chem student knows that mixing HCl (hydrochloric acid) with NaOH (sodium hydroxide) yields water and salt...as well as a LOT of heat; I've personally had more than one test tube shatter on me from repeated heat stress while demonstrating this. Secondly, I don't have a particular bone to pick with you, but after so many times of trying to explain to people how drinking lemon juice will not alkalinize your body, I tend to get irritated when encountering similar misinformation. I apologize if you felt personally attacked by my admittedly unnecessary sarcasm.

Good point about self stitching, it’s possible with out local aesthetic. I had to do it once on my hand, the difficult part is getting the needle up through the bottom of the skin to complete a loop.
This lady is great! Her presentation style is PERFECT! She is so beautiful. It seems to me that her beauty comes from her soul; and from her soul came her skill, knowledge and wisdom which altogether led her into medicine, and her subsequent mastery of the art of the application of surgical medicine. If I was rushed into and ER, my life hanging by a thread, I would want this doctor to be working on me.
Honestly, I can never think of a dislocated shoulder without thinking of Martin Riggs in Lethal Weapon slamming his shoulder into the wall to push it back in. It was done to show how "crazy" he was and but later became a tough guy thing, like in the scene with Faith.
She missed out on the "Master and Commander" scene from which the doctor was accidental shot by the royal marine officer in the abdomen. Since the doctor was the only medically qualified person on board he had to carry out surgery upon himself to remove the metallic ball shot.
I loved that part especially when his mate said the piece of shirt on the ball would patch up quite nicely lol and he was also performing his own surgery using a mirror so everything had to been backwards
@@zoidbergler2058 the clothpatches being carried into the wound by the ball were a huuuuge risk for infection. Most wounded during that area died due to infection and less because of the actual injury. And you could consider yourself "lucky" if an infected wound did just cost you an arm or leg.

I am onclined to agree. 1/10? Given hos healing factor and his built up tolerances for pain, he did survive those nuclear burns in the movie, I would have given it a 1/10 if it was a regular human but I would have bumped it up to at least a 5/10.

The circumstances are of the fantasy-kind, as, not only does he have a adamantium infused skeleton, but his body also re-generates (well, partly due to the , er ... thing he has on his heart). Upon that he is used to excruciating pain which would make others pass out. I can't expect any doctor to make any comment about it. She made the best of it and just trashed the scene.
Excellent video and guest, aside from the Wolverine segment. Wolverine's bones are coated with adamantium, so going through his rib cage / sternum to access his heart isn't possible. He had no other choice than to go below it and reach up, and his mutant healing factor is the reason he's able to survive or stay conscious. That said, she's right, it IS totally unrealistic.
Context! Wolverine was able to do his own surgery because as soon as he removed the invasive robot from his heart, his injuries would heal in seconds. The tiny robot was preventing his healing factor from functioning.

Joke’s on the doctor, Wolverine’s rib cage is also made of adamantium (all his bones are), so he couldn’t cut through the diaphragm. Plus, he has a healing factor so he could survive, but cutting through the abdomen to the heart was his only way lmao 😂

In all fairness, John Wick was running out of time before he was officially excommunicated to get help from the doctor, which is why he ends up having to stitch up the rest of the wound himself. He's on the run, hitmen are after him, so obviously the scene is gonna be rushed and sloppy because of that.
To be fair, you cant go through the ribs on wolverine because they're indestructable, so he had to cut though the squishy parts and would then just heal up once he got the thing that was killing him
As a veteran and having to have CLS multiple times and regular tourniquet practice and you have to tighten it with all your strength and that person will yell, scream, and sometimes hit you it hurts that bad especially on the thigh because you’re trying to constrict someone’s thigh to the point there not pumping out blood. It truly takes all your strength for a tourniquet to truly be effective on a friend, family, victim, soldier etc. I’m sure most medics can truly say that tourniquets need to be done with the right procedures if not you can hurt the victim more than saving them.
My younger brother had extreme anger issues growing up and one day i was telling him instructions my dad gave us while he was at work for chores and he flies into a screaming rage and punches the window. He cut his wrist and artery and i panicked at first because hes spraying blood everywhere but i just grab him, hold him down to get his attention to hold the wound long enough for me to turn a tourniquet. I literally grabbed a wooden dowl and a towel, tie it off then tightened it down with the dowl as im on the phone with 911 and here i call dad to tell him what happened. At first he thought i was too panicked to do anything not realizing until paramedics got there i actually did a really good job and saved my brother. I was 13 at the time.
As a security guard (side job to support being a musician aha) I had to do the stitch by by myself at 5:25... thankfully not where an artery was, just a patch on my forearm after being stabbed in a liquor shop robbery gone wrong. No time to get to the emergency department at hospital... and I could've been waiting 2 hours or more. So I got stitches from a family friend and did a running stitch myself. Yes, I've had professional medical training, more than just first aid. So a 3/10 score she gave him is a little harsh from my perspective. I think a 5 would've been fair. I do concede that Mr Wick was stitching up a wound near an artery and using a mirror. I didn't do that. It was one stitch that I used though, as I was losing a lot of blood by the second....
Re: Die Hard and removing the glass - he's in the bathroom after getting away from bad guys! Where is he going to get tweezers? Being so literal about Logan doing self-surgery and how long it's going to take a person to heal? It's Wolverine, he has instant self-healing abilities.
I once had a surgery done in hospital on my pelvis. En they forgot to burn down a tiny bloodvessel. After a few hours home I got a bad swelling and was rushed to the hospital by my parents. I was out of concious a few times. When they opened up the swellig they said I lost 4 liter of blood. I was really lucky to get out of there with no damage at all. They just burned that vessel and stiched the old wound with 40 stiches and took me for observation for a few hours until my blood was at the normal pressure. The point the emergency docter sliced al the stiches of the surgery down to get to the wound all the 4 liter of almost clocked blood came out and all went over him. He never saw that coming.
Two things with Wolverine/Logan going up under the ribs: 1) virtually indestructible adamantium skeleton makes it a whole lot harder to go with a more direct route. 2) mutation (superpowers lol) makes it so he can basically magically heal from any damage that doesn't outright kill him (not sure if he even can really be killed at that point in time)
I believe (please correct me if i'm wrong) that 127 hours was based off a true story. The guy got his arm trapped, had tp amputate it himself, and managed to survive! The strength that humans can have is amazing.
On the 2009 Chilean earthquake all the windows were broken and ended up with god knows how many shards in my feet. Training was saying "don't pull them out" but my instincts overcame it.
I have had to do that too and I made a post commenting about it. I stepped on a broken bottle. Technically you should leave something in to reduce bleeding but I had to remove it so I could walk and drive to the hospital. It wasn't real deep either. I pulled a few pieces off, washed it off wrapped it up and went to the hospital. I probably should have called an ambulance but I am a bit of a moron lol.
Lol did anyone tell her who Wolverine was before they showed her the scene? No need to worry about fixing damage later when your body automatically heals itself in a few moments. She's absolutely right in her analysis by the way, but didn't factor in that he was a nearly immortal mutant. Also he used a giant metal blade that popped out of his knuckle to cut himself. The whole scene was preposterous. Seriously though great job, Doc.
Well technically he didn't have his healing factor there, that was the point of opening his chest cavity and stopping his heart, to stop the bot that was somehow dampening his healing factor. Which by the way the movie never explained in a decent manner

@@KingNerdius The woman specifically says "given (the character's) circumstances" regarding other characters than Wolverine. If she was consistent, either she would not take the character's circumstances into account in any of the examples or she would take into account the circumstances of all of the characters. It's okay if she doesn't know about Wolverine; that version of reality is not the end of the world you appear to be avoiding by justifying it.
8:05 Then again, Wolverine's healing factor normally means that he can pretty much regenerate his entire body from a few drops of blood, but here the spider robot around his heart is somehow inhibiting it. So his priority is just to get that thing out, no matter what collateral damage he'd inflict on himself in the process.
I popped out my shoulder once making a diving catch. Easily the second most painful thing I ever felt. As I was writhing on the ground, the rolling action popped it back in, which was the most relieving thing I ever felt. What I learned after is one you dislocate something, it’s more likely to get dislocated (in full or partially) in the future. I’ve subluxed that shoulder a few times since. (Most painful thing I dealt with was the morning after ankle surgery. My pain meds completely wore off. We had to call my poor doctor at 6am to get stronger stuff prescribed to get it under control again.)
I am not a medical professional. The following is just my opinion: My pet peeve with do-it-yourself medical procedures is the idea that you MUST remove all bullets. It's very risky and usually unnecessary as you can usually live with quite a few bullets permanently inside your body. No, you will not get lead poisoning. If you can't get medical care, you might try cleaning/sterilizing the wound as best you can, but don't do surgery except possibly surface stitching with a sterile needle and maybe something like fine fishing line (sterilized). Again this is something you should only consider if you can't get medical care reasonably soon. Note that even surgeons in an actual hospital with access to everything they need to safely remove bullets sometimes deliberately leave bullets inside someone if retrieving them is more trouble than it's worth. This is after making sure there's not a dangerous situation like a sharp edge of a fragmented bullet pressing near an artery or a major nerve or something.
She clearly doesn't know how Wolverine is a comic book character that has self healing ability and an adamantium/vibranium skeleton. He had to go thru the diaphragm bcos he couldn't cut thru his ribcage.

My only criticism is that for Wolverine, she isn't taking into account his....extreme supernatural regeneration factor. This replaces blood, heals injuries miraculously pulling the skin together and/or healing the wounds together. Its the reason he is able to remain conscious, cut into his own abdomen, touch his heart. The "healing factor" (I believe it is called in comic-book-land), if its powerful enough, will also restart his heart with and/or without the otherwise necessary shocks. The other character's she is commenting on are normal humans, albeit barely-believable extreme characters such as James Bond and John Wick, but still. Wolverine is another matter entirely: He has supernatural powers/abilities......
Doesn't make sense to critique Wolverine though because he's not a normal human being with normal human bodily functions. He's a mutant whose body and healing ability are exponential to that of a normal person.

Fascinating, even though watching all of this back to back made me very uncomfortable. Though... if I had to chose between cutting off my hand and taking out a tooth with a skate, I'd have to sit down and think for a bit.
I tend to do my own minor surgeries when needed. I always use running stitches because I can do them with one hand, or I just cauterize. I've also used a dremel to grind off one of my toenails because it had gotten fungus under it. I also had to use my dremel to cut part of my finger nail off when I slammed the center of it into the bone so I could extract it. My pain tolerance is just ridiculously high at this point, But I still don't know if I could cut my own arm off.. That is one of those situations where you can think and ponder all you want, but you will never know what you would do until it happens to you.
@Sarafina Summers I did that to my ankle once too. I didn't realize it was out of place (I just thought it was taking a while to heal right) but a month later I kicked a punching bag at the gym and it pulled my ankle apart and put it back were it was supposed to go. Funny how you can get so used to the pain you hardly notice and how good it feels when its just gone. Ouch on the spine, I apparently cracked at some point but that could have been a lot of different times... I'm not gentle with myself. Good to see I'm not the only one who doesn't fall apart when life gets rough. Stay Awesome :-)
7:44 I think what you forget is that Wolverine has a healing factor. His body will heal itself, so he doesn't have to be careful with performing surgery on himself.
this poor lady :( was doing sooo good. someone should have told her the reason Wolvie couldnt cut through his sturnum to reach his heart is because he has unbreakable adamantine laced bones so Logan HAD to go through the diaphram. she had it on the rest ov it i think. i blame disney for taking the bteam avengers and launching them to simony
