This song really impacts me. Maybe it is because I can relate to the story. I don't know. Great to find this great live video of it. I hope to see Everlast some day.

So true, I watched the official video & around 2:26 you can see the picture in the semi. It's literally on the place where normal cars have their rear view mirror. But there's actually no rear view mirror at all, like you said.
I think what he means by the rear view mirror is that he is looking back at the life he used to have and thinking about her and wanting to go back to her
