Why do experts believe this? In this video, I explain their point of view with the help of the topographical model of MS.

Fantastic! I’ve been a fan of BartsMS since before my formal diagnosis and I knew I wanted an IRT right away. I was offered Cladribine but wanted to swing harder and so went abroad for HSCT in Mexico as my first treatment. Your website, Q&As, and diagrams of the various HSCT protocols are a great resource and I’ve shared it with many others who have asked about HSCT (including my doctors who put the protocol diagram in their notes 😄). Thanks again and I hope this channel and your website continue to grow!

Some people will have a benign MS disease course & would not require a DMT. Should these people take these harsh treatments & which could cause them other health problems. There is hope that Neurofilament light chain can distinguish who is going to have a benign MS disease course. I wonder if drug companies are giving these neurologists money to promote these theories so everybody will get lemtrada.

Indeed, the million-dollar question is who to treat powerfully and who to treat more mildly. The number of people with a genuinely benign disease course that does not need benefit from a DMT is tiny. NFL (neurofilament light chain) might indeed give some more insight. Due to NFL's variability, I don't think that a decision can be made on that alone. By combining information from lumbar puncture, MRI scans, NFL, patient characteristics (sex, age, etc.), neurological examination, patient history, etc., a trained machine learning system might help in the future.

What would a first attack or a CIS be? Would one be able to know if they've had the first attack?

A CIS is a first attack suggestive of MS. This will be diagnosed by a neurologists. It means that many signs point to MS (in most cases including an MRI-scan indicating a typical MS lesion), however even though it looks like MS, at that time there's not enough evidence to fulfill the 'McDonald criteria' for an MS diagnosis.

This is an incomplete view. I strongly believe the immune system is involved all the way through a person with MS lifetime, and the concept of smouldering MS invalidates the idea that DMTs can “cure” MS by avoiding future lesions. The key actions to take are to control the immune system and cause it to stop attacking the myelin and to grow and preserve the functional reserve. DMTs should be used to stabilise the disease. To maintain and grow the functional reserve: diet, exercise, meditation. A person with MS should aim to get their immune system under control and implement a wellness routine to promote continuous repair/rebuild/growth of their central nervous system.

Thanks for the comment. A lot is still unknown about this smouldering part of MS. At the moment it is mostly being brought as that part of MS that remains after you stop new lesions being formed on MRI. One important thing to note is that a lesion (a focal damage in the brain tissue) in the model does not necessarily have to be apparent on MRI. If you use a 7Tesla MRI with a sensitive scan protocol you can visualize a lot more damage. That is not to say that there is no neuro-degenerative process happening in the white matter (normal appearing white matter). However, the question is how much of this process is influenced by early use of effective medication stopping lesions from ever forming. To answer this question very early and highly effective treatment (I didn’t come up with that term) needs to be used. To study this the ATTACK-MS trial is interesting. Where they treat MS as early as possible with natalizumab vs regular treatment. I do agree with you that diet, exercise, meditation are important. Thanks again for your comment, it is a very interesting discussion and hopefully they come up with a solution for smouldering MS.

If your browser is set to 'English' as default language RU-vid shows you the English title. When it's set to Dutch or when are logged in with a Dutch RU-vid account it will show the Dutch version. (You can't imagine how long that took me to find out ;) )
